TIAPRIDE-INDUCED CHRONIC HYPERPROLACTINAEMIA: INTERFERENCE WITH THE HUMAN MENSTRUAL CYCLE

ABSTRACT Four regularly menstruating volunteers were submitted to an oral treatment, for 3 consecutive cycles and starting on the first day of a cycle, with tiapride at daily doses ranging from 1 × 100 mg to 2 × 100 mg. The first and the last cycle under treatment, as well as a prior control cycle, were thoroughly studied by means of daily measurements of blood concentrations of LH, FSH, prolactin (PRL), oestradiol and progesterone. Tiapride, a benzamide derivative with dopaminergic blocking activity at the level of the lactotrophes, increased mean PRL secretion in each subject but a permanent hyperprolactinaemia above 700 μU/ml was attained only in one subject. Despite these widely fluctuating PRL levels in most subjects, the resulting overall hyperprolactinaemia induced in all cases a progressive deterioration of the function of the corpus luteum: 5 cycles showed luteal phases reduced by 2–5 days, one cycle was characterized by some slight luteinisation but questionable ovulation and the 2 remaining cycles were anovulatory. The interruption of drug intake one week after the onset of menses led thereafter to a cycle with a likely inadequate luteal phase but of normal length. It is concluded that even a non-permanent hyperprolactinaemia can impair the normal function of the hypothalamo-pituitaryovarian axis, as well as exhibit some effects in a cycle consecutive to the normalization of PRL. With the exception of the impaired luteal progesterone secretion, the pooled hormonal data from the short luteal phase cycles under tiaprideinduced hyperprolactinaemia exhibit very little significant differences, as compared to the corresponding values in the control cycles. Some delay in the onset of follicular maturation, however, should be assumed since the follicular phase had been lengthened by 1 to 31 days in 5 of the 6 cycles with luteinisation during treatment. The present results are compatible with a double impact – both at the ovarian and the hypothalamo-pituitary levels – of hyperprolactinaemia in its mechanisms of impaired function of the hypothalamo-pituitary ovarian axis.

ABSTRAK.  Progesteron merupakan hormon yang berperan penting dalam proses pemeliharaan kebuntingan dan dihasilkan oleh corpus luteum. Penelitian ini bertujuan mengetahui hubungan antara kadar hormon progesteron pada fase awal luteal dengan kematian embrio pada sapi Aceh. Dalam penelitian ini digunakan empat ekor sapi betina dewasa berumur 3-5 tahun, bobot badan 150-250 kg, sehat secara klinis, dan memiliki reproduksi normal. Sapi disinkronisasi menggunakan 5 ml prostaglandin F2 alfa (PGF2α) dengan pola penyuntikan ganda berinterval 11 hari. Koleksi sampel darah untuk pengukuran konsentrasi progesteron dilakukan pada hari ke-5, 6, dan 7 pasca inseminasi. Pengukuran konsentrasi progesteron dilakukan menggunakan metode enzymelinked-immunoassay (ELISA), pemeriksaan kebuntingan dan kematian embrio menggunakan metode transrektal ultrasonografi pada hari ke-25 pasca inseminasi. Pemeriksaan diulang setiap 10 hari sampai hari ke-55 pasca inseminasi. Puncak sekresi progesteron pada sapi bunting dengan embrio yang bertahan hidup terdapat pada hari ke-7 (2,082 ng/ml), pada sapi Late Embryonic Mortality (LEM) di hari ke-5 (8,209 ng/ml) dan pada sapi tidak bunting di hari ke-7 (3,051±1,157 ng/ml). Sekresi progesteron sapi LEM pada hari ke-5 sampai dengan ke-7 cenderung menurun sedangkan pada sapi yang bertahan hidup cenderung meningkat.  (Correlation between progesterone levels in early luteal phase and embryonic death  in Aceh cattle) ABSTRACT. Progesterone is an important hormone that functions to maintain pregnancy and is produced by the corpus luteum. The aim of this study was to see a correlation between progesterone and the incidence of embryonic death in Aceh cattle. This study used four adult female cows, 3-5 years old, 150-250 kg body weight, clinically healthy, and have a normal reproduction. The synchronized with 5 ml prostaglandin F2 alfa hormone, and double injection pattern with 11-day intervals. The blood was collected for progesterone measurements on 5th, 6th, 7th day post artificial insemination. Measurement of progesterone concentration was carried out using an enzymelinked-immunoassay (ELISA), while pregnancy and embryo mortality was performed using the trans-rectal ultrasonography method on the 25th day after insemination. The examination was repeated every 10 days until day 55th after insemination. Progesterone secretion peaks in pregnant cows were on day 7th (2.082 ng/ml), in cattle Late Embryonic Mortality (LEM) on day 5th (8.209 ng/ml) and in cattle not pregnant on day 7th (3.051±1.157 ng/ml). The pattern of LEM progesterone secretion on days 5th to 7th tends to decrease while those that survive tend to increase.

