FRAILTY IN POSTMENOPAUSAL AFRICAN AMERICAN AND HISPANIC HIV-INFECTED WOMEN

Studies suggest frailty occurs earlier in HIV-infected individuals, but data in postmenopausal HIV-infected women are lacking. We assessed the prevalence of frailty and association with anthropometric measures in HIV-infected and uninfected postmenopausal women. Fried’s frailty phenotype was measured in HIV-infected and uninfected Hispanic and African American postmenopausal women participating in a study of bone metabolism; fat and lean mass were measured by whole body dual energy x-ray absorptiometry (DXA). Multivariable logistic regression evaluated frailty risk factors. The study was conducted at Columbia University Medical Center between 2002 and 2007. The participants were 61 HIV-infected and 27 uninfected Hispanic and African American postmenopausal women. The study compared prevalence and predictors of frailty in HIV-infected and uninfected postmenopausal women. Prevalence of frailty tended to be higher among HIV-infected than uninfected controls (11.5% vs 0% p=0.07). Surprisingly, among HIV-infected women, total body fat, not lean mass, was associated with frailty in multivariate analysis. Higher prevalence of frailty in African American and Hispanic HIV-infected postmenopausal women (11.5%) was similar to the 11% prevalence reported in minority women who were 10 years older in the general population. Our data suggest that frailty occurs earlier in HIV-infected postmenopausal women, but larger longitudinal studies are necessary to confirm whether musculoskeletal aging is accelerated by HIV infection.

Rheumatoid arthritis (RA) and spondyloarthritis (SpA) are associated with changes in body composition. Ectopic intramuscular fat (IMAT) may alter muscle function and contribute to cardiometabolic disorders. In a pilot study, we analyzed IMAT in the calf with peripheral quantitative computed tomography (pQCT) and examined correlations between IMAT quantity and body composition parameters. In 20 patients with active RA and 23 with active SpA, IMAT was correlated with visceral fat (VAT; r = 0.5143 and 0.6314, respectively; p < 0.05) and total lean mass (r = 0.5414 and 0.8132, respectively; p < 0.05), but not with whole body fat mass. Total lean mass mediated 16% and 33% of the effects of VAT on IMAT in RA and SpA, respectively. In both RA and SpA, calf muscle area was correlated with total lean mass (r = 0.5940 and r = 0.8597, respectively; p < 0.05) and fat area was correlated with total body fat (r = 0.6767 and 0.5089, respectively; p < 0.05) and subcutaneous fat (r = 0.6526 and 0.5524, respectively; p < 0.05). Fat area was inversely correlated with handgrip and walking tests, and it was associated with disease activity and disability. We showed that ectopic IMAT, measured with pQCT, was correlated with VAT, but not with total body fat, in RA and SpA. This result suggests that metabolically active fat was specifically associated with IMAT.

Context: Body fat distribution varies among different ethnic groups, yet less is known regarding differences in lean mass and how this may affect insulin resistance. Objective: Our objective was to compare total body fat to lean mass ratio (F:LM) in Aboriginal, Chinese, European, and South Asian individuals with differences in insulin resistance. Participants, Design, and Setting: Aboriginal (196), Chinese (222), European (202), and South Asian (208) individuals were recruited across a range of body mass index to participate in this cross-sectional community study. Main Outcome Measures: Total body fat, lean mass, and insulin resistance were assessed using homeostasis model assessment (HOMA). Results: After adjustment for confounders and at a given body fat, South Asian men had less lean mass than Aboriginal [3.42 kg less; 95% confidence interval (CI) = 1.55–5.29], Chinese (3.01 kg less; 95% CI = 1.33–4.70), and European (3.57 kg less; 95% CI = 1.82–5.33) men, whereas South Asian women had less lean mass than Aboriginal (1.98 kg less; 95% CI = 0.45–3.50), Chinese (2.24 kg less; 95% CI = 0.81–3.68), and European (2.97 kg less; 95% CI = 1.67–4.27) women. In adjusted models, F:LM was higher in South Asian compared with Chinese and European men and higher in South Asian compared with Aboriginal, Chinese, and European women (P &lt; 0.01 for all). Insulin and HOMA were greatest in South Asians after adjustment; however, these differences were no longer apparent when F:LM was considered. Conclusions: South Asians have a phenotype of high fat mass and low lean mass, which may account for greater levels of insulin and HOMA compared with other ethnic groups.

