An analysis of the experience of medical support of troops in the wars and military conflicts shows that their characteristic feature was and remains the constant improvement of the organizational forms of medical support for the troops aimed at optimizing the medical rehabilitation of the wounded and sick. The results of the analysis of medical support of troops during the Great Patriotic War showed that the composition and content of the sanitary minimum level for servicemen should be in constant accordance with the requirements for the level of professional training of servicemen. Organization of medical care for participants of the Great Patriotic War of 1941-1945 and the Afghan conflict of 1979-1989 in the postwar period shows that the basis of all measures to monitor the health of war participants must be a system of active personal dynamic observation. Active dynamic monitoring of the state of health using automated information systems allows to solve the problem of restoring health among participants in wars and military conflicts at the optimal level, provides medical information necessary for assessing the health status of participants in wars, determining the scope and composition of medical and recreational activities. Active invitation of the patient, who is on dispensary supervision, makes it possible to conduct a comprehensive assessment of his health status. However, the effectiveness of the system of medical control over the health of war participants largely depends on the high degree of automation of information processes in the system of dynamic control over the health of a workable and efficient resource on a national scale. The implementation of the principles of dynamic monitoring of the health of participants in wars can be carried out through the practical implementation of Decision 36 of the Plenum of the Academic Medical Council with the Chief of the Main Military Medical Directorate of the Ministry of Defense of the Russian Federation (1996). Special attention was paid to the necessity and urgency of the development and operation of an automated information system on the wounded and sick on the basis of the Military Medical Museum. At the same time, it is necessary that its electronic information module is located in the institution where the wounded and sick war veterans are kept in the hospital. At present, such an institution in the Armed Forces of the Russian Federation is an archive of military medical documents of the Russian Military Medical Museum.

Most of the military conflicts in the last decades with participation of the Armed forces of our country took place in the mountain landscape, namely Afghanistan, counterterrorist operations in the Northern Caucasus, peacemaking missions in South Ossetia and Abkhazia. The warriors passed through «trouble spots» know that a war in mountains is not the same as a war in plain and it requires special preparation. It is necessary to take into consideration factors of climate and geography which influence medical provision in mountains as well as sanitary casualty’s formation and mode of evacuation and treatment. Based on literature data, the historical facts of warfare conduction in mountains were presented. Factors of climate and geography adversely affected the human health during warfare conduction in mountains. The analysis of the mountain training medical provision organization was carried out. Based on that some proposals for improvement of mountain training medical provision of military personnel of the Armies forces of the Russian Federation were formulated

We studied the medical and demographic indicators in the Republic of Sakha (Yakutia) for the last 20 years (1998–2018). A decrease in the population by 4,7 % (р<0,01) was revealed in the Russian Federation in the period 1998–2008, 3,4 % decrease, followed by its growth by 2,8 % (р<0,01); a decrease in the number of rural population in the republic and an increase in the urban population were observed. By the beginning of 2019, in comparison with the 2003 data, an increase in the population by 1.9% (р<0,01), a decrease in the number of able-bodied people in the Republic of Sakha (Yakutia) at the beginning of 2019, as compared to 1998, were revealed, by 8,2 %, in the Russian Federation – by 4,7 % (р<0,01). In this work, the author examined in detail the problematic issues of organizing safe conditions during the protection of convicted and detained persons in state and municipal medical institutions. For this, the author thoroughly studied and analyzed the regulatory legal acts of the Russian Federation protecting the life and health of each person, since the data of the legal acts apply to persons in prison to the same extent. In my opinion, the problem of this article in prisons is relevant today, since in its practical activities the administration of the institution often faces a question of this nature. I believe that the relevance of this article gives rise to many gaps that are inherent in penal correction systems, such as: medical support of Penal System, downsizing, poorly organized cooperation with state medical institutions, which was also considered in this paper. Having analyzed the above-mentioned gaps inherent in the Penal System, the author proposed his own solutions, which, in his opinion, will solve both the main problem, the organization of safe conditions during the protection of convicted and detained persons in medical institutions, and related gaps in general.

