Background: Magnetic resonance imaging is a very useful diagnostic tool for the detection of abnormalities of injured knee joint. Objective: This present study was performed to see the different abnormalities of injured knee joint detected by MRI among adult patients. Methodology: This cross-sectional study was conducted in the Department of Radiology and Imaging at Bangabandhu Sheikh Mujib Medical University (BSSMU), Dhaka, Bangladesh from January to June 2021 for a period of six months. The patients in the age group of equal or more than 18 years with both male and female who were visiting Department of Orthopaedics at BSMMU, Dhaka with history of knee injuries were subjected to MRI were selected as study population. MRI findings of the knee joint were analyzed for the presence of any signal changes or lesions of varying severity for the structures of menisci, cartilage, bone marrow, tendons and ligaments of injured knee joint. Results: A total number of 65 cases of knee joints were analyzed. The mean with the SD of age of the study population was 35.34±13.371. The left knee (60.0%) was more involved than right knee (40.0%). The lesion in femur and tibia were found in 10(15.4%) cases and 8(12.3%) cases respectively. Out of 65 knee joints there were 41 joints were found the tear of anterior cruciate ligament. The complete, interstial tear in the anterior cruciate ligaments were 26(40.0%) and 15(23.1%) knee joints respectively. However, anterior cruciate ligaments of 24(36.9%) knee joints were found normal. Medial meniscus (36.9%) was more involved than lateral meniscus (18.5%). Conclusion: In conclusion young adult male are more commonly affected their left knee joint with the tearing of complete tear of anterior cruciate ligaments and involvement of medial meniscus. Synovial Chondromatosis is a rare and it is a benign condition characterized by multiple cartilaginous nodules in synovial facet spaces. Synovial Chondromatosis affects most commonly the knee joint. This is a case report of a 30-year-old male patient presented with pain and swelling over the left knee joint. On evaluation MRI shows loose bodies, for which he underwent, arthroscopic exploration. Viscous fluid and loose bodies were identified and showed synovial hyperemia. Synovial debridement was done and loose bodies were removed and sent to histopathological examination. The result signify that arthroscopy is efficient method both in diagnostic as well as therapeutic management of synovial chondromatosis.

Abstract Background Intraarticular injections are widely used to provide pain relief after arthroscopic procedures and minimize the use of opioids. Dexmedetomidine has been proven to potentiate pain relief and postpone the demand for the first analgesic drug when it is used intraarticularly following arthroscopic knee procedures. However, the effects of dexmedetomidine on articular structures have not yet been evaluated. Our aim was to determine the effects of intraarticular dexmedetomidine injection on articular structures such as cartilage and synovium. Design Animal study. Methods Twenty adult rats (Sprague-Dawley) were enrolled in the study. Following appropriate aseptic and anesthetic conditions, dexmedetomidine (100 mcg/ml) (0.25 ml) was injected into the right knee joint (the study group) and normal saline solution (0.25 ml) into the left knee joint (the control group) of the rats. Four rats were sacrificed from each group on days 1, 2, 7, 14, and 21, and knee joint samples were obtained. Histologists evaluated the articular and periarticular regions and the synovium using histological sections, and a five-point scale was used to grade the inflammatory changes in a blinded manner. Results The groups were found to be similar in terms of median congestion scores, edema and inflammation scores, subintimal fibrosis, neutrophil activation and cartilage structure at each of the time intervals. Conclusion In our placebo-controlled, in vivo trial, the intraarticular use of dexmedetomidine seemed to be safe with respect to the studied histopathological parameters. However, complementary studies investigating the histopathological effects, analgesic dosage and adverse effects of dexmedetomidine on damaged articular structure models are needed.

Introduction: The knee joint space is seen on anteroposterior radiograph as a radiolucent area between lower end of femur and upper end of tibia which is an indirect way of evaluating the knee cartilage thickness.  Objective: This study was aimed to determine the knee joint space in the medial and lateral compartments of the knee joint using digital radiograph.  Methodology : This was cross-sectional study. It consisted of digital radiographs of knee joint of 320 individuals. The medial and lateral joint space width of each knee joint was measured using the scale in the computerized software.  Results: The mean values for medial and lateral joint space widths were found to be 6.11±1.57 mm and 7.92±1.66 mm of the right knee joint respectively and 5.99±1.47 mm and 8.18±1.69 mm of the left knee joint respectively. In males, mean values for joint space widths were 6.37±1.58 mm on medial side and 8.21±1.67 mm on lateral side of the right knee; and 6.24±1.56 mm on medial side and 8.33±1.64 mm on lateral side of the left knee. In Females, these values were 5.89±1.53 mm on medial side and 7.66±1.62 mm on lateral side of the right knee; and 5.79±1.37 mm on medial side and 8.06±1.72 mm on lateral side of the left knee.  Conclusions: It was concluded that the lateral joint space was greater than the medial joint space in both knees. The joint space widths were found to be reasonably constant with increasing age among studied population.

