Introduction: Two-thirds of all households in developing countries depend on unprocessed biomass fuel for cooking. Traditional stoves have poor combustion capacity which produce heavy smoke and numerous  harmful pollutants. Switching to Improved Cooking Stove (ICS), a well-designed earthen made stove equipped with a chimney could be beneficial for health. The aim of this study was to assess the efficacy of ICS on maternal health in rural areas of Bangladesh. Methods: A quasi-experimental design was adopted to conduct the study. This study selected 150 Households from 5 villages (intervention) and 150 Households from 2 villages (control) from the Manikgonj District of Bangladesh during January 1, 2012 to July 30, 2012. Differences between control and intervention group were examined by applying t-test or one-way analysis of variance (ANOVA). The conventional cut-off value of 0.05 was taken as statistical significance. Stata (version 13) was used to do the analysis. Results: Most of the respondents (62%) were less than 30 years of age. About 94% participants resided in tin-shaded houses, and 51% kitchens were small. After the intervention period, the measured mean concentrations of Particulate Matter (PM2.5) for the intervention and control group was 259 µg/m3 and 1285 µg/m3, respectively (p<0.05). However, lung function test (LFT) did not reveal significant differences between the two groups (p>0.05). Conclusion: ICS reduced the incidence of respiratory illness among the intervention group, however, did not demonstrate significant changes in LFT during the six months of the follow-up period. Therefore, this study found ICS might have the potential to be used to improve the maternal health in rural Bangladesh. However, more longitudinal investigations are expected to demonstrate the efficacy and impact of ICS on maternal health to support the statement.

AbstractObjectiveThe objective of this study is to examine, based on theory of change, whether integrated leading-managing-and-governing for results model is plausible cause of improved institutional delivery.MethodsA team-based quasi-experimental study was conducted. One-hundred-thirty-four health facility teams were enrolled in the study. Teams were allocated to intervention and control groups in a 1:1 ratio, non-randomly. End line institutional delivery was the dependent variable while the group (main predictor) and the baseline institutional delivery (covariate) were independent variables. The intervention that was given over six months was integrated leading-managing-and-governing for results model. The institutional deliveries were measured with percentages whilst the group was measured with exposure status (yes or no) to the intervention. Data, from both groups, were collected at baseline and end line. Data were analyzed using analysis of covariance. Statistical significance was determined at (p<.05). The main effect of the intervention was determined by 95% CI, presented in the contrast results.ResultsThe adjusted mean institutional deliveries with 95% CI were 47.4 (46.2, 48.6) and 33.4 (32.2, 34.6) in the intervention and control groups, respectively. Contrast results showed that having an intervention group, p = .000, 95% CI (12.2, 15.8), of integrated leading-managing-and governing for results model significantly increased mean institutional delivery compared to having a control group.ConclusionsThis study provides some guidance regarding the plausible causation of integrated leading-managing-and-governing for results model on institutional delivery. It would serve as a baseline in identifying true causation using a randomized design.

Background Antioxidants show nephroprotective effect against vancomycin associated nephrotoxicity (VAN) in animals. This study aimed to assess the ascorbic acid nephro-protective role against VAN clinically. Methods Forty-one critically ill patients were randomly assigned to one of two groups: intervention group (vancomycin IV plus ascorbic acid, n=21) or control group (vancomycin IV only, n=20). Primary outcomes were the incidence of VAN and the absolute change in creatinine parameters, while mortality rate was the secondary outcome. Nephrotoxicity was defined as an increase in serum creatinine (S.cr) by at least 0.5 mg/dL or 50% of baseline for at least two successive measurements. This study is registered at Clinicaltrials.gov (NCT03921099), April 2019. Results Mean absolute S.cr increase was significant when compared between both groups, P-value = 0.036, where S.cr increased by 0.05(0.12) and 0.34(0.55) mg/dL in the intervention and control groups, respectively. Mean absolute Cr.cl decline was significant when compared between both groups, P-value = 0.04, where Cr.cl was decreased by 5.9(17.8) and 22.3(30.4) ml/min in the intervention and control groups, respectively. Incidence of VAN was 1/21(4.7%) versus 5/20(25%) in the intervention and control groups, respectively (RR: 0.19; CI: 0.024–1.49; P-value = 0.093). Mortality was higher in the control group; however, it was not statistically significant, P-value = 0.141. Conclusion Co-administration of ascorbic acid with vancomycin preserved renal function and reduced the absolute risk of VAN by 20.3%, however, the reduction in VAN incidence didn’t reach statistical significance level. Further large multicenter prospective trials are recommended.

This study aimed to examine the effects of the Helping Babies Breathe (HBB) training interventions program on the knowledge, psychomotor skills, and competency of health workers in managing birth asphyxia and reducing mortality of newborns experiencing asphyxia within 24 hours. This study used pre- and post-test design (quasi experimental study). Purposive sampling was employed, and a computer-generated number was used to select the participants. Health workers from Juba Teaching Hospital comprised the intervention group. They were evaluated before and after the training from February to June 2017. A post training skill and competency evaluation was performed using a NeoNatalie newborn simulator and was repeated after three months of implementation for intervention and control group. Seventy health workers were enrolled; 40 were in the intervention group and 30 in the control group. Early newborn mortality due to asphyxia within 24 hours in intervention and control measure at pre and post implementation showed a significant reduction within the intervention than the control. Knowledge, psychomotor and competency of health care workers improved immediately after training and early newborn mortality reduced by half at the end of three months. It is recommended that training of health workers on HBB should be scaled up in most of the health facilities in South Sudan.

Hospital procedures, such as infusions, are often associated with pain. Preschool-aged children who are hospitalized for infusions will often exhibit a strong fearful response because their concept of body integrity has not fully developed. One way to reduce the fear of infusion pain is by using stories as a distraction technique. The purpose of this study was to identify differences in the scale of pain between members of an intervention group, to which the distraction technique was applied, and a control group, to which it was not applied. The study used quasi-experimental methods, with intervention and control groups. The study was conducted on 46 preschool aged respondents (3-6 years old), divided into a 34 member intervention group and a 12 member control group and ran from September to December 2017 in one of the private hospitals in the East Bekasi. The research employed a questionnaire to collect demographic data and used the Wong-Baker Faces Pain Scale to assess pain levels. The data analysis technique used was the independent t-test. The results showed there was a difference of pain response between the intervention and control groups with P value < 0.05. Pediatric nurses are advised to use storytelling therapy as an option for providing atraumatic care intervention.  Keywords: Distraction Technique, Infusion installation, Pain, Stories, Preschool Abstrak Teknik Distraksi: Bercerita Menurunkan Nyeri Pada Anak Usia Pra Sekolah Selama Penggunaan Infus.
