Regional population health index as a major factor in planning the health care system

The health care system of the country and the development of Health must take into account the distribution and planning costs for medical services. The selection of these indicators takes into account the health index as part of the integral index of quality and standard of living, as measured by life expectancy at birth , and at the regional level. At this level of measurement of health, regions should not be limited to one exponent and objectively consider more of them.

This issue of the Australian Journal of Primary Health features a selection of papers about general practice in Australia. These papers reflect a range of perspectives important in general practice, including disease management in the general practice context, innovative approaches to prevention, consumer perspectives on conditions and services, future development of general practice in the primary health care system, and more. Publication of these papers occurs at a time when development of the National Primary Health Care Strategy is well under way. The focus of the National Primary Health Care Strategy is on related themes of prevention, chronic disease, evidence-based practice and better integration of general practice into the primary health care system.

According to the definition, objectives, and functions of the health care system, the performance of the system has been evaluated from the three aspects: effectiveness, equity, and efficiency. By replacement analysis and analytic hierarchy process, the model has been constructed. Considering the raw data and omitting life expectancy at birth and obesity rate, we find that some indicators are affected by the policy factor. Health care system in the United States has been compared with other countries which have different level by using the iteration model. The value of the evaluation function in the four countries is different, and the results show that the health care system in United States is the best among the four countries.

В статье рассмотрены инструменты государственного регулирования сферы здравоохранения на уровне региона, а также направления его совершенствования, что является в настоящее время весьма актуальным. Авторы, раскрывая важное значение программирования и проектирования государственного регулирования системы здравоохранения, отметили отличия и преимущества проекта перед государственной программой, что объясняет активное использование проектного метода на всех уровнях управления. Среди направлений совершенствования инструментов государственного регулирования сферы здравоохранения в республике предложены такие направления, как активное использование инструментов государственно-частного партнерства, широкая информатизация и цифровизация системы здравоохранения и др. На основе анализа развития системы здравоохранения Республики Башкортостан, показателей обеспеченности населения муниципальных районов и городских округов врачами и средним медицинским персоналом сделаны выводы о дефиците медицинских кадров в регионе и разработаны соответствующие мероприятия. В статье говорится о ключевой проблеме в целом по России, а также в республике, - о неблагополучной демографической ситуации. Представлен анализ демографических показателей за последние годы и предложены эффективные инструменты решения данных проблем.По результатам проведенного исследования авторами определены наиболее эффективные и значимые на региональном уровне мероприятия, позволяющие достичь целей национального проекта «Здравоохранение» и государственной программы «Развитие здравоохранения Республики Башкортостан». The article discusses the instruments of state regulation of the healthcare sector at the regional level, as well as the directions of its improvement, which is currently very relevant.The authors, revealing the importance of programming and designing state regulation of the health care system, noted the differences and advantages of the project over the state program, which explains the active use of the project method at all levels of management.Among the directions for improving the instruments of state regulation of the healthcare sector in the republic, such directions as the active use of public-private partnership instruments, widespread informatization and digitalization of the healthcare system, etc. are proposed.Based on the analysis of the development of the health care system of the Republic of Bashkortostan, indicators of the provision of the population of municipal districts and urban districts with doctors and nurses, conclusions were drawn about the shortage of medical personnel in the region and appropriate measures were developed.The article speaks about the key problem in Russia as a whole, as well as in the republic - the unfavorable demographic situation. The analysis of demographic indicators in recent years is presented and effective tools for solving these problems are proposed.Based on the results of the study, the authors have identified the most effective and significant measures at the regional level, allowing to achieve the goals of the national project “Health” and the state program “Development of health care in the Republic of Bashkortostan.”

Despite the evidence of links between health expenditure and health care efficiency, it is still unclear why countries with similar levels of health expenditures experience different outputs in terms of life expectancy at birth. Health care system efficiency might shed some light on the question. Using output-oriented data envelopment analysis, we compared the health systems of 140 countries in terms of attained life expectancy. Efficiency is determined by the distance from the closest country on the best practice frontier, which identifies the highest attainable life expectancy observed for any given level of health care spending. By using national data form the Human Development Data, we built the efficiency frontier and computed the potential life expectancy increase for each country. The potential improvement was, on average, 5.47 years [95%CI: 4.71–6.27 years]. The least efficient countries (10th percentile of the efficiency score) could improve by 11.78 years, while the most efficient countries (90th percentile of the efficiency score) could only improve by 0.83 years. We then analyzed, with regression analysis stratified by average education level, and by the role of health-related variables in differentiating efficient and inefficient countries from each other. The results suggest that, among countries with lower levels of education, decreasing unemployment and income inequality increases average life expectancy, without increasing health expenditure levels.

The article presents an optimized model of medical care for the population with diseases of the blood and blood-forming organs at the regional level in modern conditions of health care reform based on the current legislation of Ukraine on health care reform and scientific literature on the organization of medical care for patients with diseases of the blood and blood-forming organs. The scientific substantiation of the optimized model of hematological medical care at the regional level in the conditions of reforming the health care system of the country was carried out within the existing medical-demographic and socio-economic situation, taking into account the positive changes in the future. The model envisages the structure and organization of medical care for patients at all stages and levels of medical care. The structural basis of the model of providing medical care to patients with diseases of the blood and blood-forming organs is the available resources of the current health care system. Its implementation requires additional financial resources for logistics in accordance with industry standards, which should be provided in the provision of medical care and without the implementation of the proposed model. The model provides a concentration of resources, which determines its economic efficiency. The implementation of the proposed optimized model of medical care for the population with diseases of the blood and blood-forming organs at the regional level will provide the population suffering from these diseases to provide affordable medical care in stages and levels of its provision depending on health status.

There are a multitude of reasons that a teletinnitus program can be beneficial, not only to the patients, but also within the hospital and audiology department. The ability to use technology for the purpose of tinnitus management allows for improved appointment access for all patients, especially those who live at a distance, has been shown to be more cost effective when the patients travel is otherwise monetarily compensated, and allows for multiple patient's to be seen in the same time slots, allowing for greater access to the clinic for the patients wishing to be seen in-house. There is also the patient's excitement in being part of a new technology-based program. The Gulf Coast Veterans Health Care System (GCVHCS) saw the potential benefits of incorporating a teletinnitus program and began implementation in 2013. There were a few hurdles to work through during the beginning organizational process and the initial execution of the program. Since the establishment of the Teletinnitus program, the GCVHCS has seen an enhancement in patient care, reduction in travel compensation, improvement in clinic utilization, clinic availability, the genuine excitement of the use of a new healthcare media amongst staff and patients, and overall patient satisfaction.
