Βackground: angiogenesis is seen during the multiple stages of carcinogenesis, aswell as during the process of surgical wound healing, a fact which has led tosubstantial debate over the last decades about the potential impact of surgery upon thefinal outcome of ceratin patients treated for breast cancer.Aim: the present research aims at investigating the potential effect of surgery on theprocess of angiogenesis, by studying a number of factors that are related to the latter,in patients suffering from breast cancer before and after the time of the procedure,whilst comparing these results with those of patients that were operated on their breastfor non-malignant disease.Material-Methods: blood from 10 female patients with breast adenocarcinoma(Study Group) was collected via venipuncture before surgery (labeled as PRO), aswell as on post-operative day 3 (labeled as D3) and day 7 (labeled as D7). Moreover,blood samples were also taken from 6 female patients with fibroadenoma (ControlGroup) before surgery (PRO) and on day 3 afetr surgery (D3). These samples weremeasured for detection of circulating levels of three established angiogenesisbiomarkers using ELISA (Enzyme-Linked ImmunoSorbent Assay): VascularEndothelial Growth Factor-A (VEFG-A), Interleukin-8 (IL-8) and basic FibroblastGrowth factor (bFGF or FGF-2). In addition, circulating transcripts of 84 agiogenesirelatedgenes were determined using RT-PCR (Real Time Polymerase ChainReaction). The two groups of patients were firstly compared to each other regardingtheir results. Also, patients belonging to the Study Group were analized at differenttime points regarding surgery. Finally, the results were investigated againstclinicopathological data and patient outcome.Results: using ELISA we were able to detect increased levels of circulating VEGF-Aand IL-8 in the Study Group patients compared to the Control Group patientspreoperatively (p=0,0381 and p=0,0218 respectively), while for bFGF there was nostatistically significant difference documented. Surgery resulted in a significantincrease in VEGF-A levels on D3 (p=0,0389) and D7 (p=0,0172) as compared toPRO levels. Perioperative kinetics of IL-8 showed a mild trend towards increase,which, however, was not statistically significant. Postoperative levels of bFGF wereslightly increased on D3, but on D7 they were even lower than preoperative values(p=0,0205). Using RT-PCR certain differences between the Study Group and theControl Group were recorded regarding the circulating transcripts of a great numberof angiogenesis-related genes preoperatively: upregulation of VEGF-C, EGF, IL-8,FGF-1, SPHK1, NRP1, LAMA5, COL4A3, TEK, EFNA3, EFNB2. AKT1, ITGB3,THBS1, CCL11, TIMP3 and downregulation of CXCL10. Moreover, mastectomyinduced an altered expression in several key-genes in breast cancer patients:upregulation of THBS1, COL4A3, BAI1, ITGB3 and downregulation of EREG,SERPIFN1, CXCL9, CXCL10, IL1B, CCL2, CXCL1, HIF1A, NOTCH4. Conclusions: patients suffering from breast cancer have a different angiogenic profilein comparison to patients with fibroadenoma, as documented through their differencesin circulating levels of angiogenic factors. These levels are greatly changed after thesurgical procedure. VEGF showed a transient increase, while bFGF initially increasedbut only to finally decrease to levels that were even lower than the preoperative ones.Moreover, mastectomy promoted a shift in the expression pattern of a broad panel ofangiogenesis-related gene transcripts.

Abstract Background Breast cancer is the most common cancer among women and one of the most important causes of death among them.Angiogenesis is an important step for primary tumor growth, invasiveness, and metastases. Angiopoietins are well-recognized endothelial growth factors that are involved in angiogenesis associated with tumors. Aim To explore the diagnostic significance of serum angiopoietin-2 (Ang-2) in breast cancer and to evaluate its prognostic efficacy through studying the degree of its association with the TNM staging of the disease. Patients and Methods This study was conducted on (35) Egyptian female patients who were diagnosed as breast cancer according to histopathological examination of breast biopsy (Group 1, Breast Cancer Patients) and (25) female patients with benign breast diseases (Group II, Pathological Control Patients), in addition to (20) age - matched apparently healthy, free mammogram, females serving as healthy controls (Group III, Healthy Controls). For all participants, measurement of serum Ang-2 was done using enzyme linked immunosorbent assay (ELISA) technique. Results A highly significant increased levels of Ang-2 was observed in breast cancer patients when compared to healthy control group (Z = 4.95, p < 0.01). However, no significant difference was observed in Ang-2 levels between breast cancer patients group and pathological control group (Z = 3.37, p > 0.05). No significant difference was detected in Ang-2 levels in relation to TNM stage and histological grade. No significant correlation was found between Ang-2 levels and serum levels of CA15-3, hormone receptors, HER2/new receptor status (p > 0.05, respectively). Conclusion This study revealed that Ang-2 serum levels were significantly increased in patient with breast cancer compared with healthy controls, indicating that high Ang-2 level is a promising non invasive biomarker for breast cancer diagnosis. However, no significant difference of Ang-2 levels was detected in relation of breast TNM staging in the population studied.

