Inguinal hernioplasty is one of the most common planned surgical interventions. With the advent of minimally invasive technologies in herniology, a new page in the field of inguinal hernia surgery has opened. The emergence of laparoscopic techniques (TAPP) and fully pre-peritoneal surgery using the TEP method allowed us to achieve good long-term results. However, there remain unresolved questions of justifying the choice of a particular method of minimally invasive intervention in patients with inguinal hernias, as well as minimal information about the comparison of long-term results of treatment of this category of patients. As a result of the study, the results of treatment of 987 patients with inguinal hernias, operated from 2014 to 2018, were evaluated. in the clinic by the endoscopic method (319 by the laparoscopic method TAPP and 668 patients operated by the TEP method). The criteria for the comparison of endoscopic techniques of hernioplasty were: the duration of the operation, the intensity of the pain syndrome, the postoperative bed-day, the economic costs for one treated herniological patient. It was established that in the absence of significant contraindications, the operation of choice was hernioplasty according to the TER method due to the lower risk of damage to internal organs and its greater economic efficiency, while maintaining all the positive qualities of minimally invasive intervention. The average bed-day after surgery according to the TAPP method was 3.8 ± 0.2, and according to the TEP method - 2.6 ± 0.3. The complications, the frequency and the cause of relapses after endoscopic hernioplasty were studied. Analyzing long-term results, in 3 cases (0.9%) after laparoscopic hernioplasty, a relapse of the disease was diagnosed. After hernioplasty using the TEP method, relapse was diagnosed in 2 cases (0.3%). In 2 patients, the hernia recurred was due to the small size of the established mesh prosthesis (less than 10x15 cm), in 3 patients it was caused by turning the lower edge of the mesh prosthesis and was observed at the stage of mastering the technique. As a result of the study, it was found that the TEP technique, in contrast to laparoscopic hernioplasty TAPP, leads to a reduction in the postoperative hospital bed, is accompanied by a significantly lower pain syndrome and recurrence rate of the disease, and also has greater economic efficiency.

The immediate and long-term results of treatment of 30 patients with severe pain syndrome of the lumbar region, who underwent operations on denervation of DOS, were studied. The aim of the study was to evaluate the effectiveness of minimally invasive technologies for the treatment of pain in the lumbar region (denervation of DOS), to study the near and distant results of these treatment methods. Denervation DOS is an effective minimally invasive method for the treatment of facet syndrome caused by spondylarthrosis. It allows in the early and distant postoperative periods to significantly reduce the pain syndrome and improve the quality of life of patients.

The objective of this study was to conduct comparative analysis of the immediate and long-term results of laparoscopic inguinal hernioplasty performed with and without fixation of the mesh endoprosthesis.Methods and materials. The base group included 77 patients who underwent laparoscopic inguinal hernioplasty without fixation of the mesh by herniostepler. The control group included 76 patients who underwent laparoscopic inguinal hernioplasty with mesh fixation by herniostepler. To assess the postoperative period, the following criteria were studied: the severity of postoperative pain on a Visual Analog Scale; postoperative complications on the Clavien-Dindo scale; an early recurrence of inguinal hernias determined by ultrasound.Results. In both groups, the severity of postoperative complications did not exceed grade I. Among the entire list of complications, there were observed: pain in the area of surgery, an increase in body temperature above 38.5 °C, acute urinary retention, swelling and hematoma of the scrotum, seroma. There were no cases of conversion, intraoperative damage to internal organs and vascular structures. Complications in both groups do not differ significantly and do not depend on the method of laparoscopic inguinal hernioplasty: with fixation of the endoprosthesis by herniostepler or without fixation. The level of pain in the early postoperative period in patients operated by laparoscopic inguinal hernioplasty with mesh fixation was significantly higher. During control examinations of patients on the day after surgery, at the time of discharge and 6 months after surgery, no cases of dislocation of mesh endoprosthesis and relapse of inguinal hernia were detected.Conclusion. The obtained data indicate the effectiveness of laparoscopic hernioplasty in inguinal hernias with polypropylene mesh without fixing with herniostepler.

