<p><strong>Objective: </strong>To investigate the association between migraine and hypertension in the Northern Manhattan Study (NOMAS), a multiethnic community-based sample. <strong></strong></p><p><strong>Design: </strong>Cross-sectional cohort study. </p><p><strong>Participants:</strong>1338 NOMAS participants (mean age 68.1±9.6 years, 37% male, 15% non-Hispanic White, 19% non-Hispanic Black, 67% Hispanic). </p><p><strong>Setting: </strong>Northern Manhattan community. <strong></strong></p><p><strong>Intervention: </strong>Participants were assessed for migraine symptoms using a self-report questionnaire based on criteria from the International Classification of Headache Disorders. Hypertension was defined as blood pressure ≥140/90 mm Hg, the patient’s self-reported hypertension, or use of anti-hypertensive medications. Duration (≤9 years vs &gt;9 years) and control (BP&lt;140/90) of hypertension were examined. We estimated the association between hypertension and migraine (overall and with/without aura) using logistic regression, adjusting for sociodemographic and vascular risk factors. </p><p><strong>Results: </strong>The majority of participants (80%) had no migraine, 6% had migraine with aura, and 15% had migraine without aura. Hypertension was present in 76% of the study population (7% had controlled hypertension ≤9 years duration, 5% controlled hypertension &gt;9 years duration, 41% uncontrolled hypertension ≤9 years duration, 23% uncontrolled hypertension &gt;9 years duration). Hypertension was associated with migraine (OR: 1.76, 95% CI: 1.21-2.54), both with and without aura. This association was particularly apparent for those with uncontrolled and long duration hypertension. </p><p><strong>Conclusion: </strong>Hypertension, particularly uncontrolled and of long duration, is associated with migraine, both with and without aura, in a predominantly Hispanic community-based cohort. <em>Ethn Dis. </em>2016;26(3):323-330; doi:10.18865/ed.26.3.323 </p>

Background: Despite hypertension being a common condition among patients attending primary health care (PHC) clinics, blood pressure (BP) control is often poor. Greater insight into patient-related factors that influence the control of hypertension will assist in the development of an intervention to address the issues identified.Aim: The aim of the study was to assess patient-related variables associated with hypertension control among patients attending a peri-urban PHC clinic.Setting: The setting for this study was a peri-urban PHC clinic in KwaZulu-Natal.Method: This was an observational, descriptive and cross-sectional study with 348 patients selected over a 1-month period. A validated questionnaire was used to collect data on patients’ hypertension knowledge and self-reported adherence, and BP recordings from their medical record were recorded to ascertain control.Results: Of the 348 participants, only 49% had good BP control and 44% (152/348) had concurrent diabetes mellitus. The majority of patients had moderate levels of knowledge on hypertension and exhibited moderate adherence. There was a significant relationship between knowledge and reported adherence, between reported adherence and control, but not between reported knowledge and control.Conclusion: Despite over 90% of the study population having moderate knowledge, and 62% with moderate reported adherence, BP was well controlled in only less than 50% of the study population. These findings suggest a need to emphasise adherence and explore new ways of approaching adherence.

AbstractBackgroundThe empirical basis for a quantitative assessment of the disease burden imposed by long-COVID is currently scant. We aimed to assess the disease burden caused by long-COVID in Japan.MethodsWe conducted a cross sectional self-report questionnaire survey. The questionnaire was mailed to 530 eligible patients, who were recovered from acute COVID-19 in April 2021. Answers were classified into two groups; participants who have no symptom and those who have any ongoing symptoms that lasted longer than four weeks at the time of the survey. We compared health-related quality of life scores estimated by the EQ-5D-3L questionnaire between these two groups after adjusting basic characteristics of the participants by propensity score matching.Results349 participants reported no symptoms and 108 reported any symptoms at the time of the survey. The participants who reported any symptoms showed a lower value on a Visual Analogue Scale (median 70 [IQR 60-80]) and on the EQ-5D-3L (median 0.81 [IQR 0.77-1.0]) than those reporting no symptoms (median 85 [IQR 75-90] and 1.0 [IQR 1.0-1.0], respectively). After adjusting for background characteristics, these trends did not change substantially (Visual Analog Scale: median 70 [IQR 60-80] vs 80 [IQR 77-90], EQ-5D-3L: median 0.81 [IQR 0.76-1.0] vs 1.0 [IQR 1.0-1.0]).ConclusionsDue to their long duration, long-COVID symptoms represent a substantial disease burden expressed in impact on health-related quality of life.Trial registrationNot applicable.

