Abstract Current models of the determinants of health risk framing public health problems and solutions in ways that obscure the role that the private sector, in particular large transnational companies, play in shaping population health. This presentation will propose a new conceptual model of the commercial determinants of health which recognises the commercial sector's direct, indirect, upstream and downstream influences on health. It will also present emerging evidence-based taxonomies that draw together evidence on the key corporate practices which stymie the implementation of effective public health policies. In so doing, the presentation will explore how we move from understanding to addressing the commercial determinants of health.

This article aims to discuss the concepts of Social Determination of Health and Social Determinants of Health, by establishing a comparison between each of their guiding perspectives and investigating their implications on the development of health policies and health actions. We propose a historical and conceptual reflection, highlighting the Theory on the Social Production of Health, followed by a debate on the concepts, with a comparative approach among them.

SummaryThe aim of this study was to explore citizens' expectations about local health policies in order to create an adequate strategy. Local authorities in many countries place health in the core of their strategic planning aiming to create and improve a health-supporting social and physical environment, as they usually have primary responsibility for planning and/or delivering many of the services that are crucial to addressing the social determinants of health. Local health policies are created in consultation with local health services, NGOs and citizens. The survey was conducted among 400 citizens of Pleven Municipality. Of these, 335 (83.75%) responded to the invitation. Information on health problems and determinants of health was collected through a direct inquiry survey. The majority of citizens considered the impact of environmental, educational, cultural and economic factors as well as public security on their health. They expected that more purposeful efforts of the local authorities be directed for improving living conditions and inequalities in health. The citizens are willing to work together with municipalities to create health policies, which reduce negative factors and increase positive ones. They are aware that local policies and decisions have a very significant impact on the economic, social and environmental factors affecting health and well-being and therefore municipalities play a key role in improving and protecting the health of their citizens.

Resumo O objetivo do artigo é explorar e sistematizar o conhecimento a respeito dos determinantes estruturais da saúde, por meio da revisão de escopo nas bases de dados: Web of Science, Cinahl, Scopus, Lilacs e PubMed e nas revistas: International Journal of Epidemiology, Journal of Epidemiology and Community Health, American Journal of Public Health e American Journal of Epidemiology no período de 2005 a 2018, pelos termos de busca: Social Determinants of Health, Health Services, Health Policies, e Inequity. Como resultado, foram identificados 1.164 artigos e selecionados 19. Os determinantes estruturais, também chamados marcadores sociais, foram: raça, gênero, identidade de gênero, migração e classe social. As perspectivas teóricas direta ou indiretamente assumiram a saúde como direito. As políticas de saúde propostas tiveram como foco o impacto positivo na equidade através de funções de planejamento; capacitação de prestadores de serviços; redução de barreiras ao acesso e participação dos grupos excluídos. Assim, a literatura científica reforça que todo indivíduo deve ter a capacidade de alcançar seu estado de saúde ideal sem qualquer distinção de raça; cor da pele; religião; idioma; nacionalidade; recursos socioeconômicos; gênero; orientação sexual; identidade de gênero; incapacidade física, mental ou emocional; ou qualquer outra característica ligada à discriminação ou à exclusão de oportunidades sociais e políticas.
