AbstractA new family of polyanhydrides has been developed which can be cured photochemically to produce degradable networks. These degradable polyanhydride networks can be useful in orthopedics as bone cements and for drug delivery. This system, which is a semi-interpenetrating network (semi-IPN), has been evaluated for tissue compatibility in subcutaneous sites in rats and shown to undergo degradation by surface erosion. It was observed that the inflammatory response to the semi-IPN implants was minimal at both short (3, 6 weeks) and long (28 weeks) time points and the fibrotic response was largely absent throughout the duration of this study. Furthermore, excellent tissue infiltration and integration with good neovascularization was observed around the semi-IPN implants. The osteocompatibility and osteoconductive properties of this system have been evaluated in a longitudinal defect model in the articular surface of the distal femur in adult rabbits. This study compared the semi-IPN implants to control groups consisting of unfilled defects and PLA implants. Histological evaluation (H&E and Trichrome Masson staining) of the implant site revealed that the semi-IPN implant is osteocompatible and well tolerated by the surrounding cancellous bone and marrow constituents. The tissue reaction to the implant is characterized by an ingrowth of vascularized connective tissue and a mild fibrous capsule. Furthermore, partial to complete closure of the articular surface with fibrocartilage and fibrous tissue was observed in all experimental animals containing the semi-IPN polymer implants.

AbstractBackground:Petrous apex cholesterol granulomas are expansile, cystic lesions containing cholesterol crystals surrounded by foreign body giant cells, fibrous tissue reaction and chronic inflammation. Appropriate treatment relies on an accurate radiological diagnosis and an understanding of the distinguishing radiological features of relevant entities in the differential diagnosis of this condition.Methods:Firstly, this paper presents a pictorial review of the relevant radiological features of petrous apex cholesterol granuloma, and highlights unique features relevant to the differential diagnosis. Secondly, it reviews the histopathological and radiological findings associated with surgical drainage of these lesions.Results:Radiological features relevant to the differential diagnosis of petrous apex cholesterol granuloma are reviewed, together with radiological and histopathological features relevant to surgical management. Following surgical management, histopathological and radiological evidence demonstrates that the patency of the surgical drainage pathway is maintained.Conclusion:Accurate diagnosis of petrous apex cholesterol granuloma is essential in order to instigate appropriate treatment. Placement of a stent in the drainage pathway may help to maintain patency and decrease the likelihood of symptomatic recurrence.

Many researches focused on laser therapy of wound healing in different animal models due to the lack of a standard protocol in the application of such phototherapy. Objective:  To study the effects of 810nm laser at a constant irradiance of 41.63 mw/cm2 and exposure (illumination) time of 5,15  minutes on wounds created on Albino mice (BALB/c).      Skin wound with elliptic shape and full thickness was created on the dorsal side of  ‘45 mature male albino mice. Irradiated animals were divided into two main groups based on irradiation time, the first was irradiated for 5 min and the second for 15 min, each was subdivided into three subgroups (n=5) according to number of treatment days (3, 5 and 10 days).   Both treated and respective control (n=15) subgroups were sacrificed on days 3, 5 and 10 posttreatment.  Laser therapy was applied using a 810 nm diode laser with a continuous wave, an output power of 400 mw, and irradiance of 41.63. The 5 min dose was 12 .5 J/cm2, whereas the 15 min dose was 37.4 J/cm2. The shape of the laser beam was fitted with a convex lens as ‘beam expander’ to irradiate a circular area of 3.4 cm diameter. Laser therapy was started after surgery and repeated for 3, 5 and 10 days, while its effects were examined by histological evaluation. Results:  At day 3 of treatment with near infrared 810nm laser at doses of 12.5J/cm² and 37.4J/cm², there was no evidence of wounds healing in irradiated groups which showed no differences with the respective control groups. At day 5 of treatment, the results showed an important increase in the scores of the parameters of wound healing (formation of granulation tissue and collagen deposition) in the irradiated groups. Near infrared 810nm laser had photobiostimulation effects on wound healing at irradiance of 41.63mW/cm² and doses of 12.5J/cm² for 5 minutes and 37.4J/cm² for 15 minutes exposure time. A complete picture of wound healing response appeared in all irradiated groups within 10 days of treatment, as expressed by complete ‘re-epithelialization’, moderate granulation tissue formation, and presence of collagen fibers, while incomplete wound healing response was observed in un-irradiated control groups within the same period. The study showed that 810 nm laser therapies had significant effects on wound healing, especially at a dose of 37.4J/cm².

