The aim of the study was to show the effectiveness of treatment of acne with moderate severity in women with ovarian hyperandrogenism. A total of 25 female reproductive voerast patients with moderate acne and ovarian hyperandrogenism who were not planning pregnancy were examined. All patients were examined, which included a consultation with a gynecologist with a gynecological examination, ultrasound examination of the pelvic organs, a study of the hormonal status, a biochemical blood test, as well as a dermatological examination with the determination of the dermatological index of acne (DIA). After the examination, the patients underwent complex therapy using the external treatment recommended by the dermatovenerologist (skin cleansing, gel with azelaic acid and / or gel with adapalene), as well as the appointment of a combined oral contraceptive (COC) Jes Plus. Based on this study, it can be concluded that complex therapy, including local treatment and taking Jes Plus, is the first choice in the treatment of androgen-dependent dermopathy, manifested by moderate acne, in patients with ovarian hyperandrogenism due to PCOS. Such therapy contributes not only to the formation of a pronounced clinical result, but also causes a low probability of side effects. The administration of COC containing drospirenon and the active form of folate, in the form of the calcium salt levomefolate, allows a favorable effect on the metabolic processes in the body as a whole, and in particular, provides a positive effect on the skin, the cardiovascular system, reduces the risk of systemic and local inflammation, relieving the state of hyperhomocysteinemia, providing prevention of cardiometabolic risks.

In recent decades, the shift in priority in the treatment of patients with pulmonary granulomatosis in the direction of etiotropic therapy and reduced attention to the state of the macroorganism led to a decrease in the effectiveness of treatment. In modern conditions, when carrying out complex therapy for patients with tuberculosis and sarcoidosis, insufficient attention is paid to the state and dynamics of adaptation, resistance, homeostasis and reactivity of the organism. At the same time, the dynamics of these processes in patients is insignificant, the homeostatic balance of the organism is not restored, and the reactivity of the organism remains at the pathological level, with the predominance of paradoxical, hyporeactive and areactive types. This is due to the continuing influence on the regulatory centers of the vegetative system of not diagnosed, latent, ongoing tuberculosis intoxication. Undetected specific intoxication causes in patients activation of the sympathetic department, suppression of parasympathetic and development of dysfunction of the autonomic nervous system. Its dysfunction leads to a decrease in vascular supply of the organism, a high tension of anabolic processes, and a further change in the functional activity of both parts of this system when pathology develops. In addition, the emergence of vegetative dysfunction leads to a high consumption of plastic materials, energy resources, a decrease in the functional reserves of the body, a decrease and depletion of the body’s defense systems, its anti-inflammatory potential and the reactivity of the organism as a whole. These changes lead to an extension of the main course of treatment and the formation of expressed residual tubercular and sarcoidosis in the respiratory organs. To restore these disorders, it is necessary to carry out complex therapy with the inclusion of a personified appointment of activators of protective systems under the control of monitoring their effectiveness.

Background. In acute poisoning, accompanied by a violation of the parameters of homeostasis, the problem of its management by the enteral route has been insufficiently studied.Purpose of the study. To assess the possibility of correcting electrolyte and volemic disorders of the body using an enteral solution (ER) in case of poisoning with psychopharmacological drugs.Material and methods. The study involved 120 patients who underwent intestinal lavage (IL) with ER on the 1st day in complex therapy. In the following days, 40 of them received infusion therapy, and 80 — drank glucose enteral solution (GES), 3–4 liters per day.Results. IL had a corrective effect on the electrolyte composition of the blood, volemic and hemorheological parameters, as well as on central and peripheral hemodynamics. The subsequent administration of GER had a stabilizing effect on these indicators, comparable to that of infusion therapy.Conclusion. In case of poisoning with psychopharmacological drugs, the use of saline enteral solution in the form of intestinal lavage and subsequent oral administration of the same solution in a daily volume of 3–4 liters, but with the addition of glucose, provides correction of impaired homeostasis indicators and may be an alternative to infusion therapy.

