This paper presents the current data regarding the epidemiology, risk factors, diagnosis, clinical manifestations, and treatment of Erythroplasia of Queyrat (EQ). The possibility of using photodynamic therapy (PDT) for EQ is analyzed, and clinical observation of a patient with EQ who underwent PDT with an external gel photosensitizer based on E6 chloride is described.

Objective — to study the features of the clinical course, diagnosis and rational approach to the treatment of Gottron’s carcinoid papillomatosis of skin.Materials and methods. The article presents current data on the clinical course, pathomorphological criteria, diagnosis and treatment of Gottron’s carcinoid papillomatosis of skin. The authors’ own clinical observation is given.Results and discussion. Gottron’s carcinoid papillomatosis of skin is dermatosis that is quite rare in medical practice. The disease is characterized by a long course with a permanent progression of the pathological process. In the vast majority of patients, this pathology develops against the background of other chronic dermatoses such as psoriasis, eczema, lichen planus, pemphigus. In addition, these patients are diagnosed with concomitant multiorgan somatic pathology, which causes not only severe course and recurrence of this dermatosis, but also requires a comprehensive approach to the treatment of these patients, taking into account the identified somatic pathology.Conclusions. Gottron’s carcinoid papillomatosis of the skin is characterized by a long course against the background of permanent progression of the pathological process.Treatment of patients with Gottron’s carcinoid papillomatosis should be comprehensive and individualized, taking into account the severity of dermatosis and concomitant pathology, which is predominantly multiorgan in nature and in some cases causes severe dermatosis and resistance to traditional therapy. This clinical case indicates a significant regression of clinical manifestations of dermatosis in patients with long­term disease, provided the correct diagnosis and adequate therapy.

BACKGROUND: Extrapulmonary TB (EPTB) is more difficult to diagnose than pulmonary TB. The delayed management of EPTB can lead to complications and increase the socio-economic burden.METHODS: Patients newly diagnosed with EPTB were retrospectively enrolled from 11 general hospitals in South Korea from January 2017 to December 2018. The basic characteristics of patients were described. Univariable and multivariable analyses were performed between early and delayed diagnosis groups to identify risk factors for delayed diagnosis and treatment in EPTB.RESULTS: In total, 594 patients were enrolled. Lymph node TB (28.3%) was the predominant form, followed by abdominal (18.4%) and disseminated TB (14.5%). Concurrent lung involvement was 17.8%. The positivity of diagnostic tests showed no significant difference between the two groups. Acute clinical manifestations in disseminated, pericardial and meningeal TB, and immunosuppression were associated with early diagnosis. Delayed diagnosis was associated with outpatient clinic visits, delayed sample acquisition and diagnostic departments other than infection or pulmonology.CONCLUSION: The delay in diagnosis and treatment of EPTB was not related to differences in microbiological characteristics of Mycobacterium tuberculosis itself; rather, it was due to the indolent clinical manifestations that cause referral to non-TB-specialised departments in the outpatient clinic and delay the suspicion of TB and diagnostic testing.

The article presents current data on the etiology, diagnosis and treatment of acute pancreatitis in children. Over the past 10-15 years, the incidence of pancreatitis has increased and is 3.6-13.3 cases per 100,000 children. According to the results of an international study INSPPIRE a group of researchers involving in to the study of pediatric pancreatitis gave a definition of acute, acute recurrent and chronic pancreatitis and developed clinical guidelines for the management of this pathology in children. Acute pancreatitis is a polyetiological disease, the causes of which may be abnormalities in the development of the pancreatobiliary system, drugs, injuries, infections, systemic diseases. Diagnosis is based on clinical manifestations and laboratory and instrumental data. Clinically acute pancreatitis is manifested by abdominal, dyspeptic and asthenic syndromes. Particular attention is paid to the timely diagnosis of pancreatitis in children with unexplained abdominal pain syndrome. In such cases, it is mandatory to determine the level of amylase and lipase. Transabdominal ultrasound is the screening method among instrumental methods. Computed tomography and magnetic resonance imaging are necessary imaging techniques for suspected pancreatic necrosis. Magnetic resonance cholangiopancreatography, in turn, plays an important role in the diagnosis of anomalies and lesions of the biliary and pancreatic ducts. There are modern classification, diagnostic criteria of acute pancreatitis, laboratory and instrumental methods of the diagnosis and approaches to the treatment in children in the article.

In recent decades, the problem of chorioamnionitis (CA) (or intra-amniotic infection) takes a leading place in modern obstetrics and perinatology. It is known that CA is one of the main risk factors for preterm birth and increases neonatal morbidity. Intraamniotic infection is a polyetiology condition. It is characterised by the variety of clinical manifestations absence of clearly association between the infection`s clinical signs in mother and infants injury. In present review we defined current data about chorioamnionitis, based on the last evidence of international scientific literature.

Abstract: diagnosis and treatment of bronchial asthma patients with COPD overlapping syndrome (PS) causes lots of challenges. The importance of a true diagnosis in such cases is crucial. It could be done with the help of selective therapeutic methods. Placement of such patients into a special group to divide them from patients with isolated asthma or COPD seems justified due to separate physiological signs, therapeutic care and clinical manifestations. These patients also show higher risk of acute process, low level of lifestyle, suppressed external breath function. The PS phenotype is often found in pipeline production workers. Clinical monitoring of comorbidity of BA and COPD overlapping syndrome is shown in the paper on an example of a worker with long working experience and regular occupational exposure ( with increased MAC levels) to dust with mixed composition at pipeline accessories production factory.

The article provides an overview of the use of photodynamic therapy for photodamage of the skin. The causes, pathogenesis and clinical manifestations of skin photodamage are considered. The definition, principle of action of photodynamic therapy, including the sources of light used, the classification of photosensitizers and their main characteristics are given. Analyzed studies that show the effectiveness and comparative evaluation in the selection of various light sources and photosensitizing agents for photodynamic therapy in patients with clinical manifestations of photodamage.

Presented current data on the etiology of rosacea, the main aspects of pathogenesis, clinical forms of the disease. Reflects trigger factors leading to rosacea, as well as complicating its course. Modern methods of treatment are described, including the use of new safe preparations of ivermectin and brimonidine, providing a good, lasting effect of clinical manifestations of rosacea. The role of laser technology, actively occupying a leading place in the choice of physiotherapeutic treatment method, is noted. The need for an individual approach in the choice of therapy for each patient with rosacea is emphasized.
