Development of patenting of domestic inventions for the surgical treatment of injuries of the shoulder joint

Domestic patents for inventions on surgical treatment of shoulder joint injuries are analyzed. The database of the Federal Institute of Industrial Property of Rospatent was searched. Eliminating the so-called search noise, we created an array of 215 patents for inventions for the surgical treatment of shoulder injuries (registration mainly from 01/01/1994 to 09/30/2020). The average annual number of patents for inventions was 7 1. Surgical treatment methods were presented in 74.3% of inventions, and devices (surgical instruments, endoprostheses, etc.) in 25.7%. In some inventions, both methods and devices were patented. Inventions for the surgical treatment of shoulder joint instability accounted for 23.4%, for the treatment of tendons and periarticular tissues 8.9%, for osteosynthesis of the bones of the shoulder joint 21.9%, for surgical devices and instruments 14.2%, for methods and shoulder arthroplasty devices 13.4%; other inventions (on assessment of lesions, the likelihood of a favorable outcome of surgery, pain relief, etc.) 18.2%. It is necessary to optimize the domestic inventive activity in areas of medicine to prevent a shotage of domestic medical equipment or tools in case of sanctions imposed by certain foreign states.

This paper describes the establishment of the new joint laboratory for medical materials science in Tomsk in the framework of the Technology Platform “Medicine of the Future”. The objective of this paper is to analyze the promising research areas within Medicine and Healthcare in the field of medical materials science. The study has been carried out using patent analysis and the method of critical technologies. The research object is the science and technology thematic area “biodegradable composite materials for medical applications”. According to the analysis of patent databases of the Federal Institute of Industrial Property (Russia) and the European Patent Office, the authors have made conclusions concerning the level of inventive activity with regard to “biodegradable composite materials for medical applications”, and determined the common and distinct country features of patent activity in medical materials science. Besides, the authors have suggested a new approach to improving the inventive activity in Russia and abroad, based on the method of critical technologies.

A method for identifying small innovative enterprises (SIE) that are understood as holders of a valid patent for an invention or a license to use it is proposed. The source of information for identi cation is the patent funds and the search engine of the Federal Institute of industrial property (FIPS). The methodology includes techniques for identifying SIE if patents related to their activities were issued to individuals. The results of SIE detection in three areas of technology obtained during the development of the technique are presented. It is noted that other statistical results obtained along the way are of the same, if not greater, interest for the purposes of public administration.

Abstract Developable mechanisms conform to and emerge from developable, or specially curved, surfaces. The cylindrical developable mechanism can have applications in many industries due to the popularity of cylindrical or tube-based devices. Laparoscopic surgical devices in particular are widely composed of instruments attached at the proximal end of a cylindrical shaft. In this paper, properties of cylindrical developable mechanisms are discussed, including their behaviors, characteristics, and potential functions. One method for designing cylindrical developable mechanisms is discussed. Two example developable surgical devices that exemplify these behaviors, characteristics, and functions, along with the kinematic mechanisms comprising them, are discussed in detail.

BACKGROUND: The number of reversible shoulder joint endoprostheses installed in the world at the present stage is several times greater than the number of hemiarthroplasty performed. Nevertheless shoulder arthroplasty is considered a traumatic operation and can be accompanied by a number of complications, both from the side of implants and due to the traumatic nature of the surgical technique. During surgical interventions on the shoulder joint with a wide dissection of the skin and subcutaneous tissue, iatrogenic damage to structures such as the axillary nerve, posterior and anterior arteries and veins surrounding the humerus can often be detected, which triggers a whole cascade of pathophysiological and regulatory processes in which Interventions immediately release inflammatory mediators. Therefore, orthopedic traumatologists strive to reduce the risk of intra- and postoperative complications, and it is necessary to improve the surgical technique of surgical interventions towards their less traumatic performance. AIM: Development and evaluation of the effectiveness of the use of low-traumatic surgical access when performing reverse shoulder arthroplasty. MATERIALS AND METHODS: In the period 20172020, 169 patients with various diseases, injuries of the shoulder joint and their consequences were operated on in the Department of Adult Orthopedics of the N.N. Priorov National Research Medical Center of the Russian Federation, who underwent reverse shoulder arthroplasty according to generally accepted indications. In the main group (84 patients), surgical treatment was performed using a low-traumatic surgical approach, while the control group (85 patients) underwent standard procedures. Functional, clinical and radiological results of surgical treatment of patients of the main and control groups were evaluated and compared after 3, 6 and 12 months. RESULTS: In the main group, excellent results (25 points on DASH) were observed in 73 patients, good results (2650 points) in 10 patients. In 1 patient, the results were assessed as satisfactory (5175 points). In the control group of observation, the clinical result was worse (68 excellent, 16 good and 1 satisfactory result). CONCLUSION: On the basis of the performed study, taking into account the better results in the main group of patients, the technique of low-traumatic surgical access for reverse shoulder arthroplasty can be recommended for wide use in clinical practice.

The ability to sense force in surgery is in high demand in many applications such asforce feedback in surgical robots and remote palpation (e.g., tumor detection in endoscopic surgery).In addition, recording and analyzing surgical data is of substantial value in terms of evidence-basedmedicine. However, force sensing in surgery remains challenging because of the specific requirementsof surgical instruments, namely, they must be small, bio-compatible, sterilizable, and tolerant tonoise. In this study, we propose a modular optic force sensor using a Fabry–Perot interferometer thatcan be used on surgical devices. The the proposed sensor can be implemented like a strain gauge,which is widely used in industrial applications but not compatible with surgery. The proposed sensorincludes two key elements, a fiber-optic pressure sensor using a Fabry–Perot interferometer thatwas previously developed by one of the authors and a structure that includes a carbide pin thatcontacts the pressure sensor along the long axis. These two elements are fixed in a guide channelfabricated in a 3 × 2 × 0.5 mm sensor housing. The experimental results are promising, revealinga linear relationship between the output and the applied load while showing a linear temperaturecharacteristic that suggests temperature compensation will be needed in use.

The shoulder joint is the most flexible joint in the human body which experiences high loads during certain sport activities involving overhead motion. Overhead athletes experience rapid shoulder elevation, abduction and external rotation during the movement and rapid deceleration after the movement, making them susceptible to shoulder injuries. Due to the extreme ranges of motion, overhead athletes tend to develop increased external rotation, combined with decreased range of internal rotation, emphasizing the need for balanced shoulder care program in the training process to prevent such occurrences. The aim of the research paper was to develop training regimen for injury prevention and shoulder maintenance that can be incorporated in daily training activities of the athletes in order to decrease the occurrence of injuries. Three part training program was designed consisting of specific warm up, strengthening and stabilization exercises and stretching exercises. The aim of the specific warm up was to prepare the shoulder for the training loads, mimicking the overhead movements and stimulating the stabilization in the joint. Strengthening and stabilization exercises aimed to strengthen external rotators in the shoulder and improve the stabilization of the shoulder joint by targeting specific muscles such as serratus anterior. The focus of stretching exercises was to stretch internal rotation muscles, which experience high loads in the overhead athlete's training. An effective training program that can be performed with minimal equipment and in a short time frame was designed to enable its simple incorporation into the daily training of overhead athletes.