Expert system is a method used to implement human science to computer systems with the aim of solving a problem in the field of knowledge. Allergic disease is an overreaction from the body to certain particles from outside that enter the body which makes people feel uncomfortable and disturbed, allergies can occur with a variety of symptoms, some have experienced mild symptoms such as itchy eyes, sneezing, or reddened skin, and some are suffering from allergies with severe asthma. Factors from the causes of this allergy are very much encountered such as through the respiratory tract, through food, through drugs and so forth. Medical experts can use the application that has been built, which aims to make it easier to diagnose allergic diseases of the skin by only paying attention to the symptoms experienced. By using the Certainty Factor (CF) method, we get the value of a person's likelihood of developing allergic disease or not. The purpose of the research is evaluating the efficacy of complex therapy using dironet, lactobifadol and keratin food supplement (DLK) against toxascariosis of the arctic fox.Materials and methods. To determine the helminth fauna in the conditions of Vyatka Fur Breeding Farm, 61 females silver fox and 55 females voilevoy arctic fox were used as study objects. To conduct complex therapy against toxascariosis, the study object was 24 females culled from the breeding herd, spontaneously infected with toxascariosis and being clinically healthy. The morphological and biochemical blood parameters in female foxes were determined before and after dehelminthization, and control weighing of animals from experimental groups was carried out before and after the experiment.Results and discussion. Of the studied 116 fur-bearing animals, 26 (22.4%) were infected with Toxascaris leonina. Infection rate of Toxascaris sp. in silver fox females aged 5–7 years was 42.6% when 7-9 eggs were found in one microscope field (magnification 7 × 9). Infection with Toxascaris sp. affects on the body weight of infected animals. The females silver fox from the second experimental group infected with Toxascaris sp., which were not treated, had an average live weight of 1.3 kg less (16.6%) as compared to the control. The complex therapy with the DLK treatment-and-prophylactic complex contributed to the improved metabolic processes and digestion. The applicability of the DLK treatment-and-prophylactic complex for therapy against toxascariosis was proved.

In the conditions of the pig breeding complex, the effectiveness of two treatment regimens for antenatal hypotrophy of piglets was studied. Two groups of piglets were formed, where the ironcontaining drug «Ursoferran – 200» was used in complex therapy in the 1st experimental group, and «Ferropen-200»in the 2nd experimental group. In a clinical study, hypotrophic piglets showed characteristic symptoms of the disease. In the experiment, the live weight of piglets was measured at the time of farrowing and on the 3rd, 7th, 14th and 21st days of life. The hematological parameters were studied. The most pronounced therapeutic effect was observed in the 1st experimental group, where the body weight gain was higher by 8,3% than in the 2nd experimental group.

AbstractObjectivesα-Lipoic acid is used as an antioxidant in multivitamin formulations to restore the normal level of intracellular glutathione after depletion caused by environmental pollutants or during physiological aging of the body, as a chelating agent, as a dietary supplement, in anti-aging compositions. Lipoic acid (LA) acts as a buffer in cancer therapy and in therapy of diseases associated with oxidative stress. The effect of LA on the catalytic functions of cytochrome P450 3A4 as the main enzyme of the biotransformation of drugs was studied. It was shown that LA in the concentration range of 50–200 μM affects the stage of electron transfer (stage of cytochrome P450 3A4 heme reduction), decreasing the cathodic reduction current by an average of 20 ± 5%. The kinetic parameters (kcat) of the N-demethylation reaction of erythromycin, the antibiotic of the macrolide group, used as a marker substrate for the comparative analysis of the catalytic activity of cytochrome P450 3A4, both in the presence of α-lipoic acid and in the cytochrome P450 3A4-erythromycin complex, amounted to comparable values of 3.5 ± 0.9 and 3.4 ± 0.9 min−1, respectively. Based on these experimental data, we can conclude that there is no significant effect of α-lipoic acid on the catalysis of cytochrome P450 3A4. These results can be projected on the possibility of using α-lipoic acid in complex therapy without negative impact on the enzymatic cytochrome P450 system.MethodsThe analysis was performed in electrochemical non-invasive model systems for recording the catalytic activity of cytochrome P450 3A4, using screen-printed electrodes, modified with membranous didodecyldimethylammonium bromide.ResultsIt was shown that LA did not affect the N-demethylation of macrolide antibiotic erythromycin. Catalytic constant (kcat) of N-demethylation of erythromycin corresponds to 3.4 ± 0.9 min−1 and in the presence of LA corresponds to 3.5 ± 0.9 min−1.ConclusionsBased on the obtained experimental data, we can conclude that there is no significant effect of α-lipoic acid on individual stages and processes of catalysis of cytochrome P450 3A4. LA can be recommended for inclusion in complex therapy as an antioxidant, antitoxic and chelating compound without negative impact on the enzymatic cytochrome P450 3A4 activity of the human body.