Objective: To investigate the effect of advanced nursing on the quality of life and the prevention of complications in patients with breast cancer undergoing PICC (Peripherally Inserted Central Catheter) chemotherapy. Methods: From January 2017 to January 2020, 98 patients with breast cancer undergoing PICC chemotherapy in our hospital were divided into control group and study group by random sampling. The patients were given routine nursing and advanced nursing intervention on the basis of routine nursing. The psychological status, complications and quality of life of the patients were compared. Results: There was no significant difference in HAMA, HAMD score and QLQ-30 score before nursing (P > 0.05). The scores of HAMA and HAMD in the study group after nursing were lower than those in the control group (P < 0.05), the scores of QLQ-30 were higher than those in the control group (P < 0.05), and the complication rate was lower than that in the control group (P < 0.05). Conclusion: The intervention of advanced nursing mode in patients with breast cancer treated with PICC chemotherapy can effectively alleviate the influence of negative emotion on chemotherapy, reduce the occurrence of complications and improve the quality of life. This mode has high clinical value.

Background: Breast cancer has emerged as a rapidly growing cancer in India and has already replaced cervical cancer. According to global cancer statistics for the year 2008, breast cancer resulted in 46 million deaths. In Asia, incidence rate of breast cancer is lower but is drastically rising. Nevertheless, while dyslipidemia [high LDL-C (low density Lipo-protein-cholesterol) and low HDL-C (high density lipo-protein-cholesterol) levels] has controversial role in this disease; the specific influence of dyslipidemia in breast cancer initiation and progression is not completely understood. So, we undertook this study.Methods: The study was conducted in the department of general surgery, at Medical College and allied Hospital, in Central rural India between year 2012 and 2014. It was a case control study with a sample size of 50 female participants attending the surgery in patient department, fitting in the inclusion criteria as cases and 50 matching controls. The Data was entered in master chart was statistically analyzed by using SYSTAT version 11 and tests were applied accordingly.Results: Age is a crucial factor in malignancy, most of the studies show malignancy presents more in elder population when compared to young ones. In most patients were between 31-50 years. The youngest patient was of 36 years and the oldest was 78 years. The mean age of the patients affected was 48.54 years which is close to the values in other studies. This study also demonstrated significantly high (p value<0.01) serum TC (total cholesterol) levels of post-menopausal study group (32%, more) as compared to post-menopausal control group. The increase in Serum TC levels was more in post-menopausal study group as compared with pre-menopausal study group. Present study had not shown a significant difference (p value >0.05) between serum levels of HDL-C levels in pre-and post-menopausal control group and study group, though post-menopausal cases had slightly lower values of HDL-C when compared with pre-menopausal controls.Conclusions: We conclude from this study that there is a definite positive correlation between carcinoma breast and fasting lipid profile. The pre-and post-menopausal cases showed significant correlation as far as Total Cholesterol and LDL. Whereas there was no significant correlation between HDL, VLDL and TG (triglycerides). Body mass index has no relation with carcinoma breast.

Background Aromatase inhibitors (AI) have positive effects on disease-free life in patients with Breast Cancer (BC); on the other hand, their side effects especially arthralgia can be observed in many of patients. This study aimed to evaluate the effectiveness of Progressive Relaxation Exercises (PRE) on the common side effects of AI in patients with BC. Methods A total of 44 patients receiving AI were allocated to the study and control groups in this randomized controlled, single-blind trial. The study group (n=22) performed a combined (one-day supervised, and 3 days home-based) PRE program 4 days/week, for six weeks. The control group (n=22) was given advice about relaxation in daily life. Data was collected initially and after 6 weeks of the intervention. Pain, quality of life (QoL) and emotional status (ES) were assessed using the Brief Pain Inventory (BPI), Functional Assessment of Chronic Illness Therapy (FACT) and Hospital Anxiety and Depression (HAD) scales, respectively. Results Pain was significantly reduced within the study group in Pain Severity (p=0.001) and Pain Interference (p=0.01) sub-scores. Pain was also reduced between the groups as compared using the Pain Severity (p=0.00) and Patient Pain Experience (p=0.003) sub-scores; QoL and ES remained with no significant difference either within the groups or between the groups (p>0.05). Conclusion The results of this study showed that PRE significantly decreased pain scores in BC patients receiving AI. Although the reduction in pain is a valuable data even in the 6-week period in those cases, the long-term effects of relaxation techniques need to be followed.


