Dermatozoonoses are skin diseases caused by arthropod pathogens. This group of skin diseases creates a particular diagnostic problem. Dermatozoonoses include deep and superficial skin changes. Superficial skin changes occur at the sites of bites or after contact with poisonous animals. Deep skin changes occur due to the penetration of the parasite into the skin. A prime example of this disease is dermatobiasis. It is an obligate myiasis, caused by the larvae of Dermatobia hominis flies (Oestridae family) that inhabit mostly in forests of Central and South America. A clinical case of the development of furunculoid myiasis in a russian geologist on a business trip to Argentina is presented.

POEMS syndrome is a rare paraneoplastic syndrome whose name is an acronym formed from the initial letters of the names of the symptoms originally used to determine it: polyneuropathy, organomegaly, endocrinopathy, monoclonal protein and skin changes. Due to the rarity of the disease and the small number of cases described in the literature, its diagnosis is difficult. The average time from onset of symptoms to diagnosis is 18 months. Moreover, the prognosis of the disease depends on the early onset of specific treatment. The article describes a clinical case of POEMS-syndrome in a 53-year-old man, which illustrates the difficulties associated with the timely recognition of this unusual disease.

Abstract Botfly myiasis is an infestation of the skin or a body cavity by developing larvae of Dermatobia hominis, one of the most common flies that cause human infestation among the local population, in regions ranging from Mexico into South America and in travelers. The life cycle starts when a female fly glues the eggs to the vector, a blood-sucking arthropod, which carries the unhatched larvae to the susceptible host. A case of furuncular botfly myiasis in an 85 year-old female with recent travel to Belize is presented here to highlight the parasite life cycle and review the different treatment options.

A family doctor, as well as a sexologist, gynecologist, in his medical practice encounters problems related to skin diseases significantly affecting the sense of femininity or masculinity. An example of such an issue, especially in gynecology of developmental age, is the problem of skin changes associated with androgenization syndromes. Typical dermatological diseases are sometimes associated with enormous pain, not only physical, but also mental, resulting from the fact that skin disease processes significantly disfigure the body, lower self-esteem and significantly affect the sexual sphere. Selected aspects related to the holistic approach to the issue of pain and suffering, also related to skin lesions, are presented. This issue, like all medical fields, is a source of philosophical reflection. It is inseparable from psychodermatology. The experience of the SARS-CoV-2 coronavirus pandemic, taking into account skin lesions, also prompts a similar search.

This chapter describes many of the skin diseases associated with diabetes, thyroid, pituitary, and sex hormone disorders and includes some useful treatment tips and clinical images. Diabetic reactions include skin infections, granuloma annulare, necrobiosis, eruptive xanthomas, and insulin injection site reactions. Brief descriptions of skin problems encountered in hyper- and hypothyroidism are included. The rarer but important skin changes seen in pituitary disorders such as Cushing’s syndrome, Addison’s disease, and acromegaly are described. The role of androgens on the skin and problems created from androgen or oestrogen deficiencies/excess are elaborated. The very rare polyglandular autoimmune type 1 and multiple endocrine neoplasia syndromes are also briefly discussed.

Scalp pruritus is a frequent problem encountered in dermatological practice. This disorder is caused by various underlying diseases and is a diagnostic and therapeutic challenge. Scalp pruritus may be localized to the scalp or extended to other body areas. It is sometimes not only associated with skin diseases or specific skin changes, but also associated with lesions secondary to rubbing or scratching. Moreover, scalp pruritus may be difficult to diagnose and manage and may have a great impact on the quality of life of patients. It can be classified as dermatologic, neuropathic, systemic, and psychogenic scalp pruritus based on the potential underlying disease. A thorough evaluation of patients presenting with scalp pruritus is important. Taking history and performing physical examination and further investigations are essential for diagnosis. Therapeutic strategy comprises removal of the aggravating factors and appropriate treatment of the underlying condition. All treatments should be performed considering an individual approach. This review article focuses on the understanding of the pathophysiology and the diagnostic and therapeutic management of scalp pruritus.

Pansclerotic morphea (PSM) is a rare skin disease characterized by progressive stiffening of the skin with or without the typical superficial skin changes usually seen in morphea (localized scleroderma). Standard therapy, consisting of a combination of systemic glucocorticoids and methotrexate or mycophenolate mofetil, does rarely stop disease progression, which may lead to severe cutaneous sclerosis and secondary contractures. Little is known about the efficacy of newer biologicals such as abatacept, a fusion protein antibody against CTLA-4, or tocilizumab, a fully humanized IL-6R antibody, in the treatment of this pathology. We present the case of an 8 years old girl with an unusual, progressive stiffening of the skin, which was eventually diagnosed as pansclerotic morphea. A treatment with systemic glucocorticoids and methotrexate combined with tocilizumab led to a good clinical response within 2 months after initiation. In this paper, we discuss differential diagnoses to be considered and this new promising treatment option based on a case review of the literature.

Five cases of ocular rosacea (one male and four females) are reported in this paper. Two of the patients were with keratoconjunctivitis sicca, one with conjunctivitis chronica and blepharitis, one with conjunctivitis chronica and meibomitis, and one with reccurent corneal erosions with meibomitis and chordeloum. In four patients ocular symptoms preceded the occurence of skin lesions. The treatment with oral tetracyclines significantly improved the state of ocular rosacea in four patients, while in one case the changes of the anterior eye segment progressed in more severe state of ulcerative keratitis. It is considered that in almost 20% of the patients with rosacea ocular lesions may precede the skin changes, representing a diagnostic problem. Thus, in those cases multidisciplinary approach is suggested. Such approach is particularly important because of the decrease of morbidity and the prevention of the onset of the eye complications such as drastic worsening of visual acuity, i.e., the blindness.

The skin and related structures undergo changes during pregnancy and peurperium. They can be separated into three categories - hormone related pregnancy specific and preexisting. the physiologic changes are mainly of cosmetic importance. The pregnancy specific skin diseases have associated symptoms. Most skin conditions resolve post partum and only require symptomatic treatment. She presents for care to the emergency room accompanied by a female friend, complaining of ‘nervios’, feeling guilty for not being able to perform her duties as a wife, and concerned that there may be some type of imbalance in her body. The interviewer is a female psychiatrist, born in South America and trained in the USA.
