The article presents the results of experimental research on monofilament suture with different biodegradation periods when implanted into trachea tissue. The data obtained via histological, morphological and morphometric methods have been shown that application of monofilament suture material with long biodegradation period causes evident inflammatory reaction of trachea tissue, which persists through the entire period of monitoring, by 30th day the cell index value is 0.4±0.05. Unlike monofilament non-absorbable suture and suture with short biodegradation period, the application of which is marked with active proliferative process on the 14th day, the cell index is equal to 0,9±0.05 and 1,1±0,06 respectively. The obtained data demonstrate that application of monofilament suture material with long-term biodegradation period in tracheal surgery is non-desirable as the inflammatory process maintained by the suture material under conditions of high contamination by bacteria and active process of mucus formation will adversely affect healing. Application of suture with short-term biodegradation period in acute trachea injury treatment is optimal due to the fact that the organ tissue shows minimal reaction to suture and 60 days later the suture is completely absorbed, that means will not cause further complications.

The present study compares a knotless, barbed, absorbable suture material against a conventional monofilament absorbable suture material in oral mucosal wound closure. The parameters measured include time of closure and differences in healing at 2 and 4 weeks postoperatively. A prospective study comparing a knotless, barbed suture system with conventional absorbable sutures was undertaken in 19 cats. Nineteen cats had full mouth extractions performed. Following the extraction procedures, the incisions in the arcades (maxillary and mandibular) were apposed with the barbed, knotless suture system in a continuous pattern on one side and with a conventional smooth suture in an interrupted pattern on the other. Suturing times for each arcade were recorded. The material used to close the first side of each cat varied. Healing, dehiscence, and other complications were assessed at 2 and 4 weeks postoperatively. The average closure time (± standard deviation) per quadrant with conventional monofilament suture was 8.7 (±1.3) minutes, while barbed suture required an average of 5.1 (±1.1) minutes per quadrant to complete the suture. This difference (95% confidence interval) of 3.6 (±3.2-4.1) minutes per quadrant was statistically significant ( P < .001). Dehiscence and ranula-like swelling formation were noted as uncommon postoperative complications, but the differences were not significant between the groups. Barbed, knotless sutures resulted in faster closure times than conventional, simple interrupted, monofilament sutures with similar healing and complication rates. To the author’s knowledge, there is no current literature comparing conventional absorbable monofilament sutures to a knotless, barbed, absorbable suture system for closure of oral mucosal incisions in cats.

Abstract. Background: Absorbable sutures are not generally accepted by most vascular surgeons for the fear of breakage of the suture line and the risk of aneurysmal formation, except in cases of paediatric surgery or in case of infections. Aim of this study is to provide evidence of safety and efficacy of the use of absorbable suture materials in carotid surgery. Patients and methods: In an 11 year period, 1126 patients (659 male [58.5 %], 467 female [41.5 %], median age 72) underwent carotid endarterectomy for carotid stenosis by either conventional with primary closure (cCEA) or eversion (eCEA) techniques. Patients were randomised into two groups according to the type of suture material used. In Group A, absorbable suture material (polyglycolic acid) was used and in Group B non-absorbable suture material (polypropylene) was used. Primary end-point was to compare severe restenosis and aneurysmal formation rates between the two groups of patients. For statistical analysis only cases with a minimum period of follow-up of 12 months were considered. Results: A total of 868 surgical procedures were considered for data analysis. Median follow-up was 6 years (range 1-10 years). The rate of postoperative complications was better for group A for both cCEA and eCEA procedures: 3.5 % and 2.0 % for group A, respectively, and 11.8 % and 12.9 % for group B, respectively. Conclusions: In carotid surgery, the use of absorbable suture material seems to be safe and effective and with a general lower complications rate compared to the use of non-absorbable materials.

Abstract Background It is uncommon for a bronchial stump-related complication to develop during the remote postoperative period in a case of obstructive pneumonia owing to migration of the suture material. Here, we describe a case of bronchial obstructive pneumonia that developed owing to migration of the suture material in the airway 8 years after pulmonary resection. Case presentation A 34-year-old woman underwent left lower lobectomy for a pulmonary carcinoid tumor (pT1bN0M0-stage IA) in 2010. She experienced obstructive pneumonia, and chest computed tomography revealed a mass protruding from the bronchial stump to the bronchial lumen in 2018. After treatment for pneumonia, flexible bronchoscopy showed the presence of a fibrous suture material (Teflon pledget) completely obstructing the left second carina. A week later, the Teflon pledget obstructing the bronchial lumen was removed using a flexible bronchoscope with the patient under general anesthesia. The procedure was completed without removing the small amount of granulation tissue because the bronchial lumen opened after removing the Teflon pledget. She has remained asymptomatic for 1 year after removal. Conclusions In this case, the complication of obstructive pneumonia developed owing to migration of the non-absorbable suture materials used to suture the bronchial stump. Bronchoscopic management of this rare complication comprised endobronchial removal with the patient under general anesthesia. Given our experience with this case, we believe that such conservative management should allow for excellent results in most instances and avoid the need for reoperation.