An important feature of living organisms, acquired in the process of evolution, is the ability to adapt to various external influences and changes in the environment, to maintain homeostasis and regulatory processes in organs, systems and the body as a whole. However, with modern breeding technologies, the physiological capabilities of the animal's body are often unable to change as quickly as required by the conditions of industrial maintenance, which leads to stress. Vitamin preparations and biologically active feed additives are widely used by veterinary medicine specialists to increase resistance to stressful situations, disease prevention, increase resistance, safety and productivity of farm animals. Preparation L-tsyn, solution for injection (manufactured by BIOTESTLAB LLC), is a drug, the action of which is due to the prescription combination of active substances, namely: butaphosphane, B vitamins (vitamin B12, vitamin B3) and L-carnitine. The aim of the study was to investigate the effectiveness of the drug L-tsyn on horses under the influence of stressors of various natures. Two experiments on working horses with constant, excessive physical activity with chronic exercises and with complex therapy of intoxication with signs of acute renal failure in horse were conducted. Clinical trials of the drug were performed on working horses aged 5–15 years, of different sexes. The drug was administered in a dose of 20–25 ml per animal, 1 time per day for 5 consecutive days. The clinical condition of the experimental animals was observed for 21 days from the start of the drug. The morpho-functional state of the animal's body before and after the drug was established according to the indicators of the clinical condition, morphological and biochemical parameters of the blood, which were determined according to generally accepted methods. In working horses under excessive load the drug L-tsyn activated the processes of hematopoiesis, improved the physiological state of the body and contributed to the increase the general tone of the body. The drug as part of complex therapy in the treatment of intoxication, showed hepato- and nephroprotective properties, reduced the manifestations of intoxication, helped to restoration of the animal’s normal physiological state. According to the results of clinical, hematological and biochemical studies, it can be concluded that the drug L-tsyn has a stimulating effect on metabolic processes in the animals’ body, shows tonic properties, increases the body's resistance to toxins and adverse environmental factors and can be used as monotherapy and for the complex treatment of diseases of various etiologies in horses.

Introduction. Epilepsy is a chronic brain disease, which treatment consists of relieving the seizures with the help of constant long-term use of anti-epileptic drugs. This, in turn, can cause various negative effects on the body. Epilepsy is not a single disease with various attacks, it is a whole spectrum associated with comorbid conditions, including anxiety and depression. Additional medication is suggested to correct this. Therefore, developing the effective drug-free methods with the help of osteopathic treatment to correct anxiety and depression in epilepsy complex therapy received priority.Goal of research — to study the osteopathic treatment potential to reduce anxiety and depression levels in children with various forms of epilepsy.Materials and methods. The study included 72 children with various forms of epilepsy, aged from 7 to 17 years, 35 girls, 37 boys. The selection criteria were as follows: at least 1 year of epilepsy course duration, no status or serial course of epileptic seizures, no motor and cognitive deteriorations, all children studied in public schools. All children received osteopathic treatment for 2 months, along with medical treatment. Before and after treatment, each patient′s anxiety and depression levels were assessed according to the Hospital Anxiety and Depression Scale (HADS), along with an osteopathic examination.Results. All children taken in the study revealed a range of somatic dysfunctions of craniosacral system, skull and reciprocal tension membranes, with biomechanical abnormality. By initial testing, anxiety and depression average values according to HADS scale were above normal in all children. After osteopathic treatment, a statistically signifi cant decrease in anxiety and depression levels was found.Conclusion. Osteopathic treatment is effi cient for epilepsy complex therapy in children. It helps to reduce drug load in correction of anxiety and depression that accompany the underlying disease.

Effective and safe treatment of patients with polymorbidity is an urgent task of modern healthcare. Of particular difficulty is the treatment of patients with cardiovascular comorbidity, which requires an integrated approach to the treatment and development of a special, so-called patient-oriented approach. Modern scientific evidence proposes the use of levocarnitine, as part of complex therapy to increase its effectiveness. So, it was shown that levocarnitine can have a beneficial effect on blood pressure in patients with hypertension, especially those who are overweight and obese. Levocarnitine is also effective in the treatment of heart failure, which has been studied in several clinical studies, which demonstrated its ability to increase the ejection fraction of the left ventricle, stroke volume of blood and other indicators. Optimization of the bioavailability of nitrogen oxide (NO) and a decrease in systemic oxidative stress while taking levocarnitine plays an important positive role in complex therapy in patients with coronary artery disease: exertional angina and post-infarction cardiosclerosis, reducing the number of anginal attacks. Since levocarnitine has powerful antioxidant effects, it also has the neuroprotective effect found in in vitro studies in animal experiments. In case of impaired renal function, due to accelerated elimination and impaired reabsorption, a deficiency of levocarnitine in the body tissues develops, and therefore the US Food and Drug Administration (US FDA) decided on the possibility of using levocarnitine in patients on hemodialysis. Thus, taking into account the positive effects of levocarnitine in a number of frequently combined diseases, it can be considered as the drug of choice in the treatment of patients with polymorbidity.