Abstract Transforming growth factor α (TGFα) inhibits hormone production by cultured follicular cells but evidence of an effect of TGFα on ovarian hormone secretion in vivo is still required. Eleven ewes with an autotransplanted ovary received, by ovarian arterial infusion, either 5 μg/h recombinant rat TGFα (n=6) or placebo (n=5) for 12 h on day 10 of the luteal phase. Two hours before the start and 1 hour before the end of the infusion each ewe received a single injection of gonadotrophin-releasing hormone (GnRH; 150 ng i.v.). Two hours after the end of the infusion luteal regression was induced with prostaglandin F2α (PGF2α; 125 μg i.m.). Ovarian and jugular venous blood samples were taken at 10-min, 15-min or 4-h intervals from 2 h before the start of the infusion until 96 h after PGF2α and the rates of secretion of ovarian oestradiol, inhibin, progesterone and androstenedione were determined. Jugular venous concentrations of LH and FSH were also measured and follicle populations monitored by real-time ultrasound scanning. Infusion of TGFα resulted in a significant (P<0.05) depression in the amplitude of the pulsatile response of oestradiol and androstenedione secretion to the GnRH-induced LH pulse at the end of the infusion. Ovarian inhibin secretion was acutely suppressed by TGFα infusion (P<0·001) and remained lower than controls for the period of the experiment. Luteal phase progesterone secretion was also acutely inhibited (P<0·001) by infusion of TGFα and in one treated ewe progesterone secretion was elevated 48–84 h after PGF2α. Jugular venous concentrations of FSH in TGFα-treated ewes were significantly (P<0·001) elevated over controls during the first 48 h of the follicular phase and the LH surge was delayed for about 10 h (P<0·05). Infusion of TGFα caused a marked decline (P<0·05) in the number of large follicles within 12 h of the end of the infusion. Two of the six treated ewes, including the one with high follicular phase progesterone, had unusually large (8·7 and 10 mm) follicles present from 48–96 h after PGF2α. We conclude that direct arterial infusion of TGFα results in acute inhibition of ovarian steroid and inhibin secretion that is associated with induction of atresia in the population of large follicles. The lack of feedback of ovarian hormones results in a rebound increase of FSH which stimulates the growth of more ovarian follicles and the eventual re-establishment of ovarian hormone secretion and normal cyclicity. Low progesterone concentrations during the bovine oestrous cycle induce enhanced responsiveness to oxytocin challenge late in the luteal phase of the same cycle. The delayed effect of low progesterone concentrations during one oestrous cycle on uterine PGF(2alpha) secretion after oxytocin challenge on day 15 or 16 of the subsequent cycle was studied by measuring the concentrations of the major PGF(2alpha) metabolite (13,14-dihydro-15-keto PGF(2alpha); PGFM) in plasma. Two experiments were conducted, differing in the type of progesterone treatment and in the shape of the low progesterone concentration curves. In Expt 1, progesterone supplementation with intravaginal progesterone inserts, with or without an active corpus luteum, was used to obtain high, or low and constant plasma progesterone concentrations, respectively. In Expt 2, untreated cows, representing high progesterone treatment, were compared with cows that had low but increasing plasma progesterone concentrations that were achieved by manipulating endogenous progesterone secretion of the corpus luteum. Neither experiment revealed any differences in plasma progesterone concentrations between the high and low progesterone groups in the subsequent oestrous cycle. In both experiments, both groups had similar basal concentrations of PGFM on day 15 (Expt 1) or 16 (Expt 2) of the subsequent oestrous cycle, 18 days after progesterone treatments had ended. In both experiments, the increases in PGFM concentrations in the low progesterone groups after an oxytocin challenge were markedly higher than in the high progesterone groups. These results indicate that low progesterone concentrations during an oestrous cycle have a delayed stimulatory effect on uterine responsiveness to oxytocin during the late luteal phase of the subsequent cycle. This resulting increase in PGF(2alpha) secretion may interfere with luteal maintenance during the early stages of pregnancy.

