This book offers a critique of the dominant understanding and deployment of empathy in the mainstream medical humanities. Drawing on feminist theory, it positions empathy not as something that one has or lacks, and needs to accrue, but as something that one does and that is embedded within structural, institutional and cultural relations of power. It aims to provide a critically informed definition of empathy, drawing on phenomenology, in order to counter the vagueness of the term as it has often been used. It questions, too, the assumption that empathy is limited to the clinical relation, looking to a broader and more encompassing definition of the ‘medical’. Combining theoretical argument with literary case studies of Mark Haddon’s The Curious Incident of the Dog in the Night-Time, Pat Barker’s Life Class, Ian McEwan’s Saturday, Aminatta Forna’s The Memory of Love and Kazuo Ishiguro’s Never Let Me Go, this book contends that contemporary fiction is not a vehicle for accessing another’s illness experience, but itself engages critically with the question of empathy and its limits. The volume marks a key contribution to the rapidly evolving field of the critical medical humanities.

If illness behaviour is to be fully understood, the social and behavioural sciences must work together to understand the wider forms in which illness is experienced and communicated with individuals and society. The current paper synthesised literature across social and behavioural sciences exploring illness experience and communication through physical and mental images. It argues that images may have the capacity to embody and influence beliefs, emotions, and health outcomes. While four commonalities exist, facilitating understandings of illness behaviour across the fields (i.e., understanding the importance of the patient perspective; perception of the cause, sense of identity with the illness, consequences, and level of control; health beliefs influencing illness experience, behaviours, and outcomes; and understanding illness beliefs and experiences through an almost exclusive focus on the written or spoken word), we will focus on exploring the fourth commonality. The choice to focus on the role of images on illness behaviour is due to the proliferation of interventions using image-based approaches. While these novel approaches show merit, there is a scarcity of theoretical underpinnings and explorations into the ways in which these are developed and into how people perceive and understand their own illnesses using image representations. The current paper identified that the use of images can elucidate patient and practitioner understandings of illness, facilitate communication, and potentially influence illness behaviours. It further identified commonalities across the social and behavioural sciences to facilitate theory informed understandings of illness behaviour which could be applied to visual intervention development to improve health outcomes.

Ken Wilber's integral metatheory is an interpretive framework that can that be applied to the clinical practice of medicine and medical and nursing education. It offers a comprehensive view of the patient illness experience superior to current models of patient care and may provide a valuable guide for nursing and medical practice and teaching. This article seeks to explain some of the basic concepts of integral metatheory and show their potential application to practice and teaching using the current COVID-19 pandemic as an illustrative model.

AbstractWe aimed to characterize the variability in the illness experience and recovery process from COVID-19. We conducted in-depth individual interviews with participants enrolled in the Long-term Immunological Impact of Novel Coronavirus (LIINC) cohort study in San Francisco, California from June through October of 2020. Participants were adults who had a previously confirmed positive SARV-CoV-2 nucleic acid amplification test result, had recovered or were recovering from acute infection, and underwent serial evaluations at our clinical research center. We purposefully sampled 24 English- and Spanish-speaking adults with asymptomatic, mild and severe symptomatic infection, including those who were hospitalized, and those with HIV co-infection. Half of our sample (50.0%) identified as Latinx/Hispanic and most of the participants were men (62.5%). We used thematic analysis to characterize the illness experience, recovery process, and mental health impact of experiencing COVID-19 and present clinical data for each participant. Emergent themes were: (1) across symptom profiles and severity, experiencing COVID-19 was associated with psychological distress, (2) among participants with symptomatic infection, the illness experience was characterized by uncertainty in terms of managing symptoms and recovery, and (3) despite wide-ranging illness experiences, participants shared many common characteristics, including health information-seeking behavior facilitated by access to medical care, and uncertainty regarding the course of their illness and recovery. COVID-19 was associated with elevated levels of psychological distress, regardless of symptoms.
