Abstract. Background: Nurturing gatekeepers is an effective suicide prevention strategy. Internet-based methods to screen those at high risk of suicide have been developed in recent years but have not been used for online gatekeeping. Aims: A preliminary study was conducted to examine the feasibility and effects of online gatekeeping. Method: Advertisements to promote e-mail psychological consultation service use among Internet users were placed on web pages identified by searches using suicide-related keywords. We replied to all emails received between July and December 2013 and analyzed their contents. Results: A total of 139 consultation service users were analyzed. The mean age was 23.8 years (SD = 9.7), and female users accounted for 80% of the sample. Suicidal ideation was present in 74.1%, and 12.2% had a history of suicide attempts. After consultation, positive changes in mood were observed in 10.8%, 16.5% showed intentions to seek help from new supporters, and 10.1% of all 139 users actually took help-seeking actions. Conclusion: Online gatekeeping to prevent suicide by placing advertisements on web search pages to promote consultation service use among Internet users with suicidal ideation may be feasible.

<strong>Introduction:</strong> The aim of this paper was to assess the prevalence of suicide ideation and attempts in Portuguese psychiatry trainees (adult and child and adolescence), and compare the data with the general population and other European countries.<br /><strong>Material and Methods:</strong> A structured and anonymous questionnaire was sent by email to 159 portuguese trainees of adult psychiatry and child and adolescence psychiatry with questions about personal history of suicidal ideation and suicide attempts, as well as family history of suicide attempts and completed suicides. This is part of the BoSS Study (Burnout Syndrome Study) performed in 21 countries worldwide. Data was analysed in SPSS v.19.<br /><strong>Results:</strong> From the inquired population, 62 trainees (40,3%) partially responded, and 46 (29%) were complete responders - these entered the final analysis. There was a ratio of 2:1 (female:male) and a mean age of 29 years. The suicidal ideation was present in passive form in 44% and in active form in 33%; also, 4.3% of respondents had previous suicide attempts. In first degree relatives, 22% had attempted suicide and 13% completed suicide.<br /><strong>Discussion:</strong> The results are worriying and may be associated with some factors to which this population is exposed.<br /><strong>Conclusion:</strong> It is necessary further research to better understand this phenomenon, its causes and potential modifiers.

Abstract Background Meehl portrayed pervasive, chronic suicidality as a key sign of schizotypy. Consistent with this view, positive schizotypy predicts concurrent and future suicidality, is particularly predictive of greater lethality, and is a more potent predictor of suicidality than other psychopathology. Whereas the most prominent explanation for this relationship is that it is spurious, several possible causal connections have not been tested. Also, most existing evidence relates to positive schizotypy and psychosis experience. We describe three studies of the schizotypy – suicidality link. Methods In the first, we tested whether, as Meehl argued, schizotypy is associated with suicide dread. A general population sample (n = 350) of young adults (18 to 24 years) reported on schizotypy and suicidality, including efforts to avert suicide attempts. In univariate analyses, positive, negative, and disorganized components of schizotypy each significantly predicted persistent or frequent suicidal ideation in the past month (ORs from 2.10 to 3.71), history of attempts with intent to die (1.59 to 2.15), fear or dread of the possibility of making an attempt (1.58 to 1.63), and worry about acting on an unwanted impulse to attempt suicide (2.48 to 2.62). In fully-adjusted analyses (controlling for depression, anxiety, stress, and all schizotypy components), positive schizotypy predicted reporting of greater worry about impulsive suicidal behaviour (OR = 1.71, p = .009, 95% CI 1.15 to 2.56). In the second, we tested whether the schizotypy – suicidality link can be understood using contemporary suicide theory. In a random sample of high school pupils (n = 177), schizotypy components predicted classification as an active suicidal ideator (R2 = 0.76, 95% CI 0.56 to 0.95). These effects were accounted by the influences of magical thinking, unusual perceptual experiences, and suspiciousness being mediated in part by perceived burdensomeness, as per the interpersonal theory of suicide. However, direct effects were also observed from social anxiety and magical ideation components of schizotypy. In the third, we modelled latent growth mixtures of suicidality using data from five waves of the Dunedin Multidisciplinary Health and Development Study. We test how growth in suicidality (from 18 to 38 years) is related to psychosis experience (age 11 years) and schizotypy (age 13 and 15 years). Schizotypy predicted membership of a growth class characterised by chronically death- and suicidal-ideation that, in turn, predicted attempt behaviour. Results See above. Discussion The complexity of the observed links of schizotypy and psychosis experience with suicidality do not lend themselves to being discounted as spurious or due to common underlying causal factors. Research addressing possible causal connections is warranted, as are efforts to identify whether reduction of suicidality may result from interventions targeting features of subclinical psychosis.

