Malignant cardiac tumors are rare but dangerous tumors with a high mortality rate, not represented at the population level in any statistical collection. The study of its prevalence is possible only using the system of population cancer registries created in the world. Moreover, not at the regional level, only at the level of the federal district or the entire country. In 1993 we created the first in Russia Population-based Cancer Registry. In February 2019, it was possible to combine cancer registries of all 11 administrative territories of the North-West of Russia, with a population of more than 13 million inhabitants and a database of more than 1 million 100 thousand patients. It was only after the creation of the population cancer registry of the Northwestern Federal District that it became possible to study the patterns of prevalence and survival of patients in all tumor localizations, including, among other things, rare malignant cardiac tumors.

Злокачественные новообразования глаза и его придаточного аппарата (С69) относятся к редким опухолям. До настоящего времени государственная статистика в России (ф. № 7) «Сведения о злокачественных новообразованиях» располагает данными только для расчета заболеваемости населения. Эти данные были включены в ф. № 7 по нашему предложению только с 2011 г. В 2018 г. мы впервые смогли располагать данными о заболеваемости населения по возрасту отдельно для мужчин и женщин, но только в целом по России. Такая возможность имеется и у каждого территориального ракового регистра. Расчет показателей одно-и пятилетней выживаемости больных по этой патологии осуществляется только нами по 10 административным территориям Северо-Западного федерального округа, где в феврале 2019 г. был нами сформирован первый в России Популяционный раковый регистр (ПРР) на уровне федерального округа с общим объемом базы данных более 1 млн наблюдений. Ранее аналогичные работы осуществлялись только на материалах ПРР Санкт-Петербурга. В настоящей работе впервые представлены особенности изменения структуры онкологической патологии глаза и его придаточного аппарата (С69) и расчеты пятилетней выживаемости по возрасту. К сожалению, Управление Федеральной службы государственной статистики относит патологию глаза (С69) к группе «прочие ЗНО» и не публикует данные о смертности. Вместе с тем, используя базу данных ПРР, нужно иметь в виду, что индекс достоверности учета (отношение числа умерших к числу впервые в жизни заболевших) по ЗНО глаза (С69) составляет около 0,50 % и постоянно снижается, а достоверность учета повышается. Malignant tumors of the eye and its adnexa (C69) are rare malignancies. To date state statistics in Russia (Form No. 7) have data only for estimating the incidence of the population. These data, at our proposal, were included in Form No. 7 only since 2011. In 2018 for the first time we were able to have data on the incidence of the population by age separately for men and women but only in Russia as a whole. The estimation of rates of a 1- and 5-year patient survival for this pathology is carried out only by us in the North-West Federal Region of Russia, where, in February 2019, we have formed the fi rst Population-based Cancer Registry (PCR) in Russia at the Federal Region’s level with a total database of more than 1 million cases. Previously similar work was conducted on the materials of the Population-based Cancer Registry of Saint-Petersburg. For the first time this work presents the features of data on changes in the structure of oncological pathology of the eye and its adnexa (C69) and estimations of a 5-year survival by age. Unfortunately the Office of the Federal State Statistics Service classifies the pathology of the eye (C69) as a group of «other malignant tumors» and does not publish mortality data. At the same time, using the PCR database, it must be borne in mind that index accuracy (the ratio of a number of deaths to a number of sick for the fi rst time in life) according to malignant tumors of the eye (C69) is about 0,50 % and is constantly decreasing.

In the previous article, we presented an analysis of the prevalence and quality of registration of patients with malignant neoplasms of the skin (C44) in Russia and the North-Western Federal District, showed the quality of accounting and revealed the features of the dynamics of localization and histological structure. In this report, for the first time in Russia, our attention is focused on the nature of evaluating the effectiveness of anti – cancer measures based on the main criterion-calculating the survival rates of patients with malignant skin neoplasms at the level of the North-Western Federal District, whose level of "rough" and standardized indicators of morbidity and mortality of the population practically does not differ from the average Russian one. Calculations of all types of survival rates of patients with malignant skin neoplasms (C44) at the federal district level became possible after the organization in February 2019 of a single database (DB) of the cancer registry of the North-Western Federal District, numbering more than 1 million cases of ZNO, including more than 20,000 with malignant melanoma of the skin (2.0%). The created database allows for in-depth development of the dynamics of the localization and histological structure for any part of the localization structure (lip, ear, trunk, etc. parts of the body), which is not available for conducting on the basis of state reporting forms formed before receiving all the information characterizing the state of the oncological service, not to mention that when analyzing the activities of the oncological service, databases of population cancer registers are not used and calculations of the main criterion for its assessment – the calculation of patient survival.

