The statistics of American psychiatric hospitals (Pennsylvania, New York) show that women are more likely to be affected by hereditary mental illnesses than men and more often than the latter pass on their ailment to female offspring, namely in the ratio 1, 7: 1. English statistics show that men are inherited passes more often from the father (in 1, 1 times), and to women - from mothers (in 1, 3 times more often than the father of the father).

Two syenite sills intrude the local Paleozoic strata of eastern New York State and are exposed along the western shore of Lake Champlain. The sills are fine-grained, alkali feldspar syenites and quartz syenites, with phenocrysts of sanidine and albite. The two sills are compositionally distinct, with crossing rare earth element profiles and different incompatible element ratios, which eliminates the possibility of a simple petrogenetic relationship. Zircon extracted from the upper sill yields a U–Pb age of 131.1 ± 1.7 Ma, making the sills the youngest known igneous rocks in New York State. This age is similar to that of the earliest intrusions in the Monteregian Hills of Quebec, >100 km to the north. Sr and Nd radiogenic isotope ratios are also similar to those observed in some of the syenitic rocks of the eastern Monteregian Hills. The Cannon Point syenites have compositions typical of A-type, within-plate granitoids. They exhibit unusually high Ta and Nb concentrations, resulting in distinct trace element signatures that are similar to those of the silicic rocks of the Valles Caldera, a large, rift-related magmatic system. We suggest that the Cannon Point syenites were melts derived primarily by anatexis of old, primitive, lower crustal material in response to Mesozoic rifting and to the intrusion of mantle-derived magmas. The sills indicate that the effects of continental rifting were spatially and temporally extensive, resulting in the reactivation of basement faults in the Lake Champlain Valley hundreds of kilometers west of the active rift boundary, and crustal melting >50 Ma after the initiation of rifting.

ObjectivesDelirium shares symptoms with some mental illnesses. This may lead to misdiagnosis of delirium in psychiatric patients and a risk of inadequate management. Moreover, literature on delirium in psychiatric patients is sparse. The aim was to analyse possible changes in the diagnostic incidence of delirium in psychiatric patients from 1995 to 2011, and to investigate the patients with regard to sex, age, and type of patient.MethodsAll first time ever diagnoses of delirium among psychiatric patients were identified in the nationwide Danish Psychiatric Central Research Register (DPCRR) from 1995 to 2011. The delirium diagnoses include (1) delirium unspecified, (2) delirium with dementia, and (3) drug-related delirium, all in accordance with International Classification of Diseases-10. The incidence rates were age standardised.ResultsA total of 15 680 persons diagnosed with delirium for the first time were identified in the DPCRR between 1995 and 2011. The total incidence rate of delirium has decreased, reaching 8.4/1000 person-years in 2011. In 2011, 2.6% of the demented patients were diagnosed with delirium with dementia. Diagnosis of delirium is significantly more common in men, and the three groups of delirium showed a characteristic age distribution.ConclusionOur incidences were markedly lower when compared with previous studies. This suggests a possible underdiagnosis of delirium in psychiatric hospitals and should be investigated further, as delirium is a serious state and identifying the syndrome is important for sufficient treatment.

Individuals with serious mental illness are particularly vulnerable to COVID-19. The New York State (NYS) Office of Mental Health implemented patient and staff rapid testing, quarantining, and vaccination to limit COVID-19 spread in 23 state-operated psychiatric hospitals between November 2020 and February 2021. COVID-19 infection rates in inpatients and staff decreased by 96% and 71%, respectively, and the NYS population case rate decreased by 6%. Repeated COVID-19 testing and vaccination should be priority interventions for state-operated psychiatric hospitals. Mental illness is both a cause and sometimes a consequence of homelessness. The prevalence of mental illness in the homeless is greater than for the population as a whole. The chapter considers the evidence for whether the closure of long-stay psychiatric hospitals has added to the problem, and to what extent other factors are at play. Types of social housing and residential support are presented together with the legal framework for entitlement to housing. Programmes aimed at people with long-term homelessness and severe mental illnesses, such as Housing First, are presented with evidence for effectiveness.

Abstract Objectives Using a mouse gestational diabetes mellitus (GDM) model, this study investigated whether maternal choline supplementation (MCS) could alter postnatal growth and metabolic abnormalities associated with GDM. Methods C57BL/6 mice were either fed a low fat (LF, 10kcal % fat) control diet or a high fat (HF, 60kcal % fat) diet prior to and during pregnancy to induce GDM. These mice received either 25mM choline (MCS) or plain drinking water, After weaning, offspring were fed the HF diet for 6 weeks before glucose tolerance testing and dissection. Results In male offspring from MCS-GDM mothers, we observed a decrease in fasting blood glucose levels and an increase in glucose tolerance when comparing to other groups (P < 0.05). Liver choline metabolite measurements demonstrated that free choline content was lower (P = 0.01) in the MCS-GDM male offspring than control GDM male offspring; there is also an increase in liver sphingomyelin concentrations (P = 0.007) in female offspring from MCS-GDM compared to control GDM dams. Conclusions MCS during GDM leads to improvements in blood glucose control in male mouse offspring exposed to a postnatal HF environment. Abstract Aims Although shared decision-making (SDM) has the potential to improve health outcomes, psychiatrists often exclude patients with more severe mental illnesses or more acute conditions from participation in treatment decisions. This study examines whether SDM is facilitated by an approach which is specifically adapted to the needs of acutely ill patients (SDM-PLUS). Methods The study is a multi-centre, cluster-randomised, non-blinded, controlled trial of SDM-PLUS in 12 acute psychiatric wards of five psychiatric hospitals addressing inpatients with schizophrenia or schizoaffective disorder. All patients fulfilling the inclusion criteria were consecutively recruited for the trial at the time of their admission to the ward. Treatment teams of intervention wards were trained in the SDM-PLUS approach through participation in two half-day workshops. Patients on intervention wards received group training in SDM. Staff (and patients) of the control wards acted under ‘treatment as usual’ conditions. The primary outcome parameter was the patients' perceived involvement in decision-making at 3 weeks after study enrolment, analysed using a random-effects linear regression model. Results In total, 161 participants each were recruited in the intervention and control group. SDM-PLUS led to higher perceived involvement in decision-making (primary outcome, analysed patients n = 257, mean group difference 16.5, 95% CI 9.0–24.0, p = 0.002, adjusted for baseline differences: β 17.3, 95% CI 10.8–23.6, p = 0.0004). In addition, intervention group patients exhibited better therapeutic alliance, treatment satisfaction and self-rated medication compliance during inpatient stay. There were, however, no significant improvements in adherence and rehospitalisation rates in the 6- and 12-month follow-up. Conclusions Despite limitations in patient recruitment, the SDM-PLUS trial has shown that the adoption of behavioural approaches (e.g. motivational interviewing) for SDM may yield a successful application to mental health. The authors recommend strategies to ensure effects are not lost at the interface between in- and outpatient treatment.
