Tongue tie also known as Ankyloglossia is congenital disorder characterized by short, thick frenulum which causes reduced mobility of tongue it reduces possible extension such as protrusion and elevation of tongue. It is caused by short frenum or genioglossus muscle. It is seen in various age group. It causes difficulty in feeding, dental hygiene maintenance and speech. Abstract: After labioschizis, palatoschizis is the most frequently encountered congenital disorder with multifactorial causes, and is a disorder that can be found in every nation in the world. Among the factors, the genetic factor was agreed upon to be a major factor. This was a descriptive retrospective study. This study aimed to find out palatoschizis cases in the Division of Plastic Surgery, Prof. Dr. R.D. Kandou Hospital, Manado, from January to December 2011. The results showed that there were 40 cases of both disordersd in the Division of Plastic Surgery Prof Dr. R.D. Kandou Manado Hospital from January 2011 through December 2011, showing 55% with palatoschizis and 45% with labiopalatoschizis. From the total cases, 53% were females in contrast to 47% males. Fifty-seven percent represented the most frequently treated age group of 1-4 years with 72.5% requiring palatoplasty. From the total number of palatoschizis cases, as many as 58% were caused by environmental factors. Fifty-two percent were unilateral palatoschizis, in comparison to 2.5% bilateral. Conclusion: In the Division of Plastic Surgery, Prof. Dr. R.D. Kandou Hospital, Manado, from January to December 2011 most of the palatoschizis and labiopalatoschizis cases were unilateral. Environmental factors were the mot common etiology for most of the women who suffered from this disease. Palatoplasty procedures were most widely used among the largest age group of 1-4 years. Abstrak: Palatoskisis merupakan kelainan kongenital yang paling sering dijumpai setelah labioskisis dengan penyebab multifaktorial, dan dijumpai pada setiap bangsa di dunia. Diantara faktor-faktor penyebabnya, faktor genetika disepakati menjadi faktor utama. Palatoskisis lebih banyak ditemukan pada perempuan. Palatoskisis unilateral kiri lebih sering dari pada yang kanan. Penelitian ini bersifat retrospektif deskriptif untuk mengetahui tentang kejadian palatoskisis di Bagian Ilmu Bedah Plastik RSUP Prof. Dr. R.D. Kandou Manado periode Januari-Desember 2011. Hasil penelitian memperlihatkan jumlah kasus di Bagian Bedah Plastik RSUP Prof. Dr. R.D. Kandou Manado periode Januari-Desember 2011 sebanyak 40 kasus dengan palatoskisis (55%) dan labiopalatoskisis (45%), lebih sering pada perempuan (53%) dibandingkan laki-laki (47%). Kelompok usia yang tersering ditangani 1-4 tahun (57%) dan jenis operasi yang digunakan yaitu palatoplasty (72,5%). Faktor penyebab yang tersering ialah faktor lingkungan (58%). Kasus palatoskisis unilateral sebanyak 52,5% sedangkan yang bilateral 2,5%. Simpulan: Di Bagian Ilmu Bedah Plastik RSUP Prof. Dr. R.D. Kandou Manado periode Januari-Desember 2011 sebagian besar kasus palatoskisis dan labiopalatoskisis ditemukan unilateral dengan lokasi defek terbanyak di bagian kiri. Jenis kelamin perempuan lebih sering ditemukan dan etiologi tersering yaitu faktor lingkungan. Kelompok usia yang tersering ditangani yaitu 1-4 tahun dan tindakan yang tersering digunakan ialah palatoplasty. Kata kunci: palatoskisis, labiopalatoskisis.

A rapid and accurate diagnosis of venous thromboembolism (VTE) in the elderly individuals represents a dilemma due to nonspecific clinical presentation, confusing laboratory results, and the hazards of radiological examination in this age-group. d-Dimer test is used mainly in combination with non-high clinical pretest probability (PTP) to exclude VTE. d-Dimer testing retains its sensitivity, however, its specificity decreases in the elderly individuals. Raising the cutoff level improves the specificity of the d-dimer test without compromising its sensitivity. The current study aimed to explore the reliability of higher d-dimer cutoff values for the diagnosis of asymptomatic VTE in a population of bedridden hospitalized elderly patients with non-high clinical PTP. This retrospective study included 252 bedridden hospitalized elderly patients (>65 years) who were admitted to the Ain shams University Specialized Hospital with non-high clinical probability and developed later reduced mobility; all underwent quantitation of d-dimer and Doppler examination. Considering the whole population (>65 years), the age-adjusted cutoff achieved the best performance in comparison with the conventional and receiver operating characteristic (ROC)–derived cutoffs. When stratified according to age, the age-adjusted cutoff showed the best performance in the age-group 65-70 and comparable performance with the ROC-derived cutoff in the age-group 71-80, however, its sensitivity compromised in those older than 80 years. In conclusion, it is recommended to use age-adjusted cutoff value of d-dimer together with the clinical probability score in elderly individuals (65-80 years).

Degenerative pathology of the musculoskeletal system is one of the main causes of reduced mobility in patients of an older age group. The purpose of the research: to study the morphological features of the elements of the synovial environment of the knee joint against the background of decompensated osteoarthritis (OA). Signs of adaptation of the articular cartilage of the condyles of the femur in the form of restructuring of the cartilage tissue were revealed. The processes of replacing the subchondral bone with connective tissue followed by sclerosing, which subsequently led to decompensation of the structures of hyaline cartilage, were also revealed. Destructive-dystrophic processes in the menisci of the knee joint were noted. Collateral ligaments in patients with OA show signs of adaptation in the form of an expansion of the endotendineum interlayers between bundles of collagen fibers and an increase in the diameter of blood vessels.

In Acute Lymphocytic Leukemia motility of lymphocytes is associated with dissemination of malignancy and establishment of metastatic foci. Normal and leukemic lymphocytes in circulation reach solid tissues where due to in adequate perfusion some cells get trapped among tissue spaces. Although normal lymphocytes reenter into circulation leukemic lymphocytes are thought to remain entrapped owing to reduced mobility and form secondary metastasis. Cell surface, transmembrane interactions, cytoskeleton and level of cell differentiation are implicated in lymphocyte mobility. An attempt has been made to correlate ultrastructural information with quantitative data obtained by Laser Doppler Velocimetry (LDV). TEM of normal & leukemic lymphocytes revealed heterogeneity in cell populations ranging from well differentiated (Fig. 1) to poorly differentiated cells (Fig. 2). Unlike other cells, surface extensions in differentiated lymphocytes appear to originate by extrusion of large vesicles in to extra cellular space (Fig. 3). This results in persistent unevenness on lymphocyte surface which occurs due to a phenomenon different from that producing surface extensions in other cells.