Aim. To improve the results of treatment of periarticular tissue diseases and degenerative arthropathies at the expense of differential use of modern technologies. Materials and methods. During the period of 2010–2017, examination and treatment of 10608 patients, suffering from the locomotor diseases, was carried out. There were 5792 (54,6 %) patients with degenerative arthropathies and 4816 (45,4 %) patients with tendinites and enthesites of different localization. Results. The long-term results of treatment with chondroprotector Alflutop, used in complex therapy of 1969 (33,9 %) patients with arthropathies were studied for the period from 1 to 3 years. Pain syndrome was arrested after 4–5 injections. After 8–9 injections of this preparation, no pain syndrome was revealed, movement amplitude in the joint increased, patients returned to their previous work, Lequesne index declined and varied within the range of 3–6 scores. When using hyaluronic acid, pain syndrome was arrested in most cases after 2 injections. After the third injection of preparation, no pain factor was registered, movement amplitude in the joint increased, patients returned to their previous work, Lequesne index declined and varied within 3–6 scores. The long-term results from 1 to 5 years were studied in 1179 (20,3 %) patients. Conclusions. One-year remission with combined application of chondroprotector and hyaluronic acid was 76,5 %. Two-year and longer remission was 47,6 %. Diagnostic algorithm for locomotor diseases was developed and approach to conservative therapy was differentiated.

A new minimally invasive technique is presented to improve the results and reduce the duration of treatment of patients with stenosing ligamentitis of dorsal carpal ligament (de Quervain’s disease). The high efficiency of the proposed method (on the Disabilities of the Arm, Shoulder, and Hand scales and Visual Analogue Scale) is established, comparable with de Quervain’s open ligamentotomy but with the reduction of rehabilitation period after the surgery from 16-21 days to3-4 days. Clinical approbation of minimally invasive ligamentotomy in all patients revealed excellent long-term results of treatment, with minimal risk of complications. Comparison of the results of treatment of patients of both groups showed the advantage of minimally invasive ligamentotomy before open ligamentotomy in the early postoperative period with the same long-term results, which allows patients to start their labor and professional duties in the early stages. The proposed method can be performed in an outpatient mode and does not require special post-operative treatment, as well as special instruments and equipment, which allows reducing the costs of both the patient and the medical institution.

Medical, social and economic relevance of inflammatory diseases of the woman's reproductive organs requires a very careful attitude to the diagnosis and treatment of this pathology. The number of patients with genital infections and inflammatory diseases of the pelvic organs can takes the first place in structure of gynecological morbidity, and is 60.4-65.0%, and this fact is not unique to Russia, but all over the world. Incidence rate of inflammatory diseases of the pelvic organs in the first decade of the twenty-first century is increased at 1.4 times in patients who are from 18 to 24 years old and at 1.8 times in patients aged 25-29 years. At the same time, the cost of diagnosis and treatment has increased, reaching 50-60% of the total cost of providing gynecological care for population. The inflammatory diseases of pelvic organs are a collective concept. It includes of various nosological forms. There are numerous contradictions in the views on diagnostic approaches and treatment tactics, the nature of screening and control over the long-term results of treatment, the etiological and pathogenetic significance of various microorganisms found in the genital tract in patients with inflammatory diseases of the pelvic organs. Currently, there are many opinions among specialists about diagnostic approaches and treatment tactics, the type of screening and monitoring the long-term results of treatment, the etiological and pathogenetic role of various microorganisms which can be found in the genital tract in patients with inflammatory diseases. This review presents the results of a modern approach to the diagnosis, management and rehabilitation of patients with inflammatory diseases of the pelvic organs.

AbstractTo obtain the best possible results of treatment of acquired cholesteatoma, we made a subdivision of cholesteatoma types into attic and pars tensa cholesteatomas, and subdivided the latter further into tensa retraction cholesteatoma and sinus cholesteatomas. Tensa retraction cholesteatoma is defined as arising from a retraction or perforation of the whole pars tensa, whereas sinus cholesteatoma is defined as arising from a retraction or perforation of the postero-superior part of the tensa. We present the long-term results obtained in tensa retraction cholesteatomas treated with one stage surgery from 1964 to 1980. Median observation time was 9 years, range 2 to 19 years. Sixty-one ears were treated without mastoidectomy, whereas 71 ears had canal wall-up mastoidectomy and 64 ears had canal wall-down mastoidectomy. The total recurrence rate was 13.3 per cent; 17 ears had residual cholesteatoma, and nine ears had recurrent cholesteatoma. The best results were obtained in ears with an intact ossicular chain where mastoidectomy was not performed. In 49 per cent of the cases, the cholesteatoma was confined to the tympanic cavity without reaching the aditus, antrum or mastoid process. About one-third to one-quarter of the ears had tympanoplasty only, with removal of the cholesteatoma through the ear canal.
