Healthcare expenditures in the United States are growing at an alarming level with the Centers for Medicare and Medicaid Services (CMS) projecting that they will reach $5.7 trillion per year by 2026. Inflammatory diseases and related syndromes are growing in prevalence among Western societies. This growing population that affects close to 60 million people in the U.S. places a significant burden on the healthcare system. Characterized by relatively slow development, these diseases and syndromes prove challenging to diagnose, leading to delayed treatment against the backdrop of inevitable disability progression. Patients require healthcare attention but are initially hidden from clinician’s view by the seemingly generalized, non-specific symptoms. It is imperative to identify and manage these underlying conditions to slow disease progression and reduce the likelihood that costly comorbidities will develop. Enhanced diagnostic criteria coupled with additional technological innovation to identify inflammatory conditions earlier is necessary and in the best interest of all healthcare stakeholders. The current total cost to the U.S. healthcare system is at least $90B dollars annually. Through unique analysis of financial cost drivers, this review identifies opportunities to improve clinical outcomes and help control these disease-related costs by 20% or more.

This chapter describes the Gulf of Mexico (GOM) region and the major issues facing this marine fisheries ecosystem, and presents some summary statistics related to the 90 indicators of ecosystem-based fisheries management (EBFM) criteria. The region contains high numbers of marine species comprising commercially and recreationally important invertebrate (e.g., penaeid shrimp, blue crab, eastern oyster) and finfish (e.g., red snapper, grouper, red drum, pelagic sportfishes) fisheries, which contribute heavily to national landings and seafood supply. The northern GOM contains one of the nation’s largest marine economies (among the eight U.S. regional marine ecosystems), which is dependent on offshore mineral extractions, tourism, marine transportation, living marine resources (LMRs), and other ocean uses. The GOM provides critical social and economic benefits to the region and the nation, is a region with high numbers of managed species, yet exploitation of these resources and an increasing human population makes the GOM an area subject to significant natural and human stressors, including the highest number of hurricanes in the U.S. Atlantic region, large expanses of hypoxic bottom water, overfishing, and major oil spills like the 2010 DWH event.

AbstractAs countries continue to implement sustainable and renewable energy goals, the need for affordable low-carbon technologies, including those related to offshore wind energy, is accelerating. The U.S. federal government recognizes the environmental and economic benefits of offshore wind development and is taking the necessary steps to overcome critical challenges facing the industry to realize these benefits. The U.S. Department of Energy (DOE) is investing in buoy-mounted lidar systems to facilitate offshore measurement campaigns that will advance our understanding of the offshore environment and provide the observational data needed for model validation, particularly at hub height where offshore observations are particularly lacking. On behalf of the DOE, the Pacific Northwest National Laboratory manages a Lidar Buoy Program that facilitates meteorological and oceanographic data collection using validated methods to support the U.S. offshore wind industry. Since being acquired in 2014, two DOE lidar buoys have been deployed on the U.S. east and west coasts, and their data represent the first publicly available multi-seasonal hub height data to be collected in U.S. waters. In addition, the buoys have undergone performance testing, significant upgrades, and a lidar validation campaign to ensure the accuracy and reliability of the lidar data needed to support wind resource characterization and model validation (the lidars were validated against a reference lidar installed on the Air-Sea Interaction Tower operated by the Woods Hole Oceanographic Institution). The Lidar Buoy Program is providing valuable offshore data to the wind energy community, while focusing data collection on areas of acknowledged high priority.

The target audience is industry, state agency, and public stakeholders involved in decisions and policy making related to the use of neonicotinoid insecticides in the U.S. ornamental plants industry.

After surveying the historical and legal background of the return migration to Israel, the empirical investigation reported in this paper—based on a random sample of Israeli citizens who returned during 1970 from the U.S., Canada, and France—addressed itself to the subject of motivations to migrate. The analysis has shown that the attraction that Israel has for its citizens residing abroad is primarily associated with patriotic commitments and family ties, whereas forces pushing people from other countries play only a minor role in return migration to Israel. The Israeli Government plan of granting special economic benefits to returnees during 1968–1970 did not achieve its designated purpose.