Abstract Background: The coronavirus disease 2019 (COVID-19) is caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), and it is the most important of public health problem worldwide. A critical element in tackling the COVID-19 crisis is personal behavioral change; however, there is a paucity of evidence about peoples’ awareness towards COVID-19 pandemic. Hence, the aim of this was to assess the community’s awareness status towards COVID-19 in Sheka, Southwestern Ethiopia.Methods: Community-based cross-sectional study was conducted 10th to 21th April 2020. Four hundred-nineteen individuals participated during the study. Both univariable and multivariable logistic regression analyses were used to identify significant factors of awareness status, and variables with p-value<0.05 indicated it significantly influences the awareness status of the individuals towards COVID-19.Results: According to the study’s results, nearly half (49.4 %) of the participants had awareness about COVID-19 pandemic. Factors such being female equal (AOR: 0.476, 95%CI: 0.293, 0.772),education being secondary and above(AOR:2.178,95%CI:1.323,3.586),and prevention methods being fully used (AOR,8.127,95%CI: 1.273, 51.871)were significantly influencing the awareness status of the community towards COVID-19 pandemic than their counterparts.Conclusion: The study showed that there is a low awareness status among people towards COVID-19.Hence, programs to improve awareness towards COVID-19 pandemic is essential to prevent and control the pandemic.

Abstract Background The burden of non-communicable diseases (NCDs) is rising rapidly in middle-income countries (MICs), where NCDs are often undiagnosed, untreated and uncontrolled. How comorbidity impacts diagnosis, treatment, and control of NCDs is an emerging area of research inquiry and have important clinical implications as highlighted in the recent National Institute for Health and Care Excellence guidelines for treating patients suffering from multiple NCDs. This is the first study to examine the association between increasing numbers of comorbidities with being undiagnosed, untreated, and uncontrolled for NCDs, in 6 large MICs. Methods Cross-sectional analysis of the World Health Organisation Study of Global Ageing and Adult Health (WHO SAGE) Wave 1 (2007–10), which consisted of adults aged ≥18 years from 6 populous MICs, including China, Ghana, India, Mexico, Russia and South Africa (overall n = 41, 557). Results A higher number of comorbidities was associated with better odds of diagnosis for hypertension, angina, and arthritis, and higher odds of having treatment for hypertension and angina. However, more comorbidities were associated with increased odds of uncontrolled hypertension, angina, arthritis, and asthma. Comorbidity with concordant conditions was associated with improved diagnosis and treatment of hypertension and angina. Conclusion Patients with more comorbidities have better diagnosis of chronic conditions, but this does not translate into better management and control of these conditions. Patients with multiple NCDs are high users of health services and are at an increased risk of adverse health outcomes. Hence, improving their access to care is a priority for healthcare systems.

