Hemorheological changes and their clinical relevance in preeclampsia

This review summarizes the hemorheological changes during gestation and their clinical relevance in preeclampsia. The gestational disease named preeclampsia, characterized by proteinuria (more than 0.3 g/day) and hypertension (blood pressure above 140/90 mmHg), exists from the 20th gestational week until the sixth post partum week. Its etiology is complex; the pathomechanism mainly involves disturbances in cross talks among the vegetative system, the placenta and the circulatory system. Soluble factors of placenta mediate circulatory changes, which result in adaptive responses in both vegetative and circulatory systems. Derailment of this adaption, however, leads to increased turbulence and local damages in cellular elements of the circulatory system. The initial local lesion progresses to a generalized form. Later, these events will continue to strengthen their own cycle. As a result, an unstable circulatory state will be established, which causes organ damages. Orv. Hetil., 2011, 152, 205–211.

1. One hundred and five primigravidae were followed sequentially at 4-weekly intervals starting at gestational week 31. They were seen again at 6 weeks post partum. 2. At each visit measurements were made of blood pressure as well as of leucocyte and erythrocyte sodium and potassium content. 3. Eighty-five subjects completed the study. Seven developed pre-eclampsia. 4. In both controls and patients who developed preeclampsia, leucocyte and erythrocyte sodium content increased with gestational age and fell post partum. These changes were of greater magnitude in the patients with pre-eclampsia. Cell potassium fell in both groups, but to a greater extent in patients with pre-eclampsia. 5. These changes in cell sodium paralleled those in blood pressure in both groups. 6. These data suggest that the excessive blood pressure changes in pre-eclampsia might be related to similar changes in cell sodium content.

<b><i>Background:</i></b> Orthostatic hypotension (OH) and impaired OH recovery derived from beat-to-beat blood pressure (BP) measurements are associated with detrimental clinical outcome, but the clinical relevance of OH recovery assessed using the widely available sphygmomanometer is still unclear. <b><i>Method:</i></b> 635 geriatric outpatients underwent comprehensive geriatric assessment, including orthostatic BP measurements using a sphygmomanometer, during supine rest and 1 and 3 min after standing up and assessment of physical performance (i.e., the timed up and go test and the Short Physical Performance Battery) and the number of falls in the past year. The association between BP recovery, defined as BP at 3 min minus BP at 1 min after standing up, with physical performance and falls was assessed using regression analyses, adjusting for age and sex, both in the entire cohort and after stratifying for the presence of OH at 1 min after standing up. <b><i>Results:</i></b> BP recovery was not associated with physical performance or number of falls, neither in the entire cohort, nor in subpopulations with or without OH. <b><i>Conclusion:</i></b> The clinical relevance of BP recovery between 1 and 3 min after standing up could not be demonstrated. The results suggest that sphygmomanometer measurements have an inadequate time resolution to record the clinically relevant dynamics of orthostatic BP recovery.

Noise in the workplace is often a separate problem for the workforce so that it can cause mental-emotional disturbance as well as the heart and circulatory system. According to the 2013 Basic Health Research, the prevalence of high blood pressure in Indonesia at the age of ≥18 years is 25.8%. So the researchers wanted to find a relationship between noise intensity and blood pressure. This research method was observational analytic with a cross-sectional approach conducted at PT. Bintang Intipersada Shipyard, Batam City. The sampling technique was a total sampling with a population of 100 workers in 2018 and obtained results of 61 workers determined by inclusion and exclusion criteria. The results of the study were analyzed with a frequency distribution tabulated and tested with the Pearson Product Moment Test. The results of this study worker who were exposed to noise intensity ≤85 dBA had a normal blood pressure of 8 (36.5%) people, in workers who were exposed to noise intensity ≤85 dBA had blood pressure with Pre-hypertension as many as 12 (54.5% ) people, for workers exposed to noise intensity ≤85 dBA have 1 (4.5%) blood pressure with Stage 1 Hypertension, workers who are exposed to noise intensity ≤85 dBA have blood pressure with Stage 2 Hypertension as much as 1 (4.5 %) person. While workers who are exposed to noise intensity> 85 dBA have normal blood pressure of 0 (0%) people, workers who are exposed to noise intensity> 85 dBA have blood pressure with Pre-hypertension as much as 2 (5.1%) people, workers are exposed to noise intensity> 85 dBA had blood pressure with Stage 1 Hypertension as many as 14 (35.9%) people, then workers exposed to noise intensity> 85 dBA had blood pressure with Stage 2 Hypertension as many as 23 (59%) workers. The results of the Pearson Product Moment analysis value of p = 0,000 <α 0.05, there is a relationship between noise intensity and blood pressure with the magnitude of the correlation coefficient (r) which is 0.795 meaning, there is a strong relationship. Based on this study it can be concluded that there is a significant relationship between noise intensity and blood pressure.

Hypertension is a disease that usually occurs in the circulatory system which can cause an increase in blood pressure above the normal value, which exceeds 140/90 mmHg. The intention of this study aims to understand the relationship between lifestyle and the incidence of hypertension in Antang Makassar Public Health Center in 2018. This research method uses quantitative analytic motives using a cross sectional approach with a population of 46 people and a sample of 36 respondents. This research was conducted on July 14-16 June. The method used is simple random sampling and the tool used to obtain data is a questionnaire. The scale used is Liker and Guttmann scale with univariate, bivariate analysis and chi-square test with significant a = 0, 05. The results of this study indicate there is a relationship between lifestyle and the incidence of hypertension in the Makassar Antang Health Center with P = 0, 01 smaller than the value of a = 0.05. Suggestions are for hypertensive patients, namely the need to check their health routinely to health workers in order to know the development of blood pressure and obtain information from health workers.

A 27-year-old woman from and resident in ourika(Marrakesh region – Morocco) with a history of anemia under iron treatment, without a history of surgery, has G1 P1. Presented to the Mohamed VI University Hospital on day 21 post partum of a vaginal birth at home, the evolution was marked by the appearance of urinary incontinence treated in the emergency room by placing a catheter urinary on day 6 post partum then send to her home for the COVID 19 context, then the patient consulted again at the gyneco-obstetric emergency room for deterioration of the general condition in a picture of hemodynamic shock. On physical examination, the patient was hemodynamically unstable with blood pressure figures of 70/40 mmhg, heart rate at 50 bpm, temperature at 35 ° and mucosal skin pallor.Examination of the vaginal cavity showed the presence of numerous whitish-looking maggots, a sample was taken by the biology team that collected the maggots for the purpose of a parasitological study to identify the parasite responsible.