Background. The reconstruction of long bone defects that occur after resection of tumors is a problem that requires constant study. Bone allografts are often used in this scenario. Unfortunately, while they are prepared, allografts partially lose their strength and osteoinductive properties; their survivability in oncological patients is only 40% after 10 years. This is why the search for superior allograft treatment methods and the study of allograft remodeling and incorporation in oncological patients, whose state has been affected by radiation or chemotherapy, is an area of interest. Purpose – study the structure of bone tissue in the distal metaphysis of a rat’s femur after bone allograft implantation (sterilized using gamma radiation or antibiotic saturation) and post-operative intraperitoneal cisplatin injection. Materials and Methods. Experiments were performed on 20 male white rats aged 5–6 months at the beginning of the experiment and weighed 365.8 ± 6.4g. All rats underwent a surgery that created a defect in the distal metaphysis of the femur which was filled with a bone allograft treated with gamma radiation (Control-1 and Experimental-1 groups) or saturated with an antibiotic (Control-2 and Experimental-2 groups). 14 days after allograft implantation, animals from the control groups received an intraperitoneal injection of 2.0–2.4 ml of 0.9% NaCl, while animals from the experimental groups received 2.5mg/kg of cisplatin. Histological analysis and histomorphometry were completed 30 days after the surgery. Results. 30 days after the operation, the smallest relative area of bone tissue (11.79%) was observed in rats from the Experimental-1 group, with gamma radiation treated allografts and post-operative intraperitoneal cisplatin injections. A somewhat higher value was found in the Experimental-2 group (antibiotic saturation + cisplatin) – 31.64%. In the control groups, (intraperitoneal injection 0.9% NaCl), the relative area of bone tissue was 16.7% (Control-1, gamma radiation treatment) and 58.09% (Control-2, antibiotic saturation). The relative area of fibrous tissue was the largest in the Experimental-1 group – 31.55% and the smallest in the Control-2 group – 12.79%. Conclusions. Allograft remodeling occurs along with the formation of bone and fibrous tissue when allografts are used to fill defects in the distal femoral metaphysis of rats, However, the relative percentages of those tissues depend on the allograft sterilization method and the use of cytostatic agents. The largest relative percentage of bone tissue (58.09%) was obtained using an allograft saturated with antibiotics and without the administration of cisplatin. The smallest (11.79%), on the other hand, occurred in gamma radiation treated allografts with cisplatin injected intraperitoneally after the operation

PURPOSE: To evaluate the histological features in lungs, peritoneum and liver of rats subjected to fecal peritonitis and treated with peritoneal lavage with 0.2% ropivacaine. METHODS: Twenty Wistar rats were subjected to laparotomy 6 h after the fecal peritonitis induction with autogenous stool. Rats were randomly distributed into 4 groups: I - (n=5) Control, no treatment; II - (n=5) Drying of the abdominal cavity; III - (n=5) Abdominal cavity lavage with 3 ml 0.9% saline solution and drying; and IV - (n=5) Abdominal cavity lavage with 3 ml 0.2% ropivacaine and drying. The animals that died underwent necropsy, and the surviving ones were subjected to euthanasia on the 11th day post-surgery. Fragments of liver, lungs and peritoneum were removed for histological evaluation. RESULTS: The animals that received peritoneal lavage (groups III and IV) showed greater survival than the drying and control groups. Lavage with ropivacaine prevented death during the observed period. Peritoneal lavage with ropivacaine maintained the architecture of the lung, peritoneum and liver without any important histological alterations. The histopathological findings analyzed correlated with greater survival of group IV. CONCLUSION: Treatment of fecal peritonitis in rats with peritoneal lavage using 0.2% ropivacaine demonstrated a reduction in histopathological alterations related to inflammatory response and sepsis.