Several studies have been conducted to establish a profile of the suicidal/parasuicidal patient. Also several factors have been identified as possibly influencing the suicidal rates, including the religious practices.Objectives:Characterize the profile of suicidal behavior in a sample of patients followed in a general psychiatric consultation.Methods:It was done an analytical observational study of a random sample of 100 patients followed in a general psychiatric consultation. A survey was conducted with the collection of socio-economic, religious aspects and clinical data, and it was consulted the patient"s clinical process. Data analysis was done in Excel 2003.Results:The sample was consisted mostly by women (74%), being the most representative age group between 40 and 50 years (27%), mostly married (61%), 24% had 2 children and 65% lived in the rural area. The clinical diagnosis (ICD-9) was in 46% of cases, neurotic depression. 52% consider themselves religious not practitioners, being 90% catholics. History of suicide attempts/parasuicide occurred in 32% of patients, in the form of drug intoxication (31%) or with another method (11%). Most of the individuals said to have already thought about suicide at least once in their lifetime (74%). Only 8% had current suicidal ideation. Family history of suicide occurred in 27%, particularly in first degree family members, mainly by drowning (7%) and hanging (7%).Conclusions:Our results suggest that exists a high prevalence of suicidal behavior in this patients. For that reason, it should be done a systematic screening for suicidal ideation in this risk population.

Abstract Background Trans women have been shown to experience disproportionately poor outcomes in physical and mental health. Although it is common to talk about the violence against trans people, little is still known about mental health outcomes and experiences of suicidality among trans women, particularly in developing countries. This study aims to investigate risk factors and associations with mental health, suicide ideation and suicide attempts among trans women in the largest metropolitan area in Brazil. Methods Trans women living in São Paulo were recruited between May 2017 and July 2019 using the long-chain peer referral method Respondent-Driven Sampling. Multivariate regression models were used to investigate the associations with K10 score classification (logistic) and suicidal ideation/suicide attempt (ordinal logistic). Results A total of 763 trans women were included in the study. Over one quarter (26.5%) of trans women had been diagnosed with anxiety in the past, and close to one in five (19.1%) trans women had received a diagnosis of depression. More than two in five (41.9%) trans women had moderate to severe psychological distress. More than half of all participating trans women reported having previously either experienced suicidal ideation or attempted to take their own lives (25.0 and 31.2% respectively). In multivariate regression, moderate to severe psychological distress was associated with homelessness, income, current sex work, use of stimulant drugs, history of physical abuse, depression diagnosis and access to mental health treatment. Suicidal ideation and suicide attempts were associated with race/skin color, living arrangements, marital status, current sex work, history of sexual violence, depression and PTSD diagnoses, access to mental health treatment and psychological distress. Conclusions This study showed that there is a significant association between mental health conditions, lack of treatment for these conditions and suicidality among trans gender women. Findings point to the need for a structural transformation in Brazil that enables a reduction in the social inequality and violence that impact the mental health of trans women. A number of recommendations to achieve this are provided.