Aim: The aim of the study was to evaluate the levels of osteoprotegerin (OPG), receptor activator of nuclear factor –κB ligand (s-RANKL), OPG/sRANKL ratio and sRANKL/OPG ratio in adolescent patients with type 1 diabetes mellitus (T1DM), and to assess their correlation with the following factors: patients’ gender, age, metabolic control, age of the diagnosis and duration of the disease. Patients and Methods: 60 T1DM patients (32 girls and 28 boys, mean±SD age: 15.0±1.9 years, diabetes duration: 5.1±3.9 years, age of the diagnosis: 9.9±3.9 years, HbA1c: 7.9±1.4%) and 18 healthy matched controls were included. Osteoprotegerin and total sRANKL (free and bound sRANKL) were measured by ELISA (enzyme-linked immunosorbent assay). Results: There were no statistically significant differences in serum OPG levels between patients and controls (51.56±12.05 vs. 50.98±13.55pmol/L, p=0.84. No correlation betweengender and OPG levels both in the study and control group has been reported, although OPG levels were significantly lower in diabetic boys (2.59±0.67 pmol/L) than in control boys (3.30±1.01 pmol/l) (p = 0.031). Tetryl analysis (qualifications) dependent on OPG levels has demonstrated statistically significant correlation between the study group and clinical factors such as: gender, age of the diagnosis and duration of the disease, but not with the age. Only the tendency toward correlation between OPG and metabolic control of diabetes (p=0.093) has been observed. No statistically significant differences in sRANKL levels between the study group and controls has been identified. In patients with T1DM no correlation between RANKL levels and clinical factors such as gender, duration of the disease, age of the diagnosis and metabolic control has been reported. Only negative correlation between RANKL level and patients’ age (p = 0.002) has been observed. No correlationbetween OPG and RANKL levels has been demonstrated. No statistically significant differences in OPG/RANKL ratios between the study group and controls has been reported, the only significant difference in these ratios was observed between control females and males (p = 0.019), but not in the study group. A positive correlation between OPG/RANKL ratio and OPG level with the age has been demonstrated in the study group.

In recent years, breast cancer (BC) is the most common cancer pathology and the most common cause of disability among women in developed countries. Finding the most effective ways of interaction between the patient and the doctor creates the preconditions for the necessary analysis of the treatment process from an objective and subjective point of view. Therefore, an important indicator to be taken into account is the quality of life of a patient. To compare the indicators of a comprehensive assessment of the quality of life of patients to the adverse locally advanced forms (LA) of breast cancer before and after systemic intravenous polychemotherapy (SPCTx) and selective endolymphatic polychemotherapy (ELPCTx) in neoadjuvant mode. The study was conducted on the basis of a random analysis of outpatient cards from 112 patients with LA BC T4A-DN0-3M0 who received a comprehensive antitumor treatment on the basis of the Donetsk regional antitumor center and the University Clinic of the Odessa National Medical University from 2000 to 2017, which was proposed a questionnaire at various stages of preoperative treatment. The first (control) group consisted of 65 patients (58 %) with inoperable forms of LA BC, which was performed in neoadjuvant mode by SPCTx. The second (study group) included 47 patients (42 %) with inoperable forms of LA BC, which was performed as a neoadjuvant course ELPCTx. According to the integral indicators of quality of life and quality of health between patients in the control and study groups, there was no statistically significant difference. In a detailed analysis of the indicators of symptomatic scales, the difference between the groups did not exceed the critical. Based on the results of a study conducted among patients receiving endolymphatic chemotherapy in a neoadjuvant mode, the subjective evaluations of treatment in absolute numbers have better reference values without statistical superiority. The study of the integrative indicator of quality of life and its discrete elements is an ergonomic and economical means of heuristic assessment of the health of patients in order to further develop more rational and convenient ways of solving urgent issues of modern oncology by increasing compliance and finding a compromise between the physician and the patient.

