Experience of the Use of Chondroitin Sulfate in the Recovery Treatment of Posttraumatic Chondropathy of the Knee in Athletes Playing Sports

The article presents results of research on the effectiveness of comprehensive rehabilitation treatment of athletes with post-traumatic chondropathies of knee joints by means of chondro-protective therapy (chondroitin sulfate injections). The study involved 60 athletes playing sports. Athletes were examined through a survey questionnaire Knee injury and osteoarthritis outcome score - Scale Exodus of injury and osteoarthritis of the knee joint, as well as clinical examination, ultrasound examination of the knee joints, bilateral isokinetic testing of the muscles of the flexor-extensor of the knee joint. 2 groups of 30 athletes (the main group and the comparison group) had course of treatment: physical therapy by alternating magnetic field, therapeutic physical training, massage. Athletes of the main group was additionally applied intramuscularly drug chondroitin-sulfate (Astragal). The study showed greater efficacy of the treatment of athletes in the main group, this is confirmed by a significant decrease of pain syndrome, improving of functional status of the knee joints, increase sporting activity and improving the quality of life of athletes according to the survey; improving the biomechanical characteristics of the periarticular muscles and deficit reduction extensor tibiae between damaged and intact limb at angular velocities of 60 and 180 0/с, the positive dynamics of the ultrasonic examination of the knee joints.

In 40 patients with patellofemoral pain syndrome, before and after the course of treatment, the strength indices of the hip muscles were studied by isokinetic dynamometry, as well as the severity of the pain syndrome in the knee joint and the lumbosacral spine according to the visual analogue scale. The patients of the main group (n = 20) used the technique of complex correction of the pelvic region and lower limb, which was mainly aimed at stabilizing the lumbosacral spine. Patients of the control group (n = 20) performed therapeutic gymnastics aimed at training the muscles of only the interested lower limb. The treatment was performed on an outpatient basis daily for 21 days. After the course of treatment in patients of the main group, the strengths of the quadriceps femoris were significantly higher (p < 0.05), and the degree of pain in the knee joint and lumbar spine was significantly lower (p < 0.05). The obtained data confirm the necessity and expediency of including exercises for stabilization of the lumbosacral spine in the rehabilitation program for patients with patellofemoral pain syndrome.

Aim. The study on the effectiveness of complex therapy for osteoarthritis (OA) of the knee joint was conducted in real clinical practice. Materials and methods. The survey involved 125 patients aged fr om 50 to 70 years (25 men and 100 women) with a diagnosis of knee joint OA (the III roentgenologic Kellgren-Lawrence stage).The average age of the patients was 62±3.21, the average duration of the disease - 9.4±2.8 years. Patients were randomly assigned to three groups of 35 people, the control group had 20 patients. Group 1 patients received non - steroidal anti - inflammatory drugs (NSAIDs) + Injectran(Chondroitin sulfate) 200 mg intramuscularly (I.M.) every other day No. 25.In group 2, patients received NSAIDs + Fermatron 1% 2 ml with an interval of 7 days intra - articularly (I.A.) No. 3. In group 3 - NSAIDs + Injectran 200 mg (I.M.) every other day No. 25 + Fermatron 1% 2 ml with an interval of 7 days (I.A.) No. 3. In the control group (20 people), patients received only NSAIDs. Evaluation of the symptoms was carried out using the WOMAC index before the start of thetherapy, after 8 and 12 weeks of treatment. The intensity of pain while walking was estimated on a visual analogue scale. Results. The article presents the literature data and the results of our own investigation on the efficacy and safety of Muscomed cream in the treatment of patients with osteoarthritis of the knee joint. The study included 20 women (mean age 62 years). The study design included clinical and radiological examination of patients and survey using the Western Ontario and McMaster Universities Arthritis Index (WOMAC) and the visual analogue scale at the beginning of treatment, on days 7 and 14 of using Muscomed cream. Muscomed cream was applied topically as a component of physiotherapeutic procedures as a part of a comprehensive conservative treatment. Two weeks after completing the course of treatment, there was a significant decrease in the intensity of pain syndrome and a decrease in the total WOMAC index in patients who used Muscomed cream locally. There was also a more pronounced decrease in the need for non-steroidal anti-inflammatory drugs in patients of the main group. The safety and efficacy of the Muscomed cream in the treatment of osteoarthritis of the knee joints were demonstrated, which resulted in a decrease in the severity of pain syndrome and an improvement in the functional activity and quality of life of such patients.

