Safety analyses of drilling operations are often written from the perspectives of regulation, economics, industry structure, etc. The ergonomic perspective on safety emphasizes that equipment and operations should be designed in light of human capabilities and limitations. To demonstrate this approach a scenario analysis was performed on records for 134 safety incidents on oilwell drilling rigs. The characteristics of the most critical scenarios were then considered to determine the extent to which the ergonomics of environment, equipment, and work methods might have contributed. Ergonomic data was collected at four drilling sites, including a prototype semi-automated rig. From both ergonomic and safety perspectives, the work situation of operators on a conventional rig floor is most in need of remediation. Mechanical pipe handling would provide the most complete solution to this unpleasant and unsafe environment, its strenuous and over-extending tasks, and the risks inherent in putting people near heavy moving objects. Significant improvements can be made at the detail level and at minimum cost in some tasks. Improvement in other tasks requires basic conceptual changes in rig systems and architecture. To realize their potential, new rig concepts must be carefully and systematically designed, and ergonomics should be considered throughout their design.

Subsea equipment such as the drilling riser and the subsea Blow-Out Preventer (BOP) are mandatory in traditional systems used in deep sea drilling for ocean floor research and petroleum wellbore construction. The drilling riser is the vertical steel pipe that transfers and guides the drill column and attached drilling bit into a wellbore at the sea bottom. The BOP is used to protect the wellbore against uncontrolled well pressures during the offshore drilling operation. Presently, there is a high level of drilling activity worldwide and in particular in deeper and ultra-deeper waters. This shift in depth necessitates not only faster drilling systems but drilling rigs upgraded with a capacity to drill in the deep water. In this scenario, two general drilling systems are today considered as alternatives: the traditional system with the subsea BOP and the alternate system with the surface BOP. In the present paper, the two systems are initially described in detail, and a numerical simulation in time domain to estimate the system behavior is presented. Simulations of a floating drilling rig coupled with the subsea and surface BOP in waves and current are carried out for a comparison between the two methods. Results are shown for riser and BOP displacements. Critical riser issues for the systems are discussed, comparing results from both drilling system calculations. Conclusions are addressed showing advantages and disadvantages of each drilling system, and indicating how to correct the problems detected on each system.

Összefoglaló. Bevezetés és célkitűzés: A szerzők a posztoperatív fájdalom és a sebgyógyulás tekintetében prospektív vizsgálattal hasonlították össze gyermekeken (67 fő, 1–12 év) a hagyományos hidegeszközzel történő extracapsularis tonsillectomiát (23 fő) a microdebriderrel (23 fő) és a coblatorral (21 fő) végzett intracapsularis tonsillotomiával. Módszer: A vizsgálatok a betegek által kitöltött kérdőívek, valamint prospektív klinikai adatgyűjtés alapján történtek. Eredmények: Az intracapsularis tonsillotomia gyógyulási idejét 50%-kal rövidebbnek találtuk, és az első 13 napban szignifikánsan kevesebb fájdalommal és fájdalomcsillapító igénnyel járt, mint az extracapsularis tonsillectomia eseteiben. A tonsillotomiás csoporton belül egyedül a posztoperatív első napi fájdalom tekintetében észleltünk szignifikáns különbséget a két különböző módszer között a coblator javára (p<0,05). A vizsgálatokat retrospektív áttekintéssel is kiegészítettük, 4 évi gyermek- (1–15 éves) tonsillaműtéten átesett beteganyagunk (1487 fő) eredményeinek feldolgozásával. Tonsillectomia (1253 fő) után 7,7%-os utóvérzési arányt észleltünk, műtéti vérzéscsillapításra 1,3%-ban volt szükség. Tonsillotomia esetén (234 fő) 0,43%-os utóvérzési arányt regisztráltunk. Ebben a csoportban vérzés miatt nem, de 2 esetben ismételt obstrukciót okozó hypertrophia, 1 esetben góctünetek miatt reoperációt végeztünk (1,28%). Következtetés: Eredményeiket a szerzők a nemzetközi ajánlások tükrében elemezték. Az intracapsularis tonsillotomia kisebb fájdalommal, kisebb vérzéssel és kisebb megterheléssel jár. A közösségbe való aktív visszatérés akár egy hét után lehetséges a tonsillectomiára jellemző 3 héttel szemben, mindez jelentős szocioökonómiai előnyökkel járhat. Orv Hetil. 2020; 161(45): 1920–1926. Summary. Introduction and objective: Examining operated children in this prostective study inditerscompared (67 pts, 1–12 yrs) the extracapsular tonsillectomy with conventional cold-knife (23 pts) to extracapsular tonsillotomy with microdebrider (23 pts) and coblator (21 pts) for postoperative pain and wound-healing disorders. Method: The study was based on patient-completed questionnaires as well as prospective clinical data collection. Results: The recovery time of intracapsular tonsillotomy was found less than 50%, with less pain than in the cases of extracapsular tonsillectomy. Postoperative pain was significantly less in the tonsillototomy group than the tonsillectomy group. Within the tonsillotomy group, a significant difference was observed between the two different methods in favor of the coblator for only the postoperative first-day pain. The studies were supplemented with a retrospective review by processing the 4 yrs results of their pediatric (1–15-yrs) patients who underwent tonsillectomy (1487 pts). After tonsillectomy (1253 pts), a postoperative bleeding rate of 7.7% was observed, and surgical hemostasis was required in 1.3%. In the case of tonsillotomy (234 pts), a postoperative bleeding rate of 0.43% was recorded. In this group, reoperation was not performed due to bleeding, whereas it was neccesary in 2 cases due to hypertrophy causing repeated obstruction, in 1 case due by virtue of focal symptomes (1.28%). Conclusion: Our results were analyzed on the basis of international recommendations. Intracapsular tonsillotomy is associated with less pain, less bleeding, and less strain. Active return to the community is possible after up to a week compared to the 3 weeks typical of tonsillectomy, all of which can have significant socioeconomic benefits. Background. The article provides an overview of existing complexes (units) for continuous monitoring of drilling fluid parameters in automatic mode. Aim. To justify the need to develop a complex (module) that will allow combining existing technologies and making a step forward in the field of process automation in terms of monitoring the parameters of drilling fluids. Materials and methods. In the current realities of well construction, the control of drilling fluid parameters on almost all drilling rigs operating on the territory of Russia (possibly with the exception of a few off shore projects) is carried out by the work of a solution engineer, usually a representative of a service company. The analysis of the parameters, depending on the number of personnel, the speed of penetration, the complexity or importance of the well, can be carried out from 2 to 6 times a day [1, 2]. This means a complete analysis, rather than monitoring the density and conditional viscosity, which can be measured by a representative of the drilling crew, for rapid response, and with greater frequency. Due to such a low measurement discreteness, there is a high probability of a significant deviation of the drilling fluid parameters from the design values. As a result, the probability of various complications, both geological and technological, increase significantly. Results. During the analysis of information from open sources, the most promising complexes (modules) from the point of view of application in the current conditions were identified, their positive and negative sides were evaluated. As a result of the conducted review of open sources, the most promising complexes (modules) in terms of application in the current conditions are identified, the positive and negative sides of the systems under consideration are displayed, and the need to develop a complex (module) that will combine all the best that is available today and make a qualitative step forward in the field of “peopleless” technologies used during drilling wells in terms of monitoring the parameters of drilling fluids is justified. Conclusions. The necessity of developing a complex (module) for automating processes in terms of monitoring the parameters of drilling fluids is justified.