Objective: This study aims to investigate the expression and clinical significance of Th17 cells and related factors in peripheral blood of patients with Autoimmune Hepatitis (AIH). Methods: A retrospective selection of 100 patients with AIH were included as a study group, and 100 healthy volunteers in the outpatient clinic were selected as the control group. The levels of IL- 17, IL-6, IL-21 and TNF-α in peripheral blood of all subjects were detected by enzyme-linked immunosorbent assay and the frequency of Th17 cells and Treg cells was detected by flow cytometry. Results: Results showed that the study group had higher levels of serum total bilirubin (TBil), alkaline phosphatase (ALP), γ -glutamyltranspeptidase (γ-GT), immunoglobulin G (IgG), immunoglobulin M (IgM), alanine aminotransferase (ALT) and aspartate aminotransferase (AST) than the control group, as well as higher levels of IL-17, IL-6, IL-21 and TNF-α in serum. The frequency of Th17 cells in peripheral blood was higher in the study group, while the frequency of Treg cells was lower. Also, serum IL-17, TNF-α levels and Th17 cells frequency were positively correlated with ALT and AST, whereas Treg cells frequency were negatively correlated with ALT and AST levels. Conclusion: Our finding demonstrates that Th17 cell frequency and their related factors IL-17 and TNF-α, are associated with liver damage, which might be used to monitor AIH disease severity.

Abstract Introduction Good hearing in pilots, including central auditory skills, is critical for flight safety and the prevention of aircraft accidents. Pure tone audiometry alone may not be enough to assess hearing in the members of this population who, in addition to high noise levels, routinely face speech recognition tasks in non-ideal conditions. Objective To characterize the frequency-following response (FFR) of a group of military pilots compared with a control group. Methods Twenty military pilots in the Study Group and 20 non-pilot military personnel, not exposed to noise in their work, in the Control Group, all with normal hearing, aged between 30 and 40 years old, completed a questionnaire to assess their hearing habits, and their FFRs were measured with a /da/ syllable (duration 40 milliseconds, speed 10.9/s), at 80 dB NA in the right ear. All procedures were approved by the ethical committee of the institution. Statistical analysis was performed using the t-Student or Mann-Whitney tests for quantitative variables, and the Fisher or chi-squared tests for qualitative variables, and a value of p < 0.05 was considered to be statistically significant. Results There was no significant difference between the groups regarding auditory habits. In the FFR, wave amplitudes A (p = 0.01) and C (p = 0.04) were significantly lower in the Study Group. Conclusion Working as a military pilot can be a crucial factor in determining an individual's typical FFR pattern, demonstrated in the present study by statistically significant reductions in the amplitudes of the A and C waves.

Background: Stromelysins are potential breast cancer biomarkers. The aim of the study was to evaluate if plasma levels of selected metalloproteinases (MMPs) (stromelysin-1 (MMP-3) and stromelysin-10 (MMP-10)) and cancer antigen 15-3 (CA 15-3) used separately and in combination demonstrated diagnostic usefulness in breast cancer (BC). Methods: The study group consisted of 120 patients with BC, while the control group included 40 patients with benign breast cancer and 40 healthy individuals. Concentrations of MMP-3 and MMP-10 were determined by enzyme-linked immunosorbent assay; CA 15-3 was determined by chemiluminescent microparticle immunoassay. Results: In the group of patients with BC, the area under the curve (AUC) was significantly higher for all markers (except MMP-3) and all sets of markers. At the earliest disease stage, only MMP-10 had a significantly higher AUC (AUC = 0.8692, p < 0.001). Moreover, MMP-10 had the highest AUC (0.9166) among parameters tested separately. The highest AUC was observed for the combination of MMP-10 + CA 15-3 and MMP-3 + MMP-10 + CA 15-3 in line with disease progression (stage I 0.8884 and 0.8906, stage II 0.9244 and 0.9308, stages III + IV 0.9919 and 0.9944, respectively, p < 0.001 in all cases). Conclusions: The results suggest that MMP-10 could be a potential marker in early stages of BC. Moreover, plasma concentration of MMP-10 and MMP-3 in combination with CA 15-3 may improve diagnosis of this type of cancer.