Osteoarthritis (OA) affects both elderly people, for whom it is one of the main causes of disability, and people of active working age and is an urgent clinical and social problem of resistance of pain syndrome to therapy. The disease is characterized by both destruction of intra-articular and paraarticular structures, such as subchondral bone. While OA is an important sign of pathological changes believe the bone marrow edema (BME). This work examines the effect of BME on development osteoarthritis, and therapeutic approaches to the management of patients with OA. The aim of the study was to develop a method of treatment of BME in OA of the knee joint by locally intraosseous injection of autologous thrombotic-rich plasma (PRP) into the edema zone. In this study 17 patients with the diagnosis: Osteoarthritis II-IV Grade. according to the classification of Kellgren–Lawrence, in which areas of local inflammation in the form of BME were detected on MRI in the subchondral zone in accordance with the international classification of WORMS (Whole Organ Magnetic Resonance Imaging Score). The mean age of patients was 41,7 ± 14,3 years, 10 of them were women and 7 men. Patients were treated with autological platelet-rich plasma under x-ray control injected from extra-articular intraosseous access in the area of BME. Evaluation of effectiveness of treatment performed by VAS, WOMAC and KOOS scales, before the introduction of autoplasma, after 1 and 3 months after the start of treatment. Three months after the manipulation, there was a statistically significant decrease in the intensity of inflammatory syndrome: for WOMAC by 17.5%, for KOOS by 19.4% and for VAS by 33,1% (p < 0,01). Thus, the efficiency of intraosseous Infiltration of autologous platelet-rich plasma in the treatment of patients with OA, accompanied by edema of the bone marrow in the subchondral zone, was proved.

A new method of administering pharmaceutical substances with the help of apparatuses for dynamic electroneurostimulation (DENS) was suggested. This method is available in home conditions, preventoriums, social centers for communication and health. The article sets forth the results of experimental works on electrophoresis of medical mud with the help of the DENS apparatuses in patient with deforming osteoarthritis of the knee joint. The possibility of effective reduction of pain syndrome intensity in patients with osteoarthritis of II-III degrees was shown.

According to WHO data, there has been a change in the morbidity structure in the past decades. The number of people with chronic pain syndrome (CPS) is increasing progressively. This is due to population aging and increases in the number of musculoskeletal disorders, including osteoarthritis of the knee joint, which causes not solely temporary or permanent loss of ability to work, but a significant decrease in quality of life in patients. According to WHO, gonarthrosis will become the fourth leading cause of disability among women, and eighth among men in the coming decade (1).

