Objectives: The relationship between breathing and swallowing is very dynamic during swallowing and these are highly temporally coordinated to protect the airway. The ability to protect airways during swallowing deteriorates with aging. In this study, we attempted to compare the breathing-swallowing pattern and temporal change in the respiration and swallowing coordination between young and elderly adults in Korea.Methods: A total of 80 normal people, including 40 young and 40 elderly people participated in this study. For measurement of breathing and swallowing coordination, Digital Swallowing Workstation<sup>TM</sup> was used during a 5 mL water swallowing task. Temporal parameters related to breathing-swallowing including AS (acoustic start), AP (acoustic peak), AD (acoustic duration), SAS (swallowing apnea or respiration pause start), SAD (swallowing apena duration), sES (submental sEMG start), sEP (submental sEMG peak), sED (submental sEMG duration). Additionally, DHI (Dysphagia Handicap Index) was evaluated for self-assessment of the degree of difficulty swallowing.Results: Older adults displayed delayed swallowing-related acoustic signal measurements, swallowing apnea measurements, surface EMG measurements, and delayed sequential coordination time of swallowing-related structures during swallowing. There were no significant differences according to gender. Furthermore, a significant positive correlation was observed between the total K-DHI scores and as well as swallowing apnea duration in the elderly.Conclusion: In the older population, the different breathing-swallowing pattern from that of young adults may increase the risk of dysphagia. In addition, swallowing delays due to aging can be an indicator of elderly swallowing disorders. Moreover, an increase in apnea time during swallowing may be a phenomenon that appears as a mechanism for airway protection in the elderly. However, the high correlation between apnea time and K-DHI score in the elderly may make it difficult to maintain respiration for a long time during swallowing as the respiratory function decreases due to aging, which may increase the risk of experiencing symptoms such as choking and affect the degree of subjective swallowing disorder. This suggests that even if classified as a normal elderly person without a pathological swallowing disorder, swallowing training is needed to prevent swallowing disorders and to enhance swallowing ability for older people with degraded swallowing-related abilities.

The article analyzes consumer practices and strategies of elderly, substantiates the choice of a complex criterion for typologizing consumption strategies, including the main key indicators - economic and psychological characteristics of the choice of a course of action, self-assessment of life satisfaction, the orientation of social expectations. The choice of these indicators is due to the appropriateness of their application in the study of the relationships between consumer behavior, inequality and well-being of older people. The approbation of the complex criterion in the model of the matrix of consumer behavior styles is implemented on the data of sociological surveys of people aged 55+ living in cities and rural areas of the Tomsk region, conducted in 2015 and 2019 within the framework of the scientific project “Assessment and improvement of social, economic and emotional well-being of the elderly” of the International Scientific Educational Laboratory for the Improvement of Wellbeing Technologies of Older Adults at Tomsk Polytechnic University. The analysis of sociological data made it possible to identify the relationship between material well-being and the subjective position of an elderly person, which fixes a certain consumer style. The study also revealed the influence of the factor of the use of electronic technologies on the consumer behavior of the elderly.

In line with the increasing age of a person and the deterioration process followed by the emergence of cognitive impairment, the decline in cognitive function in the elderly can be prevented by reading the Qur'an, one of which. An elderly person who wants to learn to read the Qur'an properly and correctly must have their own difficulties, but behind that there are always supporting factors that make the elderly continue to be enthusiastic about learning and obstacles in the learning process. The community service subjects were 10 elderly in Tambalang village who wanted to learn to read the Qur'an. The data obtained came from direct activities and direct interviews. Activities run with classical learning. The difficulty of the Tambalang elderly in learning to read the Koran is that it is difficult to pronounce bold letters such as the letter, in distinguishing any similar sound of letters, in determining ikhfa haqiqi and ikhfa syafawi, determining where the recitation stops and repeating it, adjusting the length of the mad, the lack of pressure on the gunnah, in reflecting the letter qalqalah. The supporting factors are being able to read the Qur'an well and correctly from a young age, always being enthusiastic and trying hard, having high motivation. The inhibiting factor is blurred vision, it has been a long time since learning to read the Qur'an, teeth have fallen out.

