Prevalence, Sex and Age Peculiarities of Chronic Heart Failure among Rural and Urban Population of the North-Ossetia-Alania

Chronic heart failure is one of the most common diseases of the cardiovascular system. It is known that the Russian epidemiological studies on the chronic heart failure are significantly different from the overseas, but at the same time it revealed differences between individual regions of the Russian Federation. A set of material was held on the basis of six medical institutions of the Republic of North Ossetia-Alania, selected as a result of the randomization. 688 families, including 1600 respondents aged from18 to 100 years of rural and urban population of North Ossetia-Alania, were examined. The diagnosis of chronic heart failure was verified on the basis of a standardized assessment of clinical symptoms used in the study of EPOCH- CHF. The prevalence of heart failure among the rural population of the North Ossetia-Alania was slightly higher and reached 14.4% , and 13.9 % in urban areas. Among both rural and urban popula-tion of the North Ossetia-Alania the prevalence of heart failure was high, more than a few times higher than international epidemiological studies and higher than epidemiological research data of EPOCH - CHF. Severe сhronic heart failure reached 3.8% in rural and 3.9 % in urban population of the entire representative sample. The prevalence of сhronic heart failure was 1.3 times higher in women than in men. Among younger age groups, the prevalence of heart failure was higher in men than in women.

Between 2006 and 2014, 439 cases of cutaneous leishmaniasis were recorded in the region of Gharb Chrarda Beni Hssen in the north-west of Morocco. With an annual incidence of 49.1 cases per year and a sex-ratio (M / F) of 0.71. The disease has affected all age groups. The most stricken population is children and young people between the ages of 6 months and 30 years with more than 60.26 % of cases. Therefore, Cutaneous leishmaniasis almost hits the rural and urban areas but with an uneven impact. It is higher in rural areas where they are registered with a percentage of 56.7% of cases in contrast to a percentage of 43.3% in urban areas.

Mass migration to urban areas constitutes the basic direct factor of the decline in rural population of Yugoslavia in the second half of the 20th century. Due to the characteristic migration patterns by age and sex, they have had a substantial impact on the change in age structure of rural population towards rapid demographic ageing. By inducing decline in fertility and an increase in mortality, the newly formed age structure is increasingly becoming one of the basic factors to further decline in population, or even the major factor to rural depopulation in the majority of regions. The paper analyzes changes in age structure of rural population in the FR of Yugoslavia and across its republics and provinces during the period from 1961 to 1991. The conditions prevailing during the last census (1991) are particularly highlighted. The author points to distinct differences in ageing of urban versus rural populations, and considerable regional differences at the achieved level of demographic age. Based on the main demographic age indicators (the share of five-year and larger age groups, average age, ageing index and movement in major age-specific contingents), the author concludes that the process of population ageing had taken place in both rural and urban populations, but was more intensive in villages (higher share of the aged, higher index of ageing and higher average age) during the period under review. The author points to distinct ageing of rural population in all republics and provinces. It was most prominent in central Serbia and Vojvodina, while being quite slow in Kosovo and Metohia and recorded mainly in between the last two censuses (1981-1991). Likewise, Kosovo and Metohia constitute the only major region of Yugoslavia in which rural population in 1991 is still demographically younger than the population in urban settlements. Rural versus urban population ageing was much more intensive in other major regions of the country, both from the base and from the apex of the age pyramid. In view of the minimal differences in fertility and mortality levels by type of settlement (particularly in central Serbia and Vojvodina), the author argues that the inherited age structure constitutes the main cause of rapid acceleration in rural population ageing in low fertility regions.

