IntroductionNeonatal mortality is an urgent policy priority to improve global population health and reduce health inequality. As health systems in Kenya and elsewhere seek to tackle increased neonatal mortality by improving the quality of care, one option is to train and employ neonatal healthcare assistants (NHCAs) to support professional nurses by taking up low-skill tasks.MethodsMonte-Carlo simulation was performed to estimate the potential impact of introducing NHCAs in neonatal nursing care in four public hospitals in Nairobi on effectively treated newborns and staff costs over a period of 10 years. The simulation was informed by data from 3 workshops with >10 stakeholders each, hospital records and scientific literature. Two univariate sensitivity analyses were performed to further address uncertainty.ResultsStakeholders perceived that 49% of a nurse full-time equivalent could be safely delegated to NHCAs in standard care, 31% in intermediate care and 20% in intensive care. A skill-mix with nurses and NHCAs would require ~2.6 billionKenyan Shillings (KES) (US$26 million) to provide quality care to 58% of all newborns in need (ie, current level of coverage in Nairobi) over a period of 10 years. This skill-mix configuration would require ~6 billion KES (US$61 million) to provide quality of care to almost all newborns in need over 10 years.ConclusionChanging skill-mix in hospital care by introducing NHCAs may be an affordable way to reduce neonatal mortality in low/middle-income countries. This option should be considered in ongoing policy discussions and supported by further evidence.

ABSTRACTObjective: to analyze of peripheral venous catheters fixation in premature newborns by nursing team at the High Risk Nursery of a public hospital in the interior of Bahia. Method: this is a descriptive-exploratory study from qualitative approach performed with sixteen professional nursing staff at the High Risk Nursery of a public hospital in the interior of Bahia from August to September 2009. Data were collected through semi-structured interviews and photographic records of peripheral venous access, and analyzed through content analysis of Bardin and semiological analysis of images, respectively. This study was approved by the Ethics Committee of the Faculty of Science and Technology, protocol number 01278-2009. Results: the results showed that the peripheral venous catheters fixation is achieved by inconsistent criteria and without scientific evidence. There is no standard to guide the professionals to carry out this practice. Conclusion: We suggest the elaboration of a care protocol to underpin these professionals and systematize this practice, based on quality and excellence of care. Conclusion: this study demonstrated the need for reform of practice and thus may contribute to the implementation of actions aimed at the attainment of nursing care with a focus on patient safety. Método: estudo do tipo descritivo, exploratório e qualitativo, realizado no Berçário de Alto Risco de um hospital público do interior da Bahia, no período de agosto a setembro de 2009, com dezesseis membros da equipe de enfermagem, após aprovação pelo Comitê de Ética em Pesquisa da Faculdade de Tecnologia e Ciências, pelo parecer no 01.278-2009. Os dados foram coletados de entrevistas semi-estruturadas e registros fotográficos de acessos venosos periféricos, sendo analisados através da Análise de Conteúdo de Bardin e Análise Semiológica de Imagens, respectivamente. Resultados: evidenciou-se que a fixação dos acessos venosos periféricos é realizada através de critérios inconsistentes e sem evidências científicas, não havendo padronização que norteiem os profissionais a realizarem-na. Sugerimos a elaboração de um protocolo assistencial que embase estes profissionais e sistematize esta prática, pautada na qualidade e excelência do cuidado. Conclusão: a realização deste estudo demonstrou a necessidade da reformulação da prática e assim, pode contribuir para a implementação de ações que visem à realização de uma assistência de enfermagem com enfoque na segurança do paciente. Método: este es un estudio descriptivo, exploratorio y cualitativo, que se celebró en un vivero de Alto Riesgo de un hospital público del interior de Bahía, en el período de agosto a septiembre de 2009, dieciséis miembros del personal de enfermería. Fue aprobado por el Comité de Ética de la Facultad de Tecnología y Ciencia, en el dictamen 01278-2009. Los datos fueron obtenidos de entrevistas semi-estructurada y registros fotográficos de una vía venosa periférica y de análisis de contenido de Bardin y el análisis semiológico de las imágenes, respectivamente. Resultados: los resultados mostraron que el establecimiento de una vía venosa periférica se realiza por criterios incoherentes y sin evidencia científica, no existe un estándar para guiar a los profesionales para llevar a cabo esta práctica. Se sugiere la elaboración de un protocolo de atención que dan soporte a los profesionales y sistematizar esta práctica, basada en la calidad y la excelencia de la atención. Conclusión: este estudio demostró la necesidad de una reforma de la práctica y por lo tanto puede contribuir a la aplicación de acciones encaminadas a la consecución de los cuidados de enfermería con un enfoque en la seguridad