RESUMOObjetivo: caracterizar a produção científica brasileira sobre a avaliação da condição de saúde da pessoa idosa na atenção básica. Método: trata-se de um estudo tipo revisão integrativa. Realizou-se a busca de artigos publicados entre os anos de 2007 e 2017 em periódicos nacionais nas bases de dados BDENF, LILACS e MEDLINE, e nas bibliotecas vituais BVS e SciELO, e utilizaram-se como descritores: idoso, nível de saúde e atenção primária à saúde. Informa-se que os resultados se apresentam em forma de figura e Análise de Conteúdo. Resultados: mostrou-se que as características socioeconômicas e demográficas, a capacidade funcional, as condições de saúde e o estado nutricional interferem na autoavaliação de saúde dos idosos e no planejamento dos serviços voltados para este público. Conclusão: entende-se que as pesquisas sobre as publicações no Brasil relacionadas à condição de saúde da pessoa idosa são de fundamental importância, pois a população apresenta um rápido envelhecimento, com o aumento na expectativa de vida e o surgimento de doenças crônicas e de incapacidades. Objetivos: Analisar fatores associados à autoavaliação de saúde em idosos com transtornos mentais. Métodos: Estudo quantitativo, analítico transversal, realizado com 138 idosos portadores de transtornos mentais em um ambulatório de saúde mental. A variável de desfecho foi a autoavaliação de saúde. As variáveis de exposição foram demográficas (sexo, idade, escolaridade e estado conjugal), atividades básicas de vida diária (ABVD), atividades instrumentais de vida diária (AIVD) e características dos transtornos mentais (idade do primeiro diagnóstico, quantidade e categorias dos diagnósticos). Para a análise, foi utilizado o teste qui-quadrado. Resultados: Ser analfabeto, ter ensino fundamental incompleto e dependência severa ou total no teste de ABVD tiveram associação com autoavaliação negativa de saúde. A análise estatística mostra significância entre características dos transtornos mentais e autoavaliação de saúde. Idosos que tem apenas um diagnóstico avaliaram sua saúde como positiva. Os diagnósticos de transtornos mentais e comportamentais do grupo F20-F29 segundo a 10ª edição da Classificação Internacional de Doenças (CID-10) apresentaram associação com autoavaliação positiva de saúde. Já os do grupo F30-F39 tiveram uma autoavaliação negativa de saúde. Conclusões: Há associação de fatores demográficos, do grau de dependência para ABVD e das características dos transtornos mentais com a autoavaliação de saúde dessa população. Esses dados mostram a necessidade de maior conhecimento das condições de saúde dos idosos com transtornos mentais, no sentido de implementar medidas que visam minimizar o impacto da doença mental no processo de envelhecimento.

In outpatient care the home care, including hospices, is recognized as a model for providing quality, cost-effective and charitable care. The focus is mainly on the care that helps everyday lifeof the patient as well as the relatives, rather than on treatment, and in most cases it takes place in the patients' home. In Europe, in recent years there has been a real "boom" in home care due to demographic processes linked to increased needs for elderly care and chronically ill under the conditions of limited financial resources.In outpatient medical care in our country by means of a national framework contract there are regulated visits to the patient's home by a doctor, as well as visits by medical staff employed by him - nurse, midwife, medical assistant / paramedic / for manipulation, counseling and monitoring. At the same time there is no regulated legal activity in the Republic of Bulgaria, which is essentially the subject of home care.Since 1994 „Caritas“ has carried out the "Home Care" service, which provides a complex - health and social care for over 360 sick adults in a place where the elderly person feels the most comfortable - in their own home. „Caritas Home Care“ is provided by mobile teams of nurses and social assistants who visit the elderly at home and provide them with the necessary care according to their health and social needs.With the establishment of the first „Home Care Center“ in Lozenets region, Sofia, with the support of the PHARE ACCESS program in 2003, the Bulgarian Red Cross introduces in Bulgaria an integrated model for provision of health care and social services in the home of adults, chronically ill and people with permanent disabilities. To date, there are a number of problems in home care related to the realization of home care for patients in need in out-of-hospital settings: lack of legal regulation for home care, lack of qualified staff in outpatient care; lack of organization and structures for care; unsettled funding and the inability of the part of the population that is most in need of care to pay for it, there is no regulation to control the activity. Although home care began over 20 years ago, our country is yet to make its way to the European program called „Home care in Europe“.

Falls are one of the leading causes of permanent injury and/or disability among the elderly. When these people live alone, it is convenient that a caregiver or family member visits them periodically. However, these visits do not prevent falls when the elderly person is alone. Furthermore, in exceptional circumstances, such as a pandemic, we must avoid unnecessary mobility. This is why remote monitoring systems are currently on the rise, and several commercial solutions can be found. However, current solutions use devices attached to the waist or wrist, causing discomfort in the people who wear them. The users also tend to forget to wear the devices carried in these positions. Therefore, in order to prevent these problems, the main objective of this work is designing and recollecting a new dataset about falls, falling risks and activities of daily living using an ankle-placed device obtaining a good balance between the different activity types. This dataset will be a useful tool for researchers who want to integrate the fall detector in the footwear. Thus, in this work we design the fall-detection device, study the suitable activities to be collected, collect the dataset from 21 users performing the studied activities and evaluate the quality of the collected dataset. As an additional and secondary study, we implement a simple Deep Learning classifier based on this data to prove the system’s feasibility.

AbstractGait disorders are common in the elderly as there are various causes of neurological and non-neurological conditions. On the other hand, most of the gait parameters do change with advancing age which is identified as age-related physiological changes in gait. At this point, the discrimination between age-related physiological changes and gait disorders may be strictly challenging. After identifying gait as an abnormal pattern, classification of it and making the responsible pathophysiology also require high-level expertise in this regard. Herein, we present a rare patient with corticobasal degeneration (CBD) who had admitted initially due to complaints of gait problems. Over a long time, the patient had received the misdiagnosis of gait abnormality due to musculoskeletal problems by multiple physicians. However, the detailed neurological exam showed a higher level gait disorder (HLGD). Further investigations at this point yielded the diagnosis of CBD.

A sample of 504 elderly persons living within a metropolitan area were questioned about their socio-medical characteristics and administered the Wakefield Self-assessment Depression Inventory. The distribution of depression scores indicated that 34.5% were depressed. Stepwise regression analysis was next used to examine the relationships between the characteristics and depression scores. Total number of self-reported symptoms and being partially housebound were positively associated with depression. Recommendations were made that health care providers for the elderly be alerted that older persons with physical complaints and those who are partially restricted to their homes may tend to develop levels of depression which might require psychiatric intervention.
