A 65 year old gentleman was admitted with the diagnosis of exophytic tumor arising from the stomach wall. His vitals were normal. Patient was poorly built and poorly nourished with BMI of blood chemistry, ECG, and ECHO was normal. His past medical and surgical history was not significant. Urethral catheterization was performed pre-operative with strict aseptic technique. Patient underwent   laparotomy with subtotal gastrectomy and gastrojejunostomy. Catheter care was done regularly by trained staff. Catheter was removed on third post operative day. After removal of catheter, Patient had a difficulty in passing   urine. On examination, single meatal ulcer was noted (Figure 1). Post –operative course was unremarkable. Patient is in follow up since last six month without any problem.

Reactive multinucleated osteoclast-like giant cells (OGCs) have been described in a variety of neoplasms but rarely in gastric carcinomas. Reported herein is a case of an 81-year-old Caucasian male presented with upper abdominal pain and dysphagia. Esophagogastroscopy revealed an ulcerative mass and a specimen of subtotal gastrectomy and lower esophagectomy was sent for histologic examination. At the gastroesophageal junction an exophytic tumor, measured 2.2 cm in greatest diameter, was observed. Sections from the tumor showed gastric adenocarcinoma, stage pT1bpN0. Diffusely among the neoplastic cells multinucleated giant cells, resembling osteoclasts, were observed, which were positive for CD68, lysozyme, and vimentin and negative for AE1/AE3, CK8/18, hHCG, and LMP1. Moreover, in a random section from the gastric fundus, a spindle cell lesion, sized 0.6 cm, was revealed, which was positive for CD117 and CD34 antigens and was diagnosed as gastrointestinal stromal tumor (GIST). The presence of OGCs is an uncommon finding in gastric carcinomas and by analogy to breast and pancreatic carcinomas it could characterize a rare distinct morphological variant of gastric adenocarcinoma. Due to the limited number of the reported cases, the prognostic value of OGCs is under discussion. Furthermore, pathologists should be aware that incidental GIST may accompany any tumor.

Background: Solitary fibrous tumors (SFTs) are benign tumors derived from mesenchymal tissues that predominantly occur in the pleura. Establishing the diagnosis of these very rare intramedullary spinal lesions, with no clear-cut pathognomonic radiographic characteristics, is particularly challenging. Case Description: Two males, 30 and 41 years of age, presented with progressive cervical myelopathies attributed to a cervical intramedullary exophytic tumor with associated spinal cord edema. One patient showed that the lesion was highly vascularized. Both patients underwent surgical excision of firm, solid, focal, and, particularly in one of them, very vascular/hemorrhagic tumors; at surgery, there was some adherence between the tumors and the cord tissue, but gross-total resections were achieved in both cases, demonstrated on postoperative MR scans. Histological and immunohistochemical findings confirmed the diagnosis of SFT (WHO Grade I). After a 6-month postoperative period, both patients neurologically improved and had no MR evidence of tumor recurrence. Conclusion: Intramedullary cervical exophytic SFTs are extremely rare. Although these solid tumors may present with hemorrhagic features and at surgery demonstrate significant adherence to the pial/cord surface, complete surgical resections are feasible resulting in good outcomes.

Abstract Objective and Importance: Surgery for excision of intramedullary spinal cord tumors without increasing neurological deficit is one of the more difficult operations in spinal surgery. In particular, infiltrating astrocytomas without a clear cleavage between the tumor and normal spinal cord parenchyma are difficult to remove totally without producing additional neurological impairment. In this study, we describe a two-stage resection facilitating total resection of intramedullary tumors. Clinical Presentation: Three cases of spinal cord astrocytomas were treated using a two-staged method. Technique: The first surgery included myelotomy, biopsy and duroplasty. A thin expanded polytetrafluoroethylene sheet was placed between the dorsal surface of the spinal cord and dura mater to prevent adhesions. Two or 3 weeks after surgery, a second surgery was performed to remove the now exophytic tumor. Results: Magnetic resonance imaging scans showed exophytic extrusion of the tumor in all three cases before the second operation. In each patient, we were able to carry out gross total removal of the tumors without additional neurological deficit except for transient joint position sense loss in one case. All three patients remain neurologically stable without evidence of tumor recurrence more than 3 years after surgery. Conclusion: A two-stage operation may enhance the surgeon's ability to completely resect extensive low-grade spinal cord astrocytomas and, at the same time preserve neurological function.

Aim. Oral lichen planus (OLP) is one of the common chronic inflammatory, noninfectious, and precancerous oral mucosal diseases that affect the stratified squamous epithelium in adults. Grinspan et al. reported an association of OLP with diabetes mellitus and vascular hypertension and called that Grinspan’s syndrome in 1966. We aim to present a case of Grinspan’s syndrome with malignant transformation. Case Presentation. A 60-year-old man who presented with a ten-year history of OLP diagnosed clinically and histologically was referred to our otolaryngology department with a painless swallowing in the left buccal mucosa for 3 months. Clinical examination revealed several plaques, striated white lesions in the buccal mucosa bilaterally, and an exophytic tumor in the left buccal mucosa. Histopathological examination showed lichen planus bilaterally and oral squamous cell carcinoma in the left buccal mucosa. The tumor had been developed on the preexisting areas of lichen planus which had been histologically proven before. The tumor was removed completely, and the tissue defect on the buccal mucosa was repaired with a split-thickness skin graft. Conclusion. Patients with OLP should be followed up periodically in a long term at close intervals for early diagnosis of malignant transformation.

The Purpose of the study: define the indications and contraindications to sphincterpreserving operation in lowerampullar cancer of rectum.The Materials and methods: In coloproctological branch of National Oncological Scientific centre (NOSC) in  2005-2009 years 142 patients were  performed radical operations,  64 (45,1%) patients  were  performed sphincterpreserving operations – abdominoanal resection of rectum (AARR), medium age of  patients  - 59,8 years, beside 78 (54,9%) patients were  performed abdomeno - perineal extirpation of rectum (APER), medium age - 54,2 years.The Results: Studied results of the morphological research of removed preparation by histological structure of tumors and by form of the growing, depending on locations of the tumors on toothed line. The remote postoperative results studied beside 51 (79,7%) patient with AARR and beside 63 (80,1%) patients with APER.The Findings: sphincterpreserving operations (SPO) of the rectum can be the operation of the choice in treatment of high differentiated adenocarcinoma (HDA) and medium differentiated adenocarcinoma (MDA) in stages T2-3 in localization of the lower pole of  tumors, not below1 centimeter of toothed line (TL) and carries combined character, in  location of the lower edge of tumors on toothed line in patients with HDA MDA  in 4% and 2,2% cases are shown that completing intrasphincternal resections of rectum, in connection with particular aggressiveness LDA question about choice of the method of the treatment under their localizations directly on TL must be solved  in favour of complete APER, in exophytic tumor located onto and above TL is indication for sphincterpreserving operation in type of AARR, indices recurrence-free and not metastatic period, one-year death-rate, three-year survivability after SPO at inferior ampular cancer of rectum has not an essential distinction from results after APER.