del paciente. ABSTRACTThe hospital discharge preparation for the newborn is of fundamental importance to reduce the expectations of relatives on the care when he/she  returns to  home. The orientations seeking to the discharge should begin from the admission. The objective was to analyze the family preparation for hospital discharge of newborn in a Neonatal Intensive Treatment Unit and to verify the family knowledge as for the disease and to the cares with newborn in the home. It is a descriptive study, of a qualitative nature, accomplished in a neonatal intensive treatment unit at Fortaleza, from December/2003 to june/2004. It was used as technique for collecting data, a semi-structured interview and the direct observation. The data were organized through content analysis. We verified that the families were partially prepared to assume cares with Rn without supervision, manifesting difficulties to accomplish some activities. We identified that the family expectation is to have the new integral home free from risks, as parents looking for to live the maternity and paternity fully until then postponed. Parents have revealed some knowledge on the disease and the child's treatment. We verified need to improve the communication process between group and family in the preparation for hospital discharge. As orientações visando à alta devem começar desde a admissão. Objetivou-se analisar o preparo da família para alta hospitalar do recém-nascido em uma Unidade de Tratamento Intensivo Neonatal e verificar o conhecimento familiar quanto à doença e aos cuidados com o recém-nascido Rn no domicílio. Trata-se de um estudo descritivo, de natureza qualitativa, realizado em uma Unidade de Tratamento Intensivo Neonatal, em Fortaleza, no período de dezembro de 2003 a junho de 2004. Utilizou-se como técnica de coleta de dados a entrevista semi-estruturada e a observação direta. Os dados foram organizados por meio de análise de conteúdo. Constatou-se que as famílias estavam parcialmente preparadas para assumir cuidados com o recém-nascido sem supervisão, manifestando, dificuldades para realizar algumas atividades. Identificou-se que a expectativa familiar é ter o novo integrante em casa livre de riscos, enquanto os pais buscam vivenciar plenamente a maternidade e paternidade até então adiados. Os pais revelaram possuir algum conhecimento sobre a doença e o tratamento da criança. Verificou-se a necessidade de aperfeiçoar o processo de comunicação entre equipe e família no preparo para alta. El objetivo fue analizar la preparación de la familia para alta hospitalaria del recién-nacido en una Unidad de Tratamiento Intensivo Neonatal y verificar el conocimiento familiar sobre la enfermedad y los cuidados con el recién-nacido en  domicilio. Se trata de un estudio descriptivo, de carácter cualitativo, realizada en una unidad de tratamiento intensivo neonatal en Fortaleza, en el periodo de diciembre de 2003 a junio de 2004. Se utilizó como técnica de colecta de datos la entrevista semi-estructurada y la observación directa. Los datos fueron organizados por medio del análisis de contenido. Constatamos que las familias estaban parcialmente preparadas para asumir cuidados con el recién-nacido sin supervisión, manifestando, dificultades para realizar algunas actividades. Se identificó que la expectativa familiar es tener al nuevo integrante en casa libre de riesgos, ya que los padres buscan a vivir  plenamente la maternidad y la paternidad hasta entonces pospuestas. algunos padres han puesto de manifiesto algunos conocimientos sobre la enfermedad y el tratamiento del niño. Verificamos la necesidad de mejorar el proceso de comunicación entre equipo y familia en la preparación para el alta. Neonatal mortality currently accounts for 45% of all child mortality in Kenya, standing at 22 per 1000 live births. Access to basic but high quality inpatient neonatal services for small and sick newborns will be key in reducing neonatal mortality. Neonatal inpatient care is reliant on nursing care, yet explicit nursing standards for such care do not currently exist in Kenya. We reviewed the Nursing Council of Kenya ‘Manual of Clinical Procedures’ to identify tasks relevant for the care of inpatient neonates. An expert advisory group comprising major stakeholders, policy-makers, trainers, and frontline health-workers was invited to a workshop with the purpose of defining tasks for which nurses are responsible and the minimum standard with which these tasks should be delivered to inpatient neonates in Kenyan hospitals. Despite differences in opinions at the beginning of the process, consensus was reached on the minimum standards of neonatal nursing. The key outcome was a comprehensive list and grouping of neonatal nursing task and the minimum frequency with which these tasks should be performed. Second, a simple categorisation of neonatal patients based on care needs was agreed. In addition, acceptable forms of task sharing with other cadres and the patient’s family for the neonatal nursing tasks were agreed and described. The process was found to be acceptable to policy-makers and practitioners, who recognised the value of standards in neonatal nursing to improve the quality of neonatal inpatient care. Such standards could form the basis for audit and quality evaluation.
