Acute poisoning in children is one of the most frequent reasons for seeking medical care and hospitalization in intensive care units, but it is extremely rare in children of the first year of life. On the example of the clinical case, the peculiarities of the course of acute severe methadone poisoning in a two-month-old child against the background of natural feeding are considered. The main data of history and clinical-laboratory examination are reflected, which allowed to suspect acute poisoning in a timely manner and to carry out reasonable intensive therapy, against the background of which the maximum rapid improvement of the child s condition is achieved. The algorithm of differential diagnosis of comatose state in children is presented, which can be used in all cases of sudden suppression of consciousness of unclear genesis. Special attention is paid to the need for maximum early recognition of exogenous poisoning and targeted pathogenetic treatment aimed at elimination of toxicant metabolites. A distinctive feature of this case was the severe course of acute methadone poisoning in the infant with the development of coma and respiratory distress, which regressed against the background of the treatment carried out with the full recovery of the patient.

The incidence of cancer in children is rising in the U.S. Although cancer in the first year of life is relatively rare, understanding the early signs and symptoms of and the historical factors associated with most common infant cancers is essential for providing optimal care to these infants and their families. Neonatal nurses and nurse practitioners play a pivotal role in early recognition and detection of infant malignancy. This article reviews the incidence of and historical factors associated with infant cancers and discusses clinical presentations and available diagnostic images as well as screening tools for the five most common types of infant malignancy.

The relationship between infant pediatric care use in the first year of life and the quality of mother-infant attachment at 1-year of age was examined. Fifty-six mother and infant pairs were classified by the Ainsworth Strange Situation procedure as securely (n = 17) or anxiously attached (n = 39). Pediatric records for the first 12 months of life were reviewed for frequency and type of visit, diagnoses, treatment, and symptoms prompting the care visit. Anxiously attached infants used acute care services (walk-in and emergency room) significantly earlier and more often than infants with a secure attachment. Use of acute care services began at a mean age of 3.8 months for the anxiously attached infants compared with 6.5 months for the securely attached. Differences in utilization persisted through the year. Mean acute care use in the anxiously attached infants was 3.6 visits compared with 1.9 visits for the securely attached. Acute care use was associated with a variety of common infant illnesses in both groups. In contrast, there was no difference in the use of well-baby care (mean 6 visits). Utilization and attachment status were independent of birth or prenatal difficulties. High cumulative use of pediatric care during the first year of life may identify infant and mother pairs who ho are experiencing strain in their interactions and relationship. Early recognition would provide an opportunity for comprehensive assessment and preventive intervention in the pediatric setting.

Tuberous sclerosis complex is a multisystem genetic disorder with characteristic skin lesions and variable neurological manifestations like seizures, cognitive impairment and autistic spectrum disorder. Epilepsy and infantile spasms during first year of life are risk factors for mental impairment and Autistic spectrum disorder (ASD). In asymptomatic TSC infants, hypsarrhythmia pattern in EEG suggests early treatment with vigabatrin to improve neurodevelopmental outcome. Early recognition of skin lesions by pediatrician is crucial for timely intervention.

Abstract Research exists that evaluates the mechanics of swallowing respiratory coordination in healthy children and adults as well and individuals with swallowing impairment. The research program summarized in this article represents a systematic examination of swallowing respiratory coordination across the lifespan as a means of behaviorally investigating mechanisms of cortical modulation. Using time-locked recordings of submental surface electromyography, nasal airflow, and thyroid acoustics, three conditions of swallowing were evaluated in 20 adults in a single session and 10 infants in 10 sessions across the first year of life. The three swallowing conditions were selected to represent a continuum of volitional through nonvolitional swallowing control on the basis of a decreasing level of cortical activation. Our primary finding is that, across the lifespan, brainstem control strongly dictates the duration of swallowing apnea and is heavily involved in organizing the integration of swallowing and respiration, even in very early infancy. However, there is evidence that cortical modulation increases across the first 12 months of life to approximate more adult-like patterns of behavior. This modulation influences primarily conditions of volitional swallowing; sleep and naïve swallows appear to not be easily adapted by cortical regulation. Thus, it is attention, not arousal that engages cortical mechanisms.