Abstract Bone is increasingly recognized as an endocrine organ that can regulate systemic hormones and metabolism through secreted factors. Although bone loss and increased adiposity appear to be linked clinically, whether conditions of increased bone formation can also change systemic metabolism remains unclear. In this study, we examined how increased osteogenesis affects metabolism by using an engineered G protein-coupled receptor, Rs1, to activate Gs signaling in osteoblastic cells in ColI(2.3)+/Rs1+ transgenic mice. We previously showed that these mice have dramatically increased bone formation resembling fibrous dysplasia of the bone. We found that total body fat was significantly reduced starting at 3 weeks of age. Furthermore, ColI(2.3)+/Rs1+ mice showed reduced O2 consumption and respiratory quotient measures without effects on food intake and energy expenditure. The mice had significantly decreased serum triacylglycerides, leptin, and adiponectin. Resting glucose and insulin levels were unchanged; however, glucose and insulin tolerance tests revealed increased sensitivity to insulin. The mice showed resistance to fat accumulation from a high-fat diet. Furthermore, ColI(2.3)+/Rs1+ mouse bones had dramatically reduced mature adipocyte differentiation, increased Wingless/Int-1 (Wnt) signaling, and higher osteoblastic glucose utilization than controls. These findings suggest that osteoblasts can influence both local and peripheral adiposity in conditions of increased bone formation and suggest a role for osteoblasts in the regulation of whole-body adiposity and metabolic homeostasis.

AbstractObjectivePrevious studies showed an inconsistent association of fruit and vegetable consumption with bone health. We assessed the associations in Chinese adolescents, young and postmenopausal women.DesignA cross-sectional study conducted in China during July 2009 to May 2010.SettingBone mineral density (BMD) and content (BMC) at the whole body, lumbar spine and left hip were measured with dual-energy X-ray absorptiometry. Dietary intakes were assessed using an FFQ. All these values were separately standardized into Z-scores in each population subgroup.SubjectsOne hundred and ten boys and 112 girls (11–14 years), 371 young women (20–34 years, postpartum within 2 weeks) and 333 postmenopausal women (50–70 years).ResultsAfter adjustment for potential covariates, analysis of covariance showed a significantly positive association between fruit intake and BMD and BMC in all participants combined (P-trend: < 0·001 to 0·002). BMD Z-score increased by 0·25 (or 2·1 % of the mean), 0·22 (3·5 %), 0·23 (3·0 %) and 0·25 (3·5 %), and BMC Z-score increased by 0·33 (5·7 %), 0·25 (5·8 %), 0·34 (5·9 %) and 0·29 (4·7 %), at the total body, lumbar spine, total hip and femoral neck in participants belonging to the top tertile compared with the bottom tertile of fruit intake (all P < 0·05), respectively. There was no significant association between vegetable intake and bone mass at all bone sites studied except for total body BMD (P = 0·030). Relatively more pronounced effects were observed in boys and postmenopausal women.ConclusionOur findings add to the existing evidence that fruits and vegetables may have a bone sparing effect.