The article is devoted to the work of the Moscow Research Institute of Psychiatry to improve psychiatric care for patients with psychotic disorders. An important feature of this work was an integrated approach, in which the clinical picture of the disease was assessed in close connection with the patient's personal and psychological characteristics, social conditions of his life, therapeutic opportunities, rehabilitation potential and organizational structure of care. The article reflects the results of many years of work of the department of outpatient psychiatry and the organization of psychiatric care under the guidance of Professor I.Ya. Gurovich. The results of scientific research carried out by the staff of the institute in a traditional humanistic manner are presented. The translational nature of the research is emphasized by its inextricable link with clinical and social approaches. As a result of many years of work, a concept was developed to provide assistance to various groups of patients, starting with the first manifestations of the disease and ending with cases of long-term chronic disorders with a pronounced level of social maladaptation. As a result, a whole spectrum of new organizational forms of psychiatric care was proposed, such as departments (clinics) of the first psychotic episode, medical rehabilitation departments, assertive community treatment departments, designed for the most difficult patients. These organizational forms were fixed in the regulatory documents of the Ministry of Health and Social Development of the Russian Federation. To date, the above departments have been established in psychiatric institutions in many regions of the Russian Federation. Further development of this area is associated with neurobiological research to identify complex biomarkers of psychotic spectrum disorders. Thus, the research carried out at the present time preserves the tradition of an integrated clinical and social approach to the study of mental disorders. It is shown that an important advantage of this approach is their translational nature.

The article reviews creation of the departmental archive of the National Commissariat of the Navy (1937) and its functioning to this day. ‘The Statute of the Branch of the Central Archive of the Ministry of Defense of the Russian Federation (Archive of Navy)’ was adopted on February 20, 2013. According to this document the Archive of Navy became a subdivision of the Central Archive of the Ministry of Defense, deployed separately and functioning independently. The departmental archive began its acquisition in September 1940. Satisfactory execution of functions by Archive was impeded by multiple changes in the structure of the Directorate of the Peoples’ Commissariat of the Navy, which complicated processing of documents entering the storage. Tasks of the Archive were reduced to the following: to control files condition and document destruction; to compile lists of documents with terms of their storage; to inspect the state of archiving in the Navy; to advise archives and records management offices of central directorates (departments) of the Peoples’ Commissariat of the Navy on formation and registration of files and their transfer to archive; to enter documents of the central directorates (departments) on storage; to track and safeguard documents. On the eve the Great Patriotic War transfer of document from fleet, flotillas, and naval bases was in its initial stage. The first months of the Great Patriotic War prompted evacuation of archival fonds from Moscow to Ulyanovsk (August 1941). By January 1945 these numbered 26550 files and 1234 bags of unsorted documents. At the end of war the Archive was relocated from Ulyanovsk to Leningrad, and then to Kronstadt (1947). In 1950s the Archive continued moving to new places — to Pushkin, to Leningrad, to Gatchina (1961). The fonds of the Archive store unique documents of the Peoples’ Commissariat and Ministry of the Navy, governing bodies under the Commander-In-Chief of the Navy, research establishments, Navy schools, river flotillas, materials on ships and submarines, air force, marines, coastal and anti-aircraft defense, rear, hydrographic, medical and sanitary, and other services. Of great interest for researchers are documents of the General Staff of the Navy.

Organizational issues of the interdepartmental application of telemedicine technologies in the provision of medical care to patients are considered. The most significant provisions of the concept of interdepartmental application of telemedicine technologies in the organization and provision of medical care to patients in critical conditions are presented. In addition, data on telemedicine consultations on federal districts of the Russian Federation performed by the federal level of health care in 2017 are shown, the frequency of emergency telemedicine consultations is revealed depending on the disease profile of patients or the specialty of a consulting physician. Prospects for the further introduction of telemedicine technologies into daily practical activities and the possibility of their adaptation to crisis situations and military conflicts are substantiated. It has been established that in various natural disasters, accidents, catastrophes, terrorist acts and during military conflicts with a large number of victims, successful organization of medical care and medical evacuation is possible only when combined with the efforts of the Ministry of Health, the Ministry of Defense, the Ministry of internal affairs and the Ministry of the Russian Federation for Civil Defence, Emergencies and Elimination of Consequences of Natural Disasters. Thus, the adequate development of telemedicine systems will increase the level of their rational and effective interdepartmental use for the benefit of saving people as Russia’s human capital.