Background Hypertension is a major public health challenge in the world. Developing countries share the major burden of it. Unlike thought before, hypertension is increasing among poor, young and women. Methods It was a cross sectional study carried out in The Hypertension Screening and Awareness Program conducted in the shopping malls at the heart of Kathmandu on the occasion of World Hypertension Day 2012 by Shahid Gangalal National Heart Center, Kathmandu. The blood pressure was measured according to World Heart Organization’s guideline by registered nurses. There were total 486 participants with the mean age of 29.51±10.77years. Majority of the participants were male (67.1%). Most of the participants were less or equal to 40 years (84.6%). Mean systolic blood pressure was 112.3±14.69mmHg and mean diastolic blood pressure was 75.09±11.89 mmHg. Average mean blood pressure was 87.41±12.28mmHg in the study population. Results Despite majority of participants were at young age, prevalence of hypertension was 23.7%, pre-hypertension was 30% and 46.3% had ideal blood pressure according to JNC-7 Classification. Prevalence of hypertension was higher among male (30.39% vs. 10%). Almost two third of the hypertensive subjects were less or equal to 40 years (64.35%). Awareness, treatment and control of hypertension were 39.13%, 18.26% and 7.38% respectively. Conclusion In conclusion, Hypertension is an emerging public health challenge in urban areas of Nepal. Most importantly, it may be increasing among young population. Moreover, awareness, treatment and control rates are not satisfactory. Background: Mosquito are one of the well known causing many diseases like Malaria, Dengue, Chikungunya, Lymphatic Filariasis, Japanese Encephalitis. They are more prevalent in rural area due to various social factors and lack of knowledge. The objective of this study was to assess knowledge of people and their practices regarding mosquito borne diseases in rural areas of Kancheepuram district, Tamilnadu. Materials and Methods: A community based cross sectional study was conducted among the residents of Pooncheri, Kancheepuram district of Tamilnadu, between the period of August 2017 to September 2017 among 124 houses selected. After obtaining informed consent from the participants they were interviewed using a pretested semi structured questionnaire regarding their knowledge and practices on mosquito borne diseases. Results: The study reported that in case of any symptoms suspected to be of mosquito born disease majority of the study population 82.2% reported consulting doctors.  The most preferred mode of prevention of mosquito bite was usage of spray (33.9%) in the breeding place followed by usage of electric repellents (31.4%). Conclusion: The study indicated that majority of the study population were unaware bout the prevention practices despite knowing about these diseases. Thus, arises the need for effective propagation in the rural area about prevention strategies of mosquito born disease through mass education and social media.

Background: Worldwide obesity prevalence has nearly doubled since 1980. Current guidelines recommend that “overweight” and “obese” individuals lose weight through engaging in lifestyle modification involving diet, exercise and other behaviour changes. It is necessary to prevent obesity so that the subsequent complications can be prevented.Methods: This community based cross sectional study was conducted in the field practice area of tertiary medical college from June 2019 to August 2019. Totally 154 participants enrolled in the study. Only those individuals who had body mass index (BMI) of more than 23 kg/m2 were included in the study.Results: Majority of the study subjects 57.1% (88) belonged to the age group of 21-40 years. We included only overweight and obese adults in the present study. In this study, 61% (94) of the study population belonged to obese- I category while 33.8% (52) belonged to obese- II and only 5.2% (3) of study population were overweight. Only 43% (66) of the study population had adopted some measures to reduce their weight. Out of these majority 69.7% (46) preferred walking while 12.1% (8) preferred jogging for reducing weight. Mean days of walking adopted was 3.7 days and mean minutes per day was 29.67 minutes.Conclusions: As seen in this study more than half of the study population had not taken any measures for weight reduction. Increased health awareness will improve their level of active participation in the weight reduction program such as diet modification and physical exercise promotes weight loss and weight maintenance.

Objective: Free or outreach clinics offer students the opportunity to work with diverse patient populations. The objective of this study was to describe the demographics and clinical characteristics of a sample of chiropractic patients at a free community-based clinic to assess clinical and educational opportunities for students to work with diverse populations, collaborate with other professions and practice health promotion through patient education. Methods: This was a prospective, descriptive cross-sectional study conducted over 2 months. Data on demographics, health status, and health risks were collected from patients and their interns. Results: Of the 158 patients, 50.6% were women and 50.6% African-American, while only 20.9% were employed full-time. Of the 24.7% tobacco users, 48.7% expressed interest in cessation. Of 80.0% overweight or obese patients, 48.8% expressed interest in weight loss. By self-report, 16.5% were diabetic, 10.1% took hypertension medication, 36.7% used prescription pain medication (9.4% opiate use), 33.5% used nonprescription pain medication, and 9.4% were under the care of a mental health professional. Conclusion: This patient population is demographically diverse. A high proportion of patients who used tobacco, or were overweight or obese expressed interest in information on those topics. A substantial proportion reported being under care with a mental health professional. This clinic provides opportunities for students to work with diverse populations, collaborate with other professions, and practice health promotion.
