The effects of lipostat and maninil on carbohydrate and lipid metabolisms and their hormonal regulators were studied in 36 obese patients aged 50- 70 years who had type 2 diabetes mellitus concurrent with dyslipidemia. A course of therapy with lipostat in a daily dose of 20 mg for 3 months was shown to lead to nor­malization of lipid metabolism, to diminished glycemia and hy- perinsulinemia, and to an increase in fasting plasma somatotrop­ic hormone levels to normal values, these were not observed in the control group.

Abstract Background To date, knowledge about the effects of testosterone (T) on the cardiovascular system of men remains controversial and requires additional research. Especially the putative impact of T replacement therapy (TRT) on lipid metabolism and endothelial dysfunction as a starting mechanism for the development of cardiovascular pathology in men with type 2 diabetes mellitus (T2DM) presenting with T deficiency has to be elucidated. The aim of this work was to evaluate the effect of T replacement therapy on lipid metabolism and function of endothelium in men with T2DM and hypogonadism. Methods The study included 95 men (mean age 55,3±2,4 years) with T2DM and late onset hypogonadism, established according to EAU 2015 criteria. Parameters of carbohydrate and lipid metabolism, as well as ultrasound assessment of flow-mediated dilatation of the brachial artery (FMD-BA) and intima-media thickness (IMT) of brachial arteries were performed at baseline and after 9 months. Patients were randomized into 2 groups: 1 - 45 men receiving TRT, using 1% transdermal T-gel and 2 (control group) - 50 men, who were not assigned to TRT. Statistical analysis was carried out using the Wilcoxon - test for repeated measurements (STATISTICA 10 software package). Results During the treatment period no serious adverse events were registered. The study revealed a significant decrease in HbA1c (p<0.001), total cholesterol (p=0.009), triglycerides (p=0.004) and LDL (p=0.01) levels as well as an increase in HDL level (p=0.001) in the 1st group vs baseline and compared to the 2nd one. There was a decrease in IMT (from 1.0 [0.8; 1.1] to 0.7 [0.6; 0.9] mm, p=0.002) in the 1st group compared to the 2nd (p<0.001). The endothelium-dependent FMD-BA increased 1.5 - fold in men applying T-Gel vs baseline as well as compared to the control group at the time point of follow- up (p<0.001). At the same time TRT leaded to acceleration of the time until maximal vasodilation – it became twice faster (from 120 [90; 120] to 60 [60; 90] sec, p<0.001) compared to baseline and the 2nd group (p<0.001). Linear blood flow velocity increased markedly in the 1st group compared to the 2nd at the time point of follow-up (p=0.03). Conclusion TRT in men with T2DM and T deficiency leads to an improvement in carbohydrate and lipid metabolism together with a significant decrease in IMT and enhancement of endothelium vasomotor function, seems to facilitate a reduction in cardiovascular risk. A sex difference of carbohydrate and lipid metabolism disorders in rats with type 2 diabetes has been studied. It was established that type 2 diabetes leads to a more pronounced deterioration in carbohydrate toleranceand insulin sensitivity in males compared to female rats, but the sex doesn’t affect basal glycemia and fructosamine levels. It was found that the increase of body weight and visceral fat in rats with type 2 diabetes is moremanifested in females than in males. It has been determined that hypertriglyceridemia is higher in diabeticmales compared to diabetic females, and the level of common lipids in the liver, both intact females and femaleswith type 2 diabetes, is lower than that of the males. The obtained results indicate a more expressive impairment of glucose and lipid metabolism in males compared to females with type 2 diabetes

Aim. Our aim was to study how different BMI scores may influence the levels of inflammation, oxidative stress, adipogenesis, glucose, and lipid metabolism, in lean, overweight, and obese Portuguese patients with type 2 diabetes mellitus (T2DM). Methods. We studied 28 lean, 38 overweight, and 17 obese patients with T2DM and 20 controls (gender and age matched). The circulating levels of oxLDL, CRP, and some adipokines—adiponectin, leptin, and chemerin—and the lipid profile were evaluated. Results. Obese patients presented significantly lower levels of adiponectin and higher leptin, oxLDL, and chemerin levels, as compared to the overweight, lean, and control groups. Overweight, compared to lean and control, subjects showed significantly lower adiponectin and higher leptin and chemerin levels; oxLDL values were significantly higher in overweight than in lean patients. Lean patients presented significantly higher chemerin values than the control. Obese patients presented significantly higher CRP values, as compared to lean patients and the control group. Obese and overweight patients presented significantly higher triglycerides values than lean patients. Except for CRP, all the observed significant changes between control and patients remained significant after statistical adjustment for the body mass index (BMI). Conclusion. The levels of leptin, adiponectin, oxLDL, CRP, and triglycerides in patients with T2DM seem to be more associated with obesity and less with diabetes. Chemerin levels were raised in lean, overweight, and obese patients, suggesting that, independently of BMI, an adipocyte dysfunction occurs. Moreover, chemerin may provide an important early biomarker of adipocyte dysfunction and a link between obesity and type 2 diabetes mellitus.

