Descritores: Enfermagem; Teleorientação; Hipertensão; Coronavírus; Pandemia.RESISTANT HYPERTENSIVE TELEORIENTATION DURING THE COVID-19 PANDEMIC: AN INNOVATIVE ACTION IN NURSINGObjective: to report on the use of teleorientation by nursing as a strategy directed to hypertensive patients in social isolation under the care of a specialized outpatient clinic. Methodology: this is an experience report about teleorientation with 110 hypertensive patients, from April 02 to 07, 2020 by nurses and nursing students, using a spreadsheet and a “guide text”, aiming at active listening , composed of questions about health condition monitoring and self-care in the COVID-19 pandemic. Results: of the 110 hypertensive patients, 53 (48.2%) were contacted, while 57 (51.8%) had no contact for operational reasons of telephony. Among those contacted, there was a predominance of women (70%) and 63% (33) aged ≥ 60 years. Regarding the reports, feelings, clinical complications, two probable cases of contamination by the virus, aspects of difficulty in maintaining social distance due to socioeconomic need and the importance of reinforcing flu vaccination for hypertensive patients stand out. Final Considerations: the teleorientation used by nursing became a strategy and an innovative action directed at hypertensive people in social isolation during the COVID-19 pandemic, and points to a valuable moment of contact, of approach, in which information, feelings are shared and expectations.Descriptors: Nursing; Teleorientation; Hypertension; Coronavirus; Pandemic.

Introduction: Pharmacogenetic (PGx) implementation has lagged behind the development of drug/gene pair guidelines. Materials & methods: This was a prospective study assessing the integration of PGx through medication therapy management in an outpatient clinic. Variables collected included patient diagnosis, current medications, failed or discontinued medications, PGx results/recommendations, turnaround time and pre/post clinical ratings. Results: A total of 91 participants completed study procedures with an average enrollment of approximately one consult per week. Participants were referred for testing primarily for guidance for current and future medications. The average number of recommendations per participant was 0.93. Conclusion: Integrating PGx testing into medication therapy management is feasible with PGx results available in under a week resulting in clinical recommendations in over half of patients tested.

AbstractPrimary aldosteronism (PA) is a severe and often underdiagnosed form of secondary hypertension. Determining the aldosterone to renin ratio (ARR) in hypertensive patients has been shown to be a valuable screening test for identification of patients suffering from PA. Since the introduction of a more widespread ARR screening the number of PA patients significantly increased worldwide. Interpretation of ARR might be challenging: Several factors from posture to interfering drugs affect the ARR and need to be taken into account when collecting samples. In addition, the wide variety of available assay methods and lack of well-established cut-offs present a challenge to the clinician. This review discusses the usefulness and possible difficulties of ARR screening.

1. The intracellular Na+ and Ca2+ activity and Na+ concentration were measured in erythrocytes of normotensive subjects, with and without a familial disposition to hypertension, in essential hypertensive patients with and without a family history of hypertension, and in patients with secondary hypertension. 2. In normotensive subjects without a genetic trait of hypertension intracellular Na+ activity and concentration were 7.00 ± 1.38 mmol/l and 5.67 ± 0.95 mmol/l respectively. The intracellular Ca2+ activity was 4.82 ± 4.49 μmol/l. In normotensive subjects with a familial hypertensive disposition intracellular Na+ activity and concentration were 9.74 ± 1.43 mmol/l (P < 0.01) and 6.63 ± 0.88 mmol/l (P < 0.05). Intracellular Ca2+ was 9.59 ± 9.71 μmol/l (P < 0.05). 3. Essential hypertensive patients without a familial genetic trait had an elevated intracellular Na+ activity (8.35 ± 2.08 mmol/l, P < 0.05). Intracellular Na+ concentration was 6.64 ± 0.79 mmol/l (P < 0.05). The intracellular Ca2+ activity was markedly elevated to 25.33 ± 19.03 μmol/l (P < 0.01). The essential hypertensive patients with a familial disposition had an elevated intracellular Na+ activity (17.19 ± 4.37 mmol/l, P < 0.001) and Ca2+ activity (32.8 ± 32.51 μmol/l, P < 0.01). The intracellular Na+ concentration was 6.25 ± 1.23 mmol/l. 4. The results indicate that in essential hypertension intracellular Na+ activity is increased, particularly in patients with a familial disposition for hypertension. Intracellular Ca2+ is increased in essential hypertension whether or not there was a family disposition to hypertension.