Настоящее исследование посвящено изучению особенностей достоверности учета и эффективности лечения больных на популяционном уровне с привлечением надежных аналитических показателей выживаемости, исчисленных по международным стандартам, с учетом гистологической структуры опухолей и стадии заболевания. Для проведения любых исследовательских разработок мы, в первую очередь, должны убедиться в надежности отобранного для исследования материала. Среди шести основных наиболее надежных критериев оценки деятельности онкологической службы, таких как заболеваемость, смертность, уровень морфологической верификации опухолей, важнейшее значение имеют такие показатели, как индекс достоверности учета, погодичная летальность и выживаемость больных злокачественными новообразованиями. Цель работы - определить уровень достоверности учета больных меланомой кожи (С43) и ее динамику с 1995 по 2018 г. по России в целом и по ряду административных территорий. Исчислить пятилетнюю выживаемость в СЗФО РФ с учетом гистологической структуры опухолей и стадии заболевания. Материалом для исследования являются статистические сборники, в том числе авторские, база данных ракового регистра СЗФО РФ. Использованы стандартные методы статистического анализа. Важно отметить, что по всем административным территориям, чьи базы данных Популяционного ракового регистра были привлечены для расчета пятилетней наблюдаемой и относительной выживаемости, уровень показателей выживаемости больных злокачественной меланомой кожи существенно ниже среднеевропейских (Eurocare-4). Таким образом, проведенное исследование выявило существенное улучшение учета больных злокачественной меланомой кожи. Вместе с тем, сохраняется относительно низкий уровень расчетных показателей пятилетней наблюдаемой и относительной выживаемости больных злокачественной меланомой кожи. The present study examines the characteristics of registration accuracy and efficiency of treatment of patients at the population level using reliable analytical indicators of survival of patients, calculated according to international standards, taking into account the histological structure of tumors and the stage of the disease. For any research development, we must first ensure the reliability of the material selected for research. Among the six main most reliable criteria for evaluating the activities of the cancer service, such as morbidity, mortality, and the level of morphological verification of tumors, such indicators as the index of reliability of accounting, primary mortality, and survival of patients with malignant neoplasms are of the most important importance. The aim of the work is to determine the level of reliability of accounting for patients with skin melanoma (C43) and its dynamics from 1995 to 2018. for Russia as a whole and for a number of administrative territories. Calculate the five-year survival rate in the North-Western Federal District, taking into account the histological structure of tumors and the stage of the disease. The material for the study is statistical collections, including author’s ones, the database of the cancer register of the North-Western Federal District of the Russian Federation. Standard methods of statistical analysis were used. It is important to note that for all administrative territories whose population cancer registry databases were used to calculate the five-year observed and relative survival rates, the survival rate of patients with malignant skin melanoma is signifi cantly lower than the European average (Eurocare-4). Thus, the study revealed a significant improvement in the treatment of patients with malignant melanoma of the skin. At the same time, the estimated fiveyear observed and relative survival rates of patients with malignant melanoma of the skin remain relatively low.

Background: Colon cancer (CC) refers to a group of malignant tumors (MT) with a high rate of morbidity increase. In Russia, only from 2000 to 2014 the standardized incidence rate has increased from 12.9 to 16.6 0/0000, or by 28.7% among males, and from 10.8 to 13.10/0000, or by 21.9 % among females. In the Republic of Belarus (RB) the level of CC incidence has increased from 12.0 to 18.0 0/0000 (by 50%) among males and from 9.0 to 13.40/0000 (by 44.4%) among females. In the City of Minsk the level of CC incidence was highest in 2000 and 2014 and has increased by 33.5%, while in St. Petersburg it was lower than in a number of regions of the North-West Federal District of the Russian Federation (NWFD RF) and has decreased by 8.8% for 15 years. Purpose of the study: To present a comparative evaluation of the main analytical rates of CC morbidity, its mortality and the state of contemporary diagnosis in RB and NWFD RF. Materials and methods: The basis of this work consisted of materials of population-based cancer registries databases of RB and NWFD RF. Also there were used published data to compare the basic parameters. For the estimation of the rates there were used the traditional methods. Standardized rates were estimated on the basis of the world standard population structure. Cartograms were prepared on the basis of the methodology of geographic information systems (GIS). Analysis of obtained data: There was performed a precise analysis of population-based data on CC, including particularly the 4th sign of ICD-10 and identified a number of patterns of CC incidence at present stage. Conclusion: CC incidence increases in Russia, North-West Federal District of the Russian Federation and in Republic of Belarus. Registering rates and the state of cancer control have improved at all comparable administrative territories. It primarily concerns such parameters as morphological verification level, early diagnosis, one-year lethality and others. Comparative monitoring and a study of experience in various administrative territories and countries with the best rates could be useful for planning effective anti-cancer programs.

Introduction. Prior to detailed review of the North- West Federal District (NWFD) pulmonology service per- formance during the COVID-19 pandemic, it is especial- ly worthwhile to evaluate its human resourcing in the prepandemic period of 2015–2019, since the perfor- mance of pulmonology service largely depends on its staffing, moreover in conditions of restructuring and decreased number of pulmonology bedspace in hospi- tals. Мethods. In the course of the study, the authors have analysed human resourcing data for pulmonology profile within the state healthcare system in NWFD for the period of 2015–2019. The results were statistically processed and data analysis carried out using Micro- soft Office Excel and SPSS software. Results . In the state healthcare system medical institutions of the NWFD throughout 2015–2019, the number of regular pulm- onologists’ positions fluctuated unrhythmically. The number of pulmonologists (MDs) in NWFD grew 0.82% during the period of 2015–2019, for comparison, in the Russian Federation the growth for the same period was 32.08%. The number of operating pulmonology depart- ments remained practically the same, but the number of occupied positions constantly changed. Practically all the pulmonologists in the NWFD have a specialist certi- ficate, but the share of doctors with qualification grade remains insufficient, and is on the decrease. Statistical analysis demonstrated significant positive Pearson cor- relation between the number of pulmonologists and general respiratory incidence (0.74 р=0.04), mortality (0.756 р=0.003), and the number of occupied pulmo- nology bedspace (0.619 р=0.024). Conclusion. Today, pulmonology service human resourcing review is to be conducted taking into account respiratory morbidity and mortality and the need for pulmonology services among the general population. Currently, there is no sci- entifically justified regulatory framework for the deve- lopment of the pulmonology service in NWFD regions, while the work of such services depends very much on the regional budgets and medical services’ tariffs. The development and fast implementation of such compre- hensive measures as salaries’ increase, adequate social support, regular residential and non-residential courses of continuous medical education, professional networ- king, etc. will allow to maintain and holistically develop the human potential of the NWFD pulmonology service.