Objective: This study evaluated the perception of users of complete dentures (successful cases) provided by the public health service throughout the course from tooth loss to rehabilitation. Methods: A total of 11 individuals who received their complete dentures through the public health service were interviewed according to a qualitative approach based on three pre-established topics: (1) tooth loss, (2) living without teeth, and (3) living with dentures. The obtained material was submitted to content analysis. Results: Individuals associated the socioeconomic status with lifelong oral health experiences and difficulty to access oral treatment. Tooth loss was the solution to pain and sometimes perceived as a natural event of life. Living without teeth was a negative surprise that resulted in physical and psychological impairments. The period of adaptation to dentures represented suffering and required psychological efforts to be successful. Conclusion: The dentures represented a reward for the suffering and recovered normal function, appearance, and socialization.

Abstract. Granulosa cells were obtained from human preovulatory follicles in 31 women undergoing in vitro fertilization and embryo transfer due to tubal infertility. Follicular maturation was stimulated and synchronized by treatment with Clomiphene or human menopausal gonadotrophin (hMG), or both, plus human chorionic gonadotrophin (hCG). Follicles were aspirated by ultrasound guided puncture approximately 34–36 h after the hCG injection. The granulosa cells were washed and suspended in modified medium 199 containing 10% foetal bovine serum and cultured as monolayers for 6–8 days in the absence and presence of hormones and reactants. Progesterone formation was analyzed by RIA. In general, the cells underwent morphological luteinization and secreted high amount of progesterone. Under basal conditions the secretion of progesterone was highest during the first 2 days in culture and then gradually declined. Progesterone secretion was stimulated by human LH, hCG and the adenylate cyclase stimulator forskolin, with a maximal effect between days 2–6. The β-adrenergic agonist isoproteronol in preliminary experiments potentiated the stimulatory effect of hCG but had no own stimulatory effect. No clear differences in progesterone secretion or responsiveness to in vitro stimulation relating to the various in vivo stimulation protocols were found.

Plasma bradykininogen levels have been shown to rise about threefold late in pregnancy in the rat. However, they declined sharply 1–2 days before delivery.Plasma bradykininogen levels remained relatively unchanged through most the oestrous and menstrual cycles (rat, man). However, they showed a fall at two similar periods, in both cycles. A smaller decline occurred in metoestrus in the rat, and (probably) in the late luteal phase in the human. The largest fall took place around the time of ovulation in both the oestrous and the menstrual cycles. The decline was about 59% in the rat and 42% in the human. Bradykininogen showed no similar changes in the blood of male human controls. The suggestion that bradykinin could be involved in ovulation is discussed.

ABSTRACT We have examined the ability of granulosa cells, from carefully selected preovulatory bovine follicles, to secrete oxytocin in vitro. Although cells from 83% of follicles underwent functional luteinisation (greatly increased progesterone secretion) in serum-supplemented culture, only 69% had cells capable of oxytocin secretion. Secretion followed a similar time course in all cultures, with the peak appearing on day 3. Oxytocin, but not progesterone, output could be consistently increased by addition of pieces of theca interna tissue, or theca conditioned medium, to the cultures. The effect could be achieved by exposure to theca tissue at any time prior to peak output without altering the time course of secretion. Oxytocin could not be detected in follicular fluid from any of the selected follicles, nor in medium from theca cultured alone. We conclude that the potential for oxytocin secretion is a feature of follicular maturation which is lost during atresia and that the stimulus to secretion is associated with luteinisation but not with progesterone output. Finally, the intermixing of follicular cells during corpus luteum formation may provide a mechanism for the enhancement of oxytocin secretion within a predetermined time frame.
