Levocetirizine is an effective Hj-blocker in complex treatment of various allergic patients. Allergic rhinitis is often part of a systemic allergic inflammation associated with other allergic diseases, therefore systemic antihistamines are the drugs of choice in it’s treatment.

In the article the actual problem of modern medicine – treatment of patients with allergic rhinitis (AR). Given the prevalence of AR are discussed classification issues, the etiology and pathogenesis of the disease. We consider the two-generation antihistamines in the treatment of patients with AR. Based on the data of several studies demonstrated benefits of antihistamines II generation, particularly levocetirizine.

Research оbjective.To determine the effect of ozone therapy on clinical indicators and the state of immunity in children with moderate persistent allergic rhinitis. Material and methods. The study included children aged 5-10 years with moderate persis-tent allergic rhinitis, which were divided into two groups depending on the therapy. The first group of patients with allergic rhinitis received complex conventional therapy, the second group of patients with allergic rhinitis received complex treatment in combination with ozone therapy. Clinical parameters were studied in patients with allergic rhinitis, parameters of im-munological reactivity were investigated during periods of exacerbation of the disease and clinical remission. Results. It was found that the inclusion of ozone therapy in the complex treatment of patients of the second group ensured a faster onset of complete clinical remission (3.7 days earlier than in the first group of patients) and normalization of most immunity parameters, and also increased the expression of toll-like receptors on leukocyte cells. The duration of complete clinical remission in the group of patients with allergic rhinitis who received complex treatment in combination with ozone therapy (9.3 + 2 months) more than doubled (2.4 times) its duration in the group of patients with allergic rhinitis who received complex conventional therapy (3.9 + 0.3 months).

Objective. To determine the effect of ozonotherapy on the clinical indicators and immunological reactivity state in children with a moderate course of persisting allergic rhinitis. Materials and methods. Children aged 510 years with a moderate persisting allergic rhinitis were under observation. They were divided into two groups depending on the therapy applied. Group I of allergic rhinitis patients received a complex generally accepted therapy, group II of allergic rhinitis patients a complex therapy associated with ozonotherapy. Clinical indices and immunological reactivity parameters were studied during exacerbation and clinical remission in allergic rhinitis children. Results. Ozonotherapy, included into a complex treatment of group II patients, was established to promote a faster occurrence of full clinical remission and normalization of most parameters of immunological reactivity. Duration of a full clinical remission in the group of allergic rhinitis patients, who received a complex therapy associated with ozonotherapy, more than twofold exceeded its duration in the group of allergic rhinitis patients receiving a complex generally accepted therapy. Conclusions. The data obtained indicate high clinical, immunomodulating and antirelapsing effect of ozonotherapy that allows recommending its wide use in complex of medical measures among children with persisting allergic rhinitis.

Daratumumab, a CD38-directed monoclonal antibody indicated for multiple myeloma treatment in adult patients, is associated with a high incidence of infusion-related reactions (IRRs). Due to CD38 receptor presence in the lungs, many reactions present similarly to asthma or allergic rhinitis. Montelukast, a leukotriene receptor antagonist, has been hypothesized to reduce daratumumab IRRs due to its efficacy in treating allergic rhinitis and asthma and the presence of leukotriene receptors in the lungs. Recently published data reported daratumumab can be safely administered via rapid rate protocol that reduces infusion time from 195 min to 90 min after completion of two doses. This retrospective, observational cohort study examined 73 patients who received daratumumab in the outpatient setting between December 2015 and April 2020. Patients were included if they were 18 years or older, had an International Classification of Disease (ICD)-10 diagnosis code for multiple myeloma, and received daratumumab intravenously. The primary outcome was a comparison of IRRs between those who did and did not receive montelukast. Secondary outcomes included IRR symptoms, rescue medications utilized for IRRs, and rapid rate administration outcomes. Montelukast use was associated with a lower rate of IRRs (44.4% vs. 65.2%, p = 0.044). Pulmonary IRR symptoms were more common in those who did not receive montelukast. Rapid rate administration of daratumumab did not lead to any IRRs. Adding montelukast as a pre-medication for daratumumab infusions led to a reduction in IRRs, and rapid rate administration was found to be safe after completion of two full doses of daratumumab.

The problem of diagnosis and treatment of allergic rhinitis today is particularly relevant, due to the increasing prevalence of that  disease both in Russia and abroad. The three highlighted directions of the treatment are: immunotherapy, elimination therapy and  drug therapy. The basis of the pharmacotherapy of allergic diseases are medicines, which are directed to the main pathogenetic  mechanisms of allergic inflammation. Among the drugs recommended for the treatment of allergic rhinitis, antihistamines of the  second generation play an important role.

The article presents the literature data and own observational data on clinical efficacy and safety of levocetirizine in the treatment of allergic diseases such as allergic rhinitis, bronchial asthma, allergic dermatoses.

Eosinophils are important to the pathogenesis of allergic asthma, and are increased in bronchoalveolar lavage within four hours after bronchoprovocation of allergic asthmatic patients, and remain significantly increased up to 24 hours later. While the components of human eosinophil granules have been recently isolated and purified, the mechanisms of degranulation have yet to be elucidated.We obtained blood from two volunteers who had a history of allergic rhinitis and asthma and a positive skin test (5x5mm wheal) to Alternaria and Ragweed. Eosinophils were obtained using a modification of the method described by Roberts and Gallin.

A group of sophisticated listeners judged the nasal resonance characteristics of normal children versus children evidencing selected rhinologic disorders under three speaking conditions. Results showed that perceptions of denasality are influenced by both speakers and speaking tasks. That is, children with allergic rhinitis and edemic adenoids were perceived as being denasal when they produced VCV utterances and recited sentences. However, their resonance characteristics were deemed normal for vowel productions. Interestingly, children with severely deviated septums were judged to have normal nasal resonance under all speaking conditions. Clinical implications are discussed.