Background: Historically, OCD has been considered to be associated with a relatively low risk of suicide. Recent studies, on the contrary, revealed a significant association between OCD and suicide attempts and ideation. A huge variation in prevalence rates, however, is reported. Objective: To estimate prevalence rates of suicide attempts and suicidal ideation in individuals with OCD, and to identify predictors of suicide risk among subjects with OCD. Methods: We systematically reviewed the literature on suicide risk (ideation and/or attempts) and OCD. We included studies with appropriate definition of OCD, cross-sectional or prospective design, separating clinical samples from epidemiological studies, that employed a quantitative measure of suicidality and/or reported an outcome measure of the association between suicidality and OCD or examined factors associated with suicidality. Results: In clinical samples, the mean rate of lifetime suicide attempts is 14.2% (31 studies: range 6- 51.7%). Suicidal ideation is referred by 26.3-73.5% of individuals (17 studies, mean 44.1%); current suicidal ideation rate ranges between 6.4 and 75% (13 studies, mean 25.9). Epidemiological studies found that OCD increases significantly the odds of having a lifetime suicidal ideation as compared to the general population (OR: 1.9-10.3) and a history of lifetime suicide attempts (OR: 1.6- 9.9). Predictors of greater suicide risk are severity of OCD, the symptom dimension of unacceptable thoughts, comorbid Axis I disorders, severity of comorbid depressive and anxiety symptoms, past history of suicidality and some emotion-cognitive factors such as alexithymia and hopelessness. Conclusion: Overall, suicidality appears a relevant phenomenon in OCD.

Prior research has shown that psychopathic personality traits are differentially related to suicide risk, and limited literature also suggests the potential that such risk manifests differently across sex. The current study sought to examine whether sex moderated associations between domains from the triarchic model of psychopathy, a comprehensive perspective of psychopathy, and various suicide variables. Our sample included 1,064 criminal defendants (760 males, 304 females), who had been administered the MMPI-2-RF, from which triarchic psychopathy scales were scored. Suicide-related variables, including current suicidal ideation during interview, history of previous suicide attempts, and number of previous suicide attempts, were reliably obtained from clinical records. The MMPI-2-RF SUI (Suicide/Death Ideation) scale was also examined as a psychometric operationalization of suicidality. Results provided evidence for the general association between psychopathy-related traits and suicide-related outcomes in a manner consistent with the literature. However, these associations did not differ as a function of sex.

Abstract. Objective: Little is known about specific professional factors influencing medical students' suicidal ideation and behavior. We aimed to evaluate the prevalence and correlates of suicidal ideation in Tunisian medical students. Method: This was a cross-sectional study; 390 second-cycle medical students were enrolled at the Faculty of Medicine of Tunis. Participants responded to an anonymous questionnaire containing sociodemographic data, educational factors, the Suicidal Ideation Questionnaire (SIQ), and the Depression, Anxiety and Stress scales (DASS-21). Results: Eleven students (2.8%) had made one or more suicide attempts in their lifetime. For a threshold value of 41 based on the SIQ scale, 7.9% of students were considered to have a potential risk of suicide. The multiple hierarchical linear regression analysis identified as main factors independently associated with suicidal ideation: tobacco consumption, perceived sleep quality, depression symptoms, the personal history of suicide attempt, the satisfaction with student–supervisor relationship, and choosing to study medicine. Conclusion: Suicide prevention efforts should target high-risk students with prior personal or family history of suicidal behavior. Moreover, medical school authorities should put in place effective strategies to optimize the learning environment at their institutions.

ABSTRACTIntroduction: Individuals who have deep and periventricular white matter hyperintensities may have a higher risk for suicidal behavior. There are mixed results in the literature regarding whether unipolar or bipolar patients who have attempted suicide have more deep white matter hyperintensities (DWMH) or periventricular hyperintensities (PVH) relative to those who have no history of suicide attempts.Methods: A meta-analysis of studies examining white matter hyperintensities (WMH) in mood disorder patients with and without a history of suicide attempts was performed.Results: Four studies, including a total of 173 patients who attempted suicide and 183 who did not attempt suicide, were included. A significantly higher number of attempters were found to have WMH than non-attempters. Unipolar depressed patients who had attempted suicide had 1.9 times more DWMH and 2.1 times more PVH than those who did not. Bipolar patients who had attempted suicide had 5.4 times more PVH than those who had not. Taken together, unipolar and bipolar patients who had attempted suicide had 2.8 times more DWMH and 4.5 times more PVH than those who had never attempted suicide.