As a further step towards the modernization of acupuncture, the objective of this review was to figure out the frequency and severity of adverse complications and events in acupuncture treatment reported from 1980 to 2013 in China. All first-hand case reports of acupuncture-related complications and adverse events that could be identified in the scientific literature were reviewed and classified according to the type of complication and adverse event, circumstance of the event, and long-term patient outcome. The selected case reports were published between 1980 and 2013 in 3 databases. Relevant papers were collected and analyzed by 2 reviewers. Over the 33 years, 182 incidents were identified in 133 relevant papers. Internal organ, tissue, or nerve injury is the main complications of acupuncture especially for pneumothorax and central nervous system injury. Adverse effects also included syncope, infections, hemorrhage, allergy, burn, aphonia, hysteria, cough, thirst, fever, somnolence, and broken needles. Qualifying training of acupuncturists should be systemized and the clinical acupuncture operations should be standardized in order to effectively prevent the occurrence of acupuncture accidents, enhance the influence of acupuncture, and further popularize acupuncture to the rest of the world.

Background: The choice of suture material for repair of episiotomy or perineal laceration is largely of one’s personal preference. Chromic catgut was widely used in most institutions. It now appears that chromic catgut is associated with more postpartum discomfort and hence chromic catgut has been largely replaced by synthetic absorbable materials like polyglactin and polyglycolic acid.Methods: The study was conducted in Institute of Social Obstetrics and Government Kasturba Gandhi Hospital, Chennai. This is a prospective, comparative study involving two groups. The use of a rapidly absorbing form of synthetic absorbable suture material, in the repair of episiotomy or perineal laceration in 100 patients during the study period February 2012 to July 2012, were simultaneously compared with the traditional natural absorbable suture material.Results: With the use of rapidly absorbing polyglactin 910, there was a significant reduction (p=0.000) in the short-term pain, 19 compared to 80 in the control group. With regard to wound dehiscence and the need for resuturing, there was statistically significant difference in the control group (15%) compared to the study group (0%). There was no statistical significance between the two groups in terms of dyspareunia (12.4% vs 10.7%).Conclusions: Fast-absorbing form of Polyglactin seems to be effective in reducing some of the morbidity associated with perineal repair following childbirth. There was significant reduction in the short-term pain and the need for analgesia. The incidence of wound dehiscence was markedly reduced.

The aim is to establish the features of morphological and morphometric changes in the skin of guinea pigs in erythemal, early post-erythemal and late post-erythemic periods after local ultraviolet irradiation. Materials and methods: Studies were conducted on 54 albino guinea pigs weighing 400-500 g. Ultraviolet erythema was caused by irradiation in 1 minimum erythemal dose. The control group included intact guinea pigs. After 2, 4 hours, on the 3rd, 8th, 15th, 21st, 28th day, the fragments of the irradiated skin were investigated using histochemical and morphometric methods. Results: After 2, 4 hours after irradiation, dyscirculatory changes in the skin develop. By the 3rd day of the experiment a morphological picture of acute inflammation in the epidermis and dermis develops, apoptotic keratinocytes appear (sunburn cells), which is accompanied by thickening of the epidermis and an increase in the density of fibroblasts. By the 8th day proliferative-hyperplastic and degenerative changes begin to prevail, including dystrophic nature, the thickness of the epidermis and the density of fibroblasts reach a maximum. In the long term, on the 15-28th day, dystrophic changes of the epidermis, dyskeratosis, changes in the number and structure of elastic fibers with an increase in uneven fibrosis, collagenization processes and the development of sclerotic changes, as well as a significant thickening of the epidermis, an increase in the density of fibroblasts are observed. Conclusions: The data obtained indicate pronounced morphofunctional changes in the skin in the zone of local ultraviolet irradiation observed throughout the entire observation period.

The study aims to analyze the region's financial state in aceh within 2015-2017. The financial condition is measured by using indexing measures developed by ritonga (2014), which are made up of six dimensions of short term solvability, long-term solvability, budget solvability, financial flexibility, financial independence, service solvability. In 2015, analysis was developed by augmenting the operational solvability dimension. This study is using qualitative descriptive methods and analysed a secondary data obtained through documentary techniques. The result of this study shows that within 3 years the financial district/city in aceh that got the best category of index value is Sabang (0.488), Banda Aceh (0.452), Aceh Tengah (0.444), Aceh Besar (0.389), and Gayo Lues (0.382). While the area government with the value of the financial conditions of the lowest financial level or ranked category is Aceh Tenggara (0,177), Aceh Singkil (0,148) Lhokseumawe (0,106).

The purpose — to compare the effectiveness of cerclage to reduce the frequency of preterm birth when using non-absorbable suture material in the form of a braided silk thread (4 mm) and a braided tape (5 mm), as well as to identify the frequency of complications when using them. Material and methods. A retrospective analysis of cases and childbirth was conducted, as well as the analysis of own experience of applying cerclage with various suture materials in patients with isthmic-cervical insufficiency. A total of 157 patients were examined. Pregnancy outcomes, neonatal outcomes, and post-suture complications were evaluated. Results. 157 women were given cerclage at gestation periods of 14 to 23 weeks. There was no significant difference in the choice of suture material relative to the duration of pregnancy. Cerclage with thread was performed in 92 patients (58,6%) — group 1 and 65 patients were sutured with tape (41,4%) — group 2. Preterm birth occurred in 28 patients of the 2nd comparison group (18%) and 45 patients of the 1st group (29%). The duration of pregnancy in preterm birth was comparable in both groups. Conclusions. The study found that preterm birth after cerclage using a braided tape occurred less frequently than with the use of a braided silk thread. There were no significant differences in neonatal outcomes in the comparison groups. Complications after cerclage, such as suture and cervical rupture, were found only in the group of women who underwent surgery using a woven silk thread.