Background Repair of chondral injuries using cartilage chips has recently demonstrated clinical feasibility. Autologous platelet-rich plasma (PRP) is a potential promising technique for improving healing response during cartilage repair. Purpose To assess the cartilage repair tissue quality after autologous cartilage chips treatment (CC) with and without repeated local injections of PRP for the treatment of full-thickness focal chondral defects of the knee. Materials and Methods Two full-thickness chondral defects (Ø = 6 mm) were created in the medial and lateral trochlea facets of each knee in 6 skeletally mature Göttingen minipigs. The 2 treatment groups were (1) CC with 1 weekly PRP injection for 3 weeks ( n = 12) and (2) CC alone ( n = 12). The animals were euthanized after 6 months. Samples of whole blood and PRP were analyzed for concentrations of platelets and nucleated cells. The composition of the cartilage repair tissue was assessed using gross appearance assessment, histomorphometry, and semiquantitative scoring (ICRS II). Results Histological evaluation demonstrated no significant difference in the content of hyaline cartilage (CC + PRP: 18.7% vs. CC: 19.6%), fibrocartilage (CC + PRP: 48.1% vs. CC: 51.8%), or fibrous tissue (CC + PRP: 22.7% vs. CC: 21.8%) between the treatment groups. Macroscopic evaluation did not demonstrate any difference between groups. Conclusions PRP injections after CC in the treatment of full-thickness cartilage injuries demonstrated no beneficial effects in terms of macroscopic and histologic composition of cartilage repair tissue.

We have developed a porous titanium implant sintered with spacer particles (porosity = 50 %, average pore size ± standard deviation = 303 ± 152 !m, yield compression strength = 100MPa). This porous titanium was successfully treated with chemical and thermal treatment that gives a bioactive micro-porous titania layer on the titanium surface, and it is expected as effective biomaterial for biological fixation on load bearing condition. In this study, ten adult female beagle dogs underwent anterior lumbar interbody fusion at L6-7 using either BT-implant or non-treated implant (NT-implant), then followed by posterior interspinous wiring and facet screw fixation. The radiographic evaluations were performed 1, 2 and 3 months postoperatively using X-ray fluoroscopy. Animals were sacrificed after 3 months postoperatively, and fusion status was evaluated by manual palpation. Histological evaluation was also performed. Both histological and radiological evaluation revealed that interbody fusion was achieved in 5 of 5 dogs (100%) in BT-group and 3 of 5 dogs (60%) in NT-group. In BT implants, we could observe a large amount of new bone formation from periphery to the center of the implant, whereas in NT implants, fibrous tissue formation was still observed even in the implants with successful fusion. The results of this study indicate that porous bioactive titanium implant will represent a new osteoconductive biomaterial with improved fusion characteristics.

This study aimed to analyze the tissue reaction caused by carvacrol paste associated or not with laser photobiomodulation (LPBM) at λ660 nm in the subcutaneous tissue of rats. Sixty Wistar rats were divided into four groups and they received the following interventions: subcutaneous implantation of empty polyethylene tubes (CTR), implantation of tubes containing carvacrol paste (CVC), implantation of empty tubes and LPBM (LLLT), and implantation of tubes containing carvacrol paste and LPBM (CVCLT). The animals were euthanized at three, eight, and 15 days after surgery. The inflammatory reaction and fibroplasia were analyzed histomorphometrically. Significant differences among the groups were determined by ANOVA and Tukey's test (p<0.05). In the 3-day period, the CVCLT group had low inflammatory infiltration (p<0.01). In the 8- and 15-day periods, the LLLT and CVCLT groups presented a low amount of lymphocytic inflammatory infiltrate (p<0.01 and p<0.05). Regarding the formation of fibrous tissue, the CVC group had the highest formation of type III collagen in the 8-day period (p<0.001). In the 15-day period, the CVCLT group had a lower formation of type I collagen than the CTR and LLLT groups (p<0.05). The use of the carvacrol paste associated with photobiomodulation optimizes the inflammatory period and tissue repair.