U.S. regions are expected to follow the national trend towards investment in renewable energy as part of their electricity portfolio. The progress of energy portfolios that typically involves traditional methods, such as centralized nuclear and coal-fired generation, and towards cleaner- and renewable-source generation will impact economic growth and public health. Renewable electricity production must strike a balance among cost, reliability, and compatibility. The economic and health benefits obtained from developing an efficient energy portfolio make renewable energy alternatives an important consideration for regions endowed with natural resources. A portfolio mix of production method that considers the economic benefits while limiting adverse health and environmental impacts is attractive. This research proposes a System Dynamics simulation framework to support policy-making efforts in assessing the impact of energy portfolios. The authors demonstrate the utility of the framework by means of analyzing data that pertain to the U.S. Hampton Roads - Peninsula Region.

The financial analyses presented in Chapters 7 and 8 indicate that commercial development of OTEC will have a significant impact on the economics of U.S. energy production and use. Two scenarios for commercial development are examined in this section: 1. Development of OTEC methanol capacity sufficient to replace all U.S. gasoline produced from imported oil. 2. Development of OTEC ammonia capacity sufficient to replace all gasoline used in U.S. transportation. Commercialization of this option implies a project goal to produce methanol plantships with enough total methanol capacity to replace the gasoline used in the United States that is now produced from imported petroleum, 47 billion gallons of gasoline in 1990 (DOE/EIA, 1990). This would require a total of 427 200-MWe plantships, each producing 199 million gallons of methanol per year (1.8 gallons of methanol give the same automobile mileage as 1 gallon of gasoline. We assume financing based on an initial nominal plant investment of $960M (1990$) and an eighth plant investment of $664M. With repeated manufacture, the cost will be reduced to $438M for the 427th plantship, assuming that an experience exponent of 0.93 applies for all production of identical plantships after the first three. The average plant investment for the total production is then $507M. If financial support is maintained to complete the program, the year 2020 is a reasonable target date for achieving the full fuel production capacity. This implies construction of OTEC plantships at an average rate of 17 per year after commercial production is established. This rate could be accommodated in U.S. shipyards with feasible modifications to satisfy specific OTEC requirements. The U.S. shipbuilding facilities are discussed in Section 4.1. In addition to the investments required for OTEC, methanol automobiles must be in production, and distribution systems for methanol must be installed. The associated costs must be included in the financial analysis. Offsetting these costs are the savings resulting from: 1. Large improvements in the U.S. balance of trade through elimination of oil imports. 2. Tax receipts accruing from reinvigorated U.S. shipbuilding and associated manufacturing industries. 3. Economic benefits of stabilized world fuel prices.

This paper applies game theory to explore the economic incentives facing foreign nations sheltering terrorists. The players are a nation that has been a target of international terrorist activities and a nation that is a host (willing or unwilling) of international terrorists. The economics of multinational terrorism involve the costs to host nations of sanctions imposed by the U.S. and others, as well as the economic benefits transferred to the host nations by terrorist groups that they shelter. The main result shows that coalitions between international terrorists and their host nations allow the latter to sell licenses to terrorists thereby frustrating the counter-terrorist activities of victim nations.

Based on a systematic qualitative analysis of articles published by The New York Times (2009–2017), this article presents the main media frames that support the access to government-sponsored health care by undocumented immigrants, just before and after passage of the U.S. Affordable Care Act in 2010. Under the umbrella of “selective inclusion,” this study highlights a “compassionate frame” that conveys sympathy toward severely ill, undocumented immigrants. This approach is reinforced by a “cost-control” frame that underlines the economic benefits of providing health care to the undocumented immigrant population in the United States. Supported by both humane and market-based approaches, these frames make a compelling case for the inclusion of particular groups into the U.S. health care safety net. Ultimately, these findings contribute to our understanding of the media framing of undocumented immigrants’ right to health care on the basis of deservingness.