Consequently, in Singapore, we have witnessed good sales on motorized mechanical oscillators touted to induce slimming based on the principles of acupressure. However, to date, no study has been done to evaluate the efficacy of such equipment. This pilot study aims to examine the efficacy of motorized mechanical oscillators in reducing waist circumference, weight and abdominal fat percentage over 4 weeks as well as augment the data on acupressure for weight reduction. Two male subjects, one mildly overweight and the other severely overweight, were recruited. They were required to keep to their existing lifestyles and dietary habits and were instructed to wear the oscillator belt for 30 minutes each time, twice a day, as per instructions given in the manual. The machine was operated using the automatic mode. The results show a slight decrease in total body fat % ascertained by DEXA scans. There was also a reduction in waist circumference and an increase in lean mass in the trunk area in both subjects. Specifically, one participant (mildly overweight) lost 1.2% of total body fat, and 0.7cm on the waist girth but gained 1 kg of lean trunk mass. The other participant (severely overweight), although having lost only 0.2% of total body fat and 1.3cm on the waist girth, lost 1.7 kg in total body weight. He also gained in trunk lean mass that resulted in trunk fat reduction of 0.4%. There appears to be some success in such treatment modality particularly with regard to reducing abdominal fat. (1) Background: Regucalcin or senescence marker protein 30 (SMP30) is a Ca2+ binding protein discovered in 1978 with multiple functions reported in the literature. However, the impact of exercise training on SMP30 in humans has not been analyzed. Aging is associated with many detrimental physiological changes that affect body composition, functional capacity, and balance. The present study aims to investigate the effects of whole body vibration (WBV) in postmenopausal women. (2) Methods: A total of 13 women (aged 54.3 ± 3.4 years) participated in the study. SMP30, body composition (fat mass, lean mass, and bone mass) and physical fitness (balance, time up and go (TUG) and 6-min walk test (6MWT)) were measured before and after the 12 weeks of WBV training. (3) Results: The WBV training program elicited a significant increase in SPM30 measured in plasma (27.7%, p = 0.004) and also in 6MWT (12.5%, p < 0.001). The WBV training also significantly reduced SPM30 measured in platelets (38.7%, p = 0.014), TUG (23.1%, p < 0.001) and total body fat mass (4.4%, p = 0.02). (4) Conclusions: There were no significant differences in balance, lean mass or bone mass. The present study suggests that 12 weeks of WBV has the potential to improve SPM30, fat mass, TUG and 6MWT in postmenopausal women.

Introduction: The prevalence of obesity is higher among African American girls compared to Caucasian girls. However, on average, African American girls also reach puberty earlier. Hypothesis: Sexual maturation affects adiposity differences between African American and Caucasian girls. Methods: The sample included 117 African American and 82 Caucasian girls aged 5–18 years. Waist circumference (cm) was measured in our outpatient clinic. Total body fat (grams) was measured by dual-energy x-ray absorptiometry. Abdominal subcutaneous and visceral adipose tissue mass (SAT and VAT in grams) were measured by magnetic resonance imaging. Abdominal images were obtained between the highest point of the liver and the lower pole of the right kidney. Analyzed slices centered on the lumbar inter-vertebral space (L4-L5), were 4.76 cm apart, and depending on the participant’s stature included 2 slices below and 2–5 slices above the L4-L5 interspace. Sexual maturity (breast development on a scale of 1–5 according to the criteria by Tanner) was determined, either by self-report (ages >9 years) or physician assessment (ages 5–9 years). Results: In linear regression models adjusted for age and race, sexual maturation significantly predicted waist circumference (beta=8.1, 95% CI=5.9 to 10.2), total body fat (beta=6767.2, 95% CI=4935.7 to 8598.8), SAT (beta=1988.4, 95% CI=1383.9 to 2592.9) and VAT (beta=56.6, 95% CI=36.8 to 76.4). African American girls had significantly higher waist circumference (p=0.0089), body fat (p=0.0171), and SAT (p=0.0184) compared to Caucasian girls when controlling for age only, but these ethnic differences attenuated to non-significance when sexual maturation was controlled. In linear regression models adjusted for age, sexual maturation, and body fat, although the race differences (African American compared to Caucasian girls) in SAT and waist circumference were attenuated, VAT was significantly lower (p<.0001). Conclusion: Sexual maturity was a significant independent predictor of adiposity in African American and Caucasian girls. Controlling for sexual maturation attenuated the race difference in waist circumference, body fat, and SAT, whereas the race difference remained in VAT. In conclusion, these results indicate that it is important to consider differences in sexual maturation when evaluating differences in adiposity between African American and Caucasian girls.