Abstract Background and Aims According to recently published WHO data, kidney disease has been the 10th leading cause of death in the world over the past 20 years. The lethality of dialysis patients with cardiovascular pathology is 3 times higher than that of patients without CVD. This is especially pronounced in developing countries. Uzbekistan is a country with a population of 34 million and has an equal urban and rural population. In this regard, it is of interest to comparatively study the structure of CVD in patients with CKD stage 5D of the rural and urban population of Uzbekistan. Method We examined 165 (90 men and 75 women) patients with CKD stage 5 receiving dialysis in urban and rural areas of Uzbekistan. The patients were divided into 2 groups depending on their permanent place of residence and the medical center where they receive hemodialysis. 104 (51 men and 53 women) patients were included in the Urban group and 61 (39 men and 22 women) patients were included in the Rural group. The average age of the urban population was 49.7 ± 1.38 years, the rural population was 45.5 ± 1.83 years. All patients underwent programmed hemodialysis according to the standard scheme for 4 hours 3 times a day (12 hours/week). The average duration of hemodialysis in the Urban group was 37.0 ± 4.77 months (M ± m) and 16.6 ± 2.4 months in the Rural group. The main causes of CKD 5D in both groups were glomerulonephritis (Urban-36.5%, Rural-62%) and type 2 diabetes (Urban-31.7%, Rural-18%). Results CVD comorbidity occurred in 55,8% (n=92) of all 165 examined patients, of which 52 were men and 40 were women. The most common CVDs in all groups were hypertension (51%, n=84), coronary heart disease, presented as angina (28%, n=47), heart failure (14%, n=23) and various types of arrhythmias (5%, n=8). 77% (n=127) of patients had anemia due ESRD. In the group Urban(n=104), 62.5% (n=65) had CVD. The main CVD was Hypertension. It was found in 92% (n=60) of patients with CVD in this group. Less (65%, n=42) were patients with angina. Heart failure was detected in 31% (n=20) of patients. Arrhythmia was diagnosed in only 5% (n=5). A large number of combined CVD have been identified. 69% (n=45) of all patients with CVD had a combined CVDs in different combinations. The most common combination was hypertension + angina (n = 26). It accounted for almost 58% of all combined cases. Only 28% (n=17) of all cases with hypertension had "isolated" hypertension. The main combination with hypertension was hypertension + angina (43%, n=26), 11% (n=11) of patients had hypertension + angina + heart failure, a combination in the form of hypertension + angina + arrhythmia had 3% (n=3) patients. Relatively fewer (n = 27, 44%) CVD were found in the Rural group. The most frequent CVD was also a hypertension. Patients with hypertension made up 89% (n = 24) of all patients with CVD in this group. The second place is occupied by angina, it was found in 18.5% (n=5) cases among patients with CVD. Combined CVS pathologies were less common in the rural group. A total of 8 patients (29.6% of all CVD cases) had several CVDs. Conclusion Сardiovascular diseases in the urban population (62.5%) occur almost one and a half times more often than in the rural population (44%). Combined CVD pathology occupies a leading place in the structure of CVD in patients with CKD 5D, both urban and rural.

Abstract This article explores the idea of movement through an analysis of the flows between rural and urban areas, more specifically between small farms (roças) and the peripheries of big cities. I turn to my own ethnographic research on rural and riverside communities in the north of Minas Gerais, as well as ethnographies produced on populations in the Cerrado Mineiro, in order to question the primacy of movement in the definitions of the city and to extend the notion through an approach that incorporates the relations between persons and things circulating in both these social spaces.

Chronic heart failure (CHF) is a complex clinical syndrome caused by a variety of heart problems, with a high incidence. The 5-year survival rate of patients with clinical symptoms is similar to that of malignant tumors. Wenyang Zhenshuai granules are a safe and effective granule of traditional Chinese medicine components, including aconite, dried ginger, licorice, and red ginseng. In contemporary clinical applications, it is widely used in acute and chronic heart insufficiency, coronary heart disease, and arrhythmia. This research cultured H9C2 cardiomyocytes and divided them into the normal control group, LncRNA-MiR143HG overexpression group, LncRNA-MiR143HG silence group, Adriamycin (ADR) group, ADR + medicated serum group, ADR + LncRNA-MiR143HG overexpression + medicated serogroup, and ADR + LncRNA-MiR143HG silence + medicated serogroup. The cells of each group were treated differently, and the survival rate of each group of cells and the expression levels of LncRNA-MiR143HG/miR-143 and ERK5 were detected at the end of the experiment, and the expression of LncRNA-MiR143HG/miR-143 in H9C2 cardiomyocytes was regulated by Wenyang Zhenshuai granules’ impact. The results of this study showed that, in the doxorubicin-induced H9C2 cardiomyocyte injury model, the expression of miR-143 was upregulated, and the expression of LncRNA-MiR143HG and ERK5 was significantly downregulated. Wenyang Zhenshuai granules can downregulate the expression of miR-143 to promote ERK5 protein expression and phosphorylation. The process is regulated by LncRNA-MiR143HG/miR-143, which may be one of its important mechanisms for the treatment of chronic heart failure.

Objective: to evaluate vasopressin (VP) concentration in patients with varying severity of chronic heart failure (CHF), intensity of clinical symptoms, and decreased level of left ventricular ejection fraction (LVEF). Materials and methods. In total, 120 patients (44 males, 76 females) with CHF of varying genesis (mean age 72.12 ± 10.18 years) and 30 clinically healthy individuals (18 males, 12 females) as a control group (mean age 33.4 ± 6.23 years) were examined. All patients underwent comprehensive clinical and instrumental examination in accordance with the standards for patients with CHF. The VP level was determined using ELISA. Statistical analysis was performed using the IBM SPSS Statistics v. 23 software.Results. The patients with CHF had significantly higher blood VP levels compared to the control group (72.91 ± 53.9 pg/ml versus 6.6 ± 3.2 pg/ml respectively; p <0.01). At the same time, patients with stage III CHF had significantly lower VP levels than patients with stages IIВ and IIА (35.61 ± 21.53 pg/ml versus 71.67 ± 48.31 pg/ml and 86.73 ± 59.78 pg/ml respectively; p<0.01). A similar picture was observed for the functional classes (FC). For instance, for CHF FC II and III, the VP level was 91.93 ± 67.13 pg/ml and 77.95 ± 54.01 pg/ml respectively, while for FC IV it decreased to 50.49 ± 28.18 pg/ml (p <0.01). The VP concentration in patients who subsequently perished was significantly lower than in patients who survived (48.79 ± 26.30 pg/ml versus 79.72 ± 57.73 pg/ml; p = 0.012). Moreover, in patients with LVEF <50 %, the VP level was significantly lower than in patients with LVEF >50 % (59.43 ± 42.51 pg/ml versus 86.43 ± 62.46 pg/ml respectively; p <0.05).Conclusion. The observed significant differences in VP in patients with stage III and IV CFH can indicate depletion of neurohumoral mediators in this patient category. However, a correlation between the VP level and the level of LVEF decrease can indicate a significant difference in the role of VP in CHF pathogenesis in patients with preserved and decreased LVEF. This observation requires further research.