The objective: to investigate the influence of the scheme of basic pharmacologic therapy on the dynamics of carbohydrate and lipid metabolism in a group of patients with type 2 diabetes on the background of the use of group DSME. Materials and methods. The study was performed on the basis of the Department of Family Medicine and Outpatient Care within the territorial boundaries of the Kyiv agglomeration. The study included 117 patients with type 2 diabetes. The study group of patients was randomly divided into a groups of classroom training (38 people), a group of remote training (39 people) and a control group (40 people), which did not receive additional training load. Subsequently, patients from the study groups took the appropriate course in classroom (outpatient) or remote (using Skype video conferencing) formats. The education of patients methodically and meaningfully complied with the consensus recommendations of the American Diabetes Association, the American Association of Diabetes Educators and the Academy of Nutrition and Dietetics. Subsequently, the relevant indicators of diabetes were monitored at 3 and 12 months after the start of the study and the correlation of their dynamics with the basic scheme of pharmacologic therapy ts and its change during the observation period was analyzed. Results. During the observation period, changes in the scheme of hypoglycemic and hypolipidemic therapy were made in 10 (8,5 %) and 20 (17,0 %) patients. It should be noted that the relevant changes were bilateral, both in the direction of simplification and in the direction of intensification of drug therapy regimens. Analysis of the ratio of 3- and 12-month dynamics of carbohydrate and lipid metabolism and the scheme of hypoglycemic and hypolipidemic therapy in all study groups did not reveal a significant correlation. Conclusions. There is no significant correlation between the basic scheme of hypoglycemic and hypolipidemic therapy with the dynamics of carbohydrate and lipid metabolism at the stages of 3- and 12-month check-points. The lack of influence of the pharmacologic therapy scheme on the positive dynamics of diabetes mellitus emphasizes the importance of group DSME in patients with type 2 diabetes mellitus as a separate independent factor of improving of the disease.

Background: Type 2 diabetes mellitus (T2DM) is a highly prevalent disease associated with increased the risk of fracture due to altered bone micro architecture and/or poor quality as key factors. Bone remodeling appears to be impaired among patients with T2DM as both markers of bone formation and markers of bone resorption are decreased when compared to healthy subjects. Objective: This study aimed to detect if weight reduction modulates adipokines and markers of bone turnover among T2DM patients. Material and Methods: Eighty obese patients with T2DM (46 men and 34 women), their age ranged from 40-53 years and their body mass index ranged from 30-36 kg/m2 were equally assigned into 2 groups: the weight reduction group received aerobic exercises, diet regimen, where the control group received medical treatment only for 6 months. Results: The mean values of body mass index (BMI), leptin, resistin, visfatin levels significantly decreased, however the mean values of adiponectin, bone alkaline phosphatase (BAP) and serum cross-linked N-telopeptides of type I collagen (NTX) levels significantly increased in the training group. While, the results of the control group were not significant. In addition, there were significant differences between both groups at the end of the study. Conclusion: Weight loss ameliorates adipocytokines and bone turnover markers among obese patients with type 2 diabetes mellitus.

The study is devoted to the Epifamin effect on values of carbohydrate and lipid metabolism, immune status in blood, and also content of 6-sulfatohymelatonin as the main metabolite of melatonin, in urine of the patients with non-alcoholic steatohepatitis developing at type 2 diabetes mellitus. It is shown that fasting glucose, postprandial glucose level and glycated hemoglobin content in the blood of patients decreased significantly during combined treatment with the Epifamin compared to the basic therapy. The results of the Epifamin reception are as follows: more expressed normalization of lipid metabolism, and content of β -lipoproteins, cholesterol, lipoproteins of low density and high density lipoprotein, and atherogenic index in comparison with the results obtained after basic treatment. The obtained results testify to positive Epifamin effect on values of the immune status of patients: level of circulating immune complexes, the main classes of immunoglobulins – IgA, IgM, IgG. It was established that in the patients with non-alcoholic steatohepatitis developing at type 2 diabetes mellitus, 6-sulfatohymelatonin level in urine was lowered on the average by 1,2 times in comparison with control. After basic treatment the reliable changes of the 6-sulfatohymelatonin content in the patients didn’t reveal. After the combined therapy with the Epifamin, the 6-sulfatohymelatonin content in the patients increased on the average for 13.9%. The obtained results allow to conclude about correction the Epifamin effect on melatonin content as a hormone, which able to participate in regulation of carbohydrate and lipid metabolism and to have immune modulating action. The Epifamin use in basic treatment had favorable influence on the immune status, values of carbohydrate and lipid metabolism in blood in the patients with non-alcoholic steatohepatitis developing at type 2 diabetes mellitus that was apparently due to melatonin-corrective effect of this drug.
