Immediate Implant Placement: A Case Series

Abstract A major drawback of implant therapy is the comparatively long healing period involved. Early and immediate loading of dental implants can significantly decrease the treatment time and thus result in an increase in patients' comfort. In this report, 3 cases of placement and immediate loading of the final implants are presented. In 2 of these cases, where preliminary augmentation was necessary, provisional implants were placed after the procedure and immediate provisional dentures were delivered. With the exception of 1 case, in which 1 implant failed, no adverse events were observed. All cases are in function now for 3 to 5 years after implant placement. The observations from this limited number of cases confirm that immediate loading of implants could be a viable treatment option.

Summary Implant placement in the esthetic zone is a complex procedure and requires a restoration-driven approach. Proper selection of patients and implant together with individual assessment of the risk of esthetic complications are very important. Correct 3D-implant positioning and sufficient bone volume should provide long-term esthetic and function. Esthetic region is a zone in which expectations and possibilities collide. Clinician should bring the important decision on the appropriate time of implant placement. Immediate implant placement is particularly challenging in the esthetic zone. Patient desire for reduced treatment time should be weighed against the possible risk factors. Protocol of immediate implant placement in conditions of unfavourable gingival biotypes, the lack of bone or soft tissue in patients with a high smile line lead to esthetic failure which is very important in the esthetic region.

SUMMARYObjective: The aim of this study was to evaluate the outcome of immediate postextraction implant placement. Immediate placement of dental implants have been claimed of the potential advantages such as reductions in the number of surgical interventions, a shorter treatment time, an ideal 3-dimensional implant positioning, the presumptive preservation of alveolar bone at the site of the tooth extraction and soft tissue aesthetics.Method: In this case series we reported to extract 15 teeth in 12 patients (8 males; 4 females, mean age: 46.08 years) and replace the teeth with implants immediately.Results: There were no signs of inflammation or infection and none of the patients had complaints subsequently. All implants were osseointegrated at the time of abutment connection. Postoperative healing was uneventful in all of the patients. No complications were observed.Conclusion: Within the limits of the present study, immediate implant placement was a predictable treatment.

INTRODUCTION: Immediate dental implants have greatly reduced the treatment time and the number of surgical interventions. Recently it has been noted that this treatment modality can be used in aesthetically demanding cases especially the anterior maxilla. CASE REPORT: In the present case report a 24 year old male patient reported to our institution with fractured upper front teeth. After careful examination and treatment planning immediate implant treatment was initiated. The teeth were extracted atraumatically. We placed three implants into the extraction sockets. The prosthetic rehabilitation was done with metal ceramic crowns. CONCLUSION: It was found that the immediate implant therapy has several advantages such as reduced treatment length, preservation of soft and hard tissues surrounding the implant and reduced number of operations. Immediate implant treatment therefore has a great future in the treatment of aesthetic zones.

Abstract The use of dental implants to support mandibular or maxillary overdentures is a widely used treatment modality. Advantages are an increase in retention, an increase in chewing ability, and easy access for oral hygiene procedures. While telescopic and conical crowns have been used for decades to connect natural teeth to overdentures, not many cases have been reported in the literature of telescopic crowns placed on implants to support overdentures. This article describes 7 patients with overdentures supported by telescopic crowns who received 65 implants (ITI Straumann). The cases presented in this report have been in function for up to 4.5 years. During that time no adverse events were reported. The use of telescopic crowns as attachments for implant-supported overdentures may be a viable treatment option.

ABSTRACT The marked rise in patients’ demands as regards quality of life and a good appearance makes it mandatory for the Prosthodontist to provide a dental prosthesis with optimal functional, esthetic and physiological requirements. Recent researches in dental implants advocate precise preparation of the implant bed and an adequate primary stability for more vital bone to be in contact with the implant interface facilitating immediate loading. Improvements in implant designs and surface modifications have favored reduced treatment time.
