Myasthenia gravis is a chronic autoimmune disorder caused by antibodies directed against the neuromuscular junction. Some patients may have an associated thymoma, which confers a worse prognosis. Eculizumab, a monoclonal antibody that inhibits the activation of terminal complement, has recently been approved for the treatment of refractory generalized myasthenia gravis. This is an early case report of thymoma-associated refractory myasthenia gravis successfully treated with eculizumab in a real-world setting.

Abstract Use of ustekinumab in Crohn’s disease was approved in 2016, and consequently data regarding its real-world safety are still limited. We here present a 29-year-old woman with severe therapy-refractory Crohn’s disease, who developed an anaplastic large cell T cell lymphoma during treatment with ustekinumab.

Background. The clinical course of bronchial asthma (BA) in children is often mild. However, in real-world clinical practice it may not always be possible to attain complete control of BA symptoms. In the majority of cases cessation of basic therapy with inhaled corticosteroids (ICS) leads to a poorer BA control in spite of satisfactory general health status in patients. As a result of incomplete BA control, children demonstrate impaired activities of daily living, decreased daily physical activity and gradual reduction or withdrawal of sports participation and other activities. Under the conditions of restricted everyday activities children with mild BA and in the absence of continuous anti-inflammatory basic therapy may feel comfortable without any complaints. This may result in faulty evaluation of asthma as well controlled, which can cause disease progression.Case report. All the above mentioned situations are exemplified by the case report of a youngster suffering from mild bronchial asthma. Also, this article clearly demonstrates the significance of use of asthma control test (ACT) in real-world clinical practice in order to work out a strategy for asthma management. Besides, the role of taking a detailed medical history for correct assessment of mild BA symptom control is demonstrated. In a given clinical example we managed to evaluate, in a short space of time, everyday life changes of a youngster with mild BA (an increase in daily activities and improved quality of life) following administration of basic therapy with ICS.Conclusion. This case report demonstrates the significance of correct assessment of BA control regardless of severity of disease, in our case mild asthma, and shows its role in conducting proper basic therapy and achieving improvement of child’s quality of life.

PURPOSE The Fast Healthcare Interoperability Resources (FHIR) is emerging as a next-generation standards framework developed by HL7 for exchanging electronic health care data. The modeling capability of FHIR in standardizing cancer data has been gaining increasing attention by the cancer research informatics community. However, few studies have been conducted to examine the capability of FHIR in electronic data capture (EDC) applications for effective cancer clinical trials. The objective of this study was to design, develop, and evaluate an FHIR-based method that enables the automation of the case report forms (CRFs) population for cancer clinical trials using real-world electronic health records (EHRs). MATERIALS AND METHODS We developed an FHIR-based computational pipeline of EDC with a case study for modeling colorectal cancer trials. We first leveraged an existing FHIR-based cancer profile to represent EHR data of patients with colorectal cancer, and then we used the FHIR Questionnaire and QuestionnaireResponse resources to represent the CRFs and their data population. To test the accuracy of and overall quality of the computational pipeline, we used synoptic reports of 287 Mayo Clinic patients with colorectal cancer from 2013 to 2019 with standard measures of precision, recall, and F1 score. RESULTS Using the computational pipeline, a total of 1,037 synoptic reports were successfully converted as the instances of the FHIR-based cancer profile. The average accuracy for converting all data elements (excluding tumor perforation) of the cancer profile was 0.99, using 200 randomly selected records. The average F1 score for populating nine questions of the CRFs in a real-world colorectal cancer trial was 0.95, using 100 randomly selected records. CONCLUSION We demonstrated that it is feasible to populate CRFs with EHR data in an automated manner with satisfactory performance. The outcome of the study provides helpful insight into future directions in implementing FHIR-based EDC applications for modern cancer clinical trials.

Objective. The medical record of Chinese medicine is a miniature of the theoretical system of traditional Chinese medicine (TCM), with a time-honored history in a real-world setting and a firm place in medicine. In modern times, people have emphasized the value and standardization of TCM cases. The aim of this study was to explore the historical origins and developments of TCM case records. Methods. A chronological narrative style was used to divide the development history of TCM case records into early (1600 BC–220 AD), middle (220–1911 AD), and modern periods (1912–till now). The historical context of the origin and development of TCM case records was analyzed through the evolution of the format and content of the case recording files with the specific documents and distinctive cases. Results. From the early to middle period, the development of TCM case record had experienced four periods: the budding, blossoming, maturity, and heyday. In modern times, they presented the following characteristics: A, the establishment and development of the discipline of TCM medical records; B, the standardization of the writing format of TCM medical records; C, a large number of books concentrating on recording and studying TCM medical records, especially those of prestigious veteran TCM doctors; D, the proliferation of TCM case reports published in journals; E, the establishment of TCM medical records databases and application platforms integrating computer programs and artificial intelligence; F, many reporting guidelines have been developed in order to improve the reporting quality of case report in TCM. Conclusions. The study analyzed and illustrated the characteristics of TCM case records of different dynasties in terms of writing content and format. TCM case record is a relatively young discipline in spite of its ancient origins. TCM case records still have far-reaching significance for the inheritance and development of TCM theory and clinical experience. From the wisdom of history, its positive impact has just been revalued to be validated and it will continue to develop.