Aim: to evaluate the efficacy of two-month therapy with chondroitin sulfate (CS) (Mucosat®) injections in patients with osteoarthritis and the drug potential in terms of structure-modifying action. Patients and Methods: an open prospective study with participation of 30 patients (mean age 52.5±11.7 years) with osteoarthritis, grade 2 gonarthrosis according to the Kellgren-Lawrence classification, and severe pain syndrome (more than 50 points according to VAS) was conducted. 76.7% of subjects had concomitant pathology. The drug was prescribed according to the standard regimen with a course of up to 25–30 injections. The examination included a clinical assessment of indicators of joint syndrome, life quality, joint function using WOMAC, Lekken, KOOS, EQ-5D scales, cartilage oligomeric matrix protein (COMP) and C-reactive protein (CRP), ultrasound of the knee joints. Results: after a two-month treatment course with Mucosat®, there was a significant (p<0.05) decrease in pain intensity by 1.8 times, the number of tender and swollen joints by 2 or more times, an improvement in indicators of functional activity and life quality of patients. Reactive synovitis of the knee joints, which was present at the beginning of the study in 53.3% of cases, was preserved clinically and according to ultrasound data only in 17% of subjects by the end of treatment. There was a tendency to decrease the levels of CRP and COMP. Ultrasound of the knee joints showed a significant (p=0.000) increase in the average thickness of articular cartilage from 1.55±0.26 mm initially to 1.67±0.27 mm after 2 months of treatment. More significant positive dynamics during therapy was observed in patients without arterial hypertension. In general, no adverse events were registered during treatment. Conclusion: in clinical practice, the use of the CS injections in patients with gonarthrosis is safe and effective. The tendency to decrease the cartilage degradation and increase its thickness suggests a positive effect of CS in the form of slowing the disease progression in the long-term period. Abstract. The purpose of the work is to develop and substantiate a technique for physical rehabilitation in knee arthrosis. We assumed that the technique we developed will contribute to the speedy restoration of knee joint functions, the removal of pain syndrome and an increase in the physical activity of patients. Of greatest importance in kinesitherapy is the determination of women's motor capabilities, abilities for household and labor skills, for which muscle testing on multifunctional simulators is used. For subjective assessment of pain, we proposed a verbal descriptive scale. This method is designed to determine the intensity of pain and allows you to evaluate the subjective pain sensations that the patient experiences at the time of the study. Hydroxynesitherapy is in the physical rehabilitation program. When performing gymnastics in water, different depth of immersion is used, dynamic exercises, exercises with elastic (rubber) bandage, rubber espanders, shovels (to increase rowing resistance), flippers and special cuffs on knee joints are used. Duration of classes 15–20 min 2–3 times a week, course 30–45 days and subsequent application of teips. The rehabilitation included treatment baths, which women conducted independently at home. When conducting kinesitherapeutic measures, we took into account the patient's complaints, we carried out medical supervision, control of heart rate and blood pressure. We presented the results of the study of influence of the proposed technique on morphofunctional state of women 45–50 years old. We noted positive dynamics of influence of kinesitherapy on motor functions of knee joint.

Despite a significant number of studies on various schemes of OA therapy, there are only some studies with the use of systemic enzyme therapy (ETS) in combination with NSAIDs and / or SYSADOA. The aim was to study the effect of different groups of drugs (NSAIDs, ETS, SYSADOA) on the clinical course of gonarthrosis, as well as to develop an algorithm for the choice of treatment tactics depending on the clinical and sonographic manifestations of this disease. Methods. The data of 46 patients with stage I and stage II OA of Kellgren-Lawrence were analyzed. Patients of the first group received combined oral therapy with chondroitin sulfate (HS) 500 mg 2 times a day and glucosamine sulfate (GS) 750 mg 2 times a day in combination with selective NSAIDs. The second group of patients received combined therapy of HS 500 mg 2 times a day with the GS 750 mg 2 times daily together with Phlogenzym (PL), 2 tablets 3 times a day. All patients included in the study were examined initially, after 8 and 12 weeks. Procedures included clinical (VAS, WOMAC) and instrumental methods with the use of ultrasound of joints to evaluate the pathology of soft tissues and detect synovitis.. Results. Comparing groups 1 (HS, GS and NSAIDs) and 2 (HS and GS together with PL) on the effect on the structural state of cartilage and the dynamics of the main ultrasonic manifestations of OA knee joint, it should be noted that the effectiveness of drugs HS and GS in combination with enzyme therapy was higher, because in this group of treatment of synovitis decreased, whereas in the group of HS and GS, together with NSAIDs, these parameters either increased or remained unchanged. Conclusions. The use of polyenzyme drugs significantly increases the effectiveness of complex therapy in patients with osteoarthritis of the knee joints. In patients with gonarthrosis treated with a combination of chondroitin sulfate, glucosamine sulfate and phlogenzym was observed a significant reduction in the severity of synovitis, as well as more frequent lower daily dose of the selective NSAID or their full cancellation, compared with a control group of patients treated with symptomatic drugs retardation in combination with NSAIDs.