Abstract Abstract 4903 Anaplastic large cell lymphoma (ALCL) accounts for approximately 10% of the childhood lymphomas. ALCL is characterized by peripheral, mediastinal or intra-abdominal lymph node involvement or extranodal involvement. We report a child with ALCL with unusual localization. An eleven-year-old boy had been admitted to the another university hospital with left knee pain and swelling for 15 days. The left knee joint punction had been done, and osteomyelitis had been diagnosed, and treated with antibiotics. However, there had been no recovery from his complaints, and left knee synoviectomy had been done. This operation had revealed anaplastic large cell lymphoma in the intraarticular soft tissue of the left knee. Then, he was referred to our hematology and oncology clinic for advanced therapy. There was nothing significant in the background and family history. Physical examination showed a 3 cm mass arising from the left knee joint with lobulated contours. PET-CT showed the left inguinal lymph node involvement, only. Chemotherapy according to ALCL 99 protocol was started because of the tumor was not resectable. After the first course of therapy, tumor mass was reduced apparently. By the end of his second chemotherapy course, tumor was not detectable. Now, he completed his therapy, and he is on follow-up without any complaint or recurrence for 18 months. In conclusion, to the best of our knowledge, this is the first reported case with ALCL originating from the intraarticular soft tissue which was treated successfully. Purpose: The aim of this study was to establish diagnostic criteria for meniscal subluxation, and to determine whether there was any connection between meniscal subluxation and other common meniscal and knee-joint abnormalities. Material and Methods: The normal position of the meniscal body was assessed in 10 asymptomatic volunteers. MR signs of meniscal subluxation were evaluated retrospectively in 60 symptomatic patients with pain in the knee, impaired mobility, and/or joint swelling who had no clear diagnosis after the evaluation of case history, clinical examination, and radiography. The criterion for subluxation of the meniscus was defined as a distance of ≥3 mm between the peripheral border of the meniscus and the edge of the tibial plateau. Results: In the volunteers, the mean distance from the medial meniscus to the edge of the tibial plateau was 0.07 mm, and that from the lateral meniscus was 0 mm. In 55 symptomatic patients without meniscal subluxation, the mean distance from.the meniscus to the edge of the tibial plateau was 0.27 mm. Five patients (8%) had evidence of meniscal subluxation, 4 in the medial meniscus and one in the lateral meniscus. The most commonly associated knee abnormality was joint effusion in 5 knees and osteoarthritis in 2 knees. Conclusion: Meniscal subluxation was not a rare finding with MR imaging in patients with painful knees. Meniscal subluxation was associated with other knee abnormalities such as joint effusion or osteoarthritis.

Aging is a physiological process that will reduce all the functions of organs, one of which is on the musculoskeletal system which can lead to limited motion. This study aimed to determine the effect of Range Of Motion (ROM) of the passive joint flexibility in the elderly in PSTW Minaula Kendari. This research was conducted in PSTW Minaula Kendari. Number of samples 12 people. This study uses the design of one group pretest-posttest. Leverage data is done through observation and documentation. Data were analyzed using paired t-test at 95% confidence level with a = 0.05 using computerized tools (SPSS-20). The results showed that there is the effect of passive ROM tehadap right knee joint flexibility by providing flexion (p = 0.00), extension (p = 0.00), the left knee with the provision of flexion (p = 0.01), extension (p = 0.00), with the provision of the right ankle dorsi flexion (p = 0.00), plantar flexion (p = 0.00), the left ankle with the provision of dorsi flexion (p = 0.00), plantar flexion (p = 0.00), the right foot by giving inverse (p = 0.00), eversion (p = 0.00), the left foot by giving inverse (p = 0.00), eversion (p = 0.00). The conclusion of this study is to show that there is the effect of passive ROM to the flexibility of the joints in the elderly.