Although the need for emergency service volunteers is increasing internationally, it is challenging to attract and retain firefighters, who are highly trained and from whom sustained and potentially heavy time investment is required. To recruit and retain fire service volunteers, it is important to understand the barriers these volunteers face, and how these barriers relate to their attitudes and behavioral intentions. We surveyed 762 volunteer firefighters throughout Finland on these issues. Lack of time, conflict with work/school, and other work-related challenges were the most frequently reported barriers, and they were also rated as some of the most severe barriers. The number and severity of barriers were positively related to the volunteers’ absence and turnover intentions, and conflicts with family commitments. A higher number of reported barriers was also negatively related to volunteer satisfaction. No differences in barriers were found between rural and urban areas, or between gender and age groups. We discuss the findings in regard to relevant literature from other countries and conclude with practical implications.

Стеноз аортального клапана (АК) - самое частое приобретенное клапанное заболевание с растущей распространенностью из-за старения населения, требующее хирургического или эндоваскулярного вмешательства. Транскатетерная имплантация АК (Transcatheter Aortic Valve Implantation, TAVI) представляет собой настоящую революцию в области интервенционной кардиологии для лечения пациентов старческого возраста или из группы высокого риска с тяжелым симптомным стенозом АК. TAVI позволяет выполнять коррекцию аортального порока наиболее тяжелой группе больных ХСН с выраженной коморбидностью, которым противопоказано выполнение открытой операции по замене АК, для прерывания порочного круга в прогрессировании ХСН, стимуляции обратного ремоделирования миокарда, облегчения симптомов, улучшения качества жизни и прогноза больных, а также в качестве «моста» к трансплантации сердца. Благодаря разработке устройств нового поколения, TAVI превратилась из сложной и опасной процедуры в эффективную и безопасную методику. Наибольший научный интерес в настоящее время представляет изучение долговечности клапана и возможности расширения показаний к выполнению TAVI у категорий пациентов с низким хирургическим риском, а также более молодого возраста. Aortic valve (AV) stenosis is the most common acquired valvular disease with an increasing prevalence due to an aging population, requiring surgical or endovascular intervention. Transcatheter Aortic Valve Implantation (TAVI) represents a real revolution in the field of interventional cardiology for the treatment of elderly or high-risk patients with severe symptomatic AV stenosis. TAVI allows for the correction of aortic defect in the most severe group of patients with chronic heart failure (CHF) with severe comorbidity, who are contraindicated for open surgery to replace AV, in order to interrupt the vicious circle in the progression of CHF, stimulate reverse myocardial remodeling, alleviate symptoms, improve the quality of life, and prognosis of patients, and also as a «bridge» to heart transplantation. With the development of next-generation devices, TAVI has evolved from a complex and dangerous procedure to an effective and safe technique. Currently, the greatest scientific interest is the study of valve durability and the possibility of expanding the indications for performing TAVI in categories of patients with low surgical risk, as well as in younger age groups.


Heart failure is a complex clinical syndrome of signs and symptoms that suggest the ability of the heart to pump effectively has been impaired. It is distinguished by dyspnoea, effort intolerance, fluid retention, and poor survival. The prevalence of heart failure is around 1–2% in the adult population in developed countries, and 920 000 people in the UK have heart failure. The incidence of heart failure has decreased; however, the number of people newly diagnosed with heart failure has increased. This is thought to be largely due to an ageing population, improvement in the management and survival of people with ischaemic heart disease, and effective treatment of heart failure. The condition can occur in all age groups; however, the incidence and prevalence steeply increase with age. The average age at first diagnosis is typically 77yrs. Chronic heart failure (CHF) has a poor prognosis, the mortality rate for CHF being worse than for many cancers. It is estimated that 70% of those hospitalized for the first time with severe heart failure will die within 5yrs. However, this has been improving, with 6mth mortality rate ↓ from 26% in 1995, 15% in 2009, to 8.9% in 2016. This chapter will outline the aetiology, pathophysiology, and management of CHF, including considerations for palliative care.
