Background: Pancreatic carcinoma is a disease with high modality and has a high incidence of recurrence after surgical resection. The prognosis depends on early diagnosis and treatment. Numerous international studies have reported use of CA 19-9 in diagnosis of pancreatic cancer. We planned this study to validate role of CA 19-9 in our local population. The objective of this study is to correlate raised serum CA 19-9 levels in patients with pancreatic tumours to distinguish between benign and malignant pancreatic disease. Subjects and methods: The present study is a validation study. Thirty-five patients with diagnosis of pancreatic tumor on radiological imaging were included after their informed consent. Data collection forms were filled, blood samples were taken and serum CA 19-9 was estimated by ELISA in Biochemistry department, SIMS. Histopathology samples were collected at time of surgical resection, sent to pathology departments of respective hospitals and histopathological reports collected. All data was collected and analysed by using descriptive method. The study was conducted in Biochemistry department of PGMI and SIMS, Lahore from May 2015 till June 2016. Results: Out of 35 patients, nineteen (54.3%) were female and sixteen (45.7%) were male. Thirty [85.7%] patients were between third to seventh decades of their life. The mean age range around 47.28. Thirty-four patients had malignant tumor and 1 benign on histopathology. CA 19-9 was raised (>37 U/ml) in 33 out of 34 patients with malignant pathology. The patient with benign pathology had CA 19-9 level <37U/ml (the cut off value). Head of pancreas was the commonest site in 32 patients (65%) for tumour occurrence. Total 28 (82%) patients had adenocarcinoma as the histological type of pancreatic tumour. Cholangiocarcinoma in Periampullary region of pancreas was second in frequency, 4 patients (12%). CA 19-9 shows sensitivity of 97% and specificity of 100% to diagnose carcinoma of pancreas in patients with pancreatic tumour. CA 19-9 has 100% positive predictive value to diagnose benign tumour and 50% negative predictive value to diagnose malignant tumours. Conclusion: Raised levels of CA 19-9 has an important role in diagnosis of patients with pancreatic tumour to differentiate between benign and malignant pathology.

the lesions of the urinary organs of a iatrogenic origin correlated to hysterectomy surgery almost exclusively concern the ureter and the bladder. Lesions of the urinary organs caused during abdominal and vaginal hysterectomy for benign pathology were studied at the 1st Clinic of Obstetrics and Gynaecology of the University of Catania between 1st January 1989 and 31st December 2000. During this period 3,138 hysterectomies were performed, of which 2,765 (88.11%) abdominally and 373 (11.89%) vaginally. Altogether there were 11 (0.35%) iatrogenic lesions of the urinary excretory organs and these included 5 (0.15%) ureteral lesions and 6 (0.19%) bladder lesions. There were no lesions of the urethra. In 2,765 abdominal hysterectomies 4 (0.14%) ureteral lesions and 4 (0.14%) bladder lesions occurred, and in 373 vaginal hysterectomies one (0.27%) ureteral lesion and 2 (0.54%) bladder lesions occurred. In all cases, after adequate treatment the urinary organs healed completely.

Duodenal stenting has been widely used on malignant pathology on selected patients with poor prognosis and advanced disease. In these last years, there has been a clear ampliation of the clinical applications of endoscopy procedures and stents. Its use on benign pathology is spreading but there is a lack of literature about the complications in this context. The incidence of stent migration is about 10-25% in self-expandable metal stent (SEMS), and 2-5% on covered self-expanding metal stents (CSEMS). We reported a clinical case of a 48 years old patient who developed a duodenal ulcer. The patient was submitted to exploratory laparotomy, with duodenal primary closure of the ulcer. Later, the patient developed a enterocutaneous fistula because of the duodenal leak. It was referred to our third level hospital to the hepatopancreatobiliary surgery service. A new exploratory laparotomy with duodenal exclusion was planned, but it was impossible to access due to frozen abdomen. CSEMS was placed in the duodenal bulb resulting in the resolution of leaking, but the stent could not be removed because of migration. The stent trajectory was followed by abdominal x ray and tomography. The patient developed multiple intestinal an fecal enterocutaneous fistulas. It was submitted to multiