Intrathecal methotrexate chemotherapy for leptomeningeal carcinomatosis

Lung and breast cancer can give meningeal metastases. Clinical manifestations of leptomeningeal carcinomatosis include all forms of defect of the central nervous system depending on the localization of carcinomatous foci. Diagnosis is based on the detection of carcinomatous cells by the cytological examination of the cerebrospinal fluid. Without treatment the prognosis is limited to only some weeks or months. In case the meningeal carcinomatosis is related to breast cancer, intrathecal methotrexate chemotherapy may allow a significant survival benefit and improve the quality of life in about half of the patients. The authors present the history of two patients. The first patient, a 69-year-old woman was diagnosed with locally invasive triple negative breast cancer with pulmonary and cerebral metastases. Complete radiological remission of the clinically asymptomatic cerebral metastases was detected under systemic chemotherapy with carboplatin-docetaxel (75 mg/m2). Later, the patient received whole brain radiotherapy and a second line of chemotherapy. The overall survival was 20 months from the diagnosis of cerebral metastases with conservation of partial autonomy. The second patient, a 57-year-old woman was diagnosed as having hormone sensitive lobular breast cancer with leptomeningeal, lymphonodular and multiple osseal metastases. Before the appearance of the lymphonodular metastasis the patient received intrathecal methotrexate chemotherapy for the leptomeningeal carcinomatosis. Her neurological symptoms completely disappeared. At the onset of the lymphonodular metastasis systemic chemotherapy with ifosfamide (1000 mg/m2, D1–3) – etoposide (100 mg/m2, D1–3) was started allowing complete clinical remission of the lymphadenomegaly and stability of the asymptomatic neurological status. The overall survival was 13 months from the diagnosis of leptomeningeal carcinomatosis with conservation of autonomy. Purpose of the study. Investigation of possible optimization of treatment in patients with breast cancer and cervical cancer with low-dose xenon therapy.Patients and methods. The study included 156 patients with pT1В2N0M0 cervical cancer (CC) and pT2N1M0 breast cancer (BC) of the reproductive age (29–45 years) after radical treatment, including forced surgical castration in hormone-positive breast cancer with concomitant gynecological pathology. Since the formation of pathological syndromes, 1 cycle (5 sessions) of low-dose xenon inhalation therapy (XT) was performed, with an algorithm for xenon dose calculation and exposure according to the exponential pattern of decreasing concentration and increasing exposure, with an individual approach. Together with general clinical and laboratory examinations, we used international scales for assessing the severity of the patient condition by the Kupperman menopausal index (MMI), ESAS, quality of life (MOS-SF-36), in a modification of the Russian International Center, pain (VAS); the types of general adaptive reactions were identified by the method of L.Kh. Garkavi.Results. Important advantages of a new method associated with a rapid regression of pathological psychosomatic symptoms were revealed after XT. MMI values (p<0.05) decreased, 96.8% of patients reported no pain at all on activity, manifestations of neurovegetative disorders significantly decreased (p=0.02–0.04), and the coefficient of antistress reactions to stress increased, which was congruent with the data on improving the quality of life. Conclusion. High efficiency of the technology demonstrated possible prevention of surgical menopause development and clinical manifestations of postcastration syndrome in order to improve the quality of life and social rehabilitation of young patients with gynecological cancers.

Leptomeningeal Carcinomatosis is a recurrent complication in different types of tumors with systemic involvement, especially breast cancers. Thus, given the different forms of treatment for this disease, this article presents the effectiveness of using Ommaya catheter as a way of administering chemotherapy. Two cases of breast cancer were studied and their therapeutic evolution reported. Despite the poor prognosis associated with meningeal carcinomatosis, these cases had a disease response and control for a longer period than the expected median.

Pancreatic cancer is the fourth leading cause of cancer-related death with a median survival of 3–11 months when metastatic. We present a patient with metastatic pancreatic cancer and an exceptional response to initial systemic chemotherapy with FOLFIRINOX (fluorouracil, leucovorin, irinotecan, and oxaliplatin). Despite evidence of disease control on body imaging, he developed symptomatic leptomeningeal disease and brain metastases 29 months into treatment. He received aggressive treatment with capecitabine and irinotecan, intrathecal topotecan, and eventually bevacizumab. He did well for 36 weeks on this regimen until developing sepsis. This patient significantly outlived his expected survival and, moreover, did so with very good quality of life. This case demonstrates the natural history of pancreatic cancer progressing to involve the central nervous system when systemic disease is otherwise responsive to chemotherapy. It is the first case to demonstrate the potential effectiveness of intrathecal topotecan in combination with systemic chemotherapy for the treatment of leptomeningeal metastases of pancreatic cancer.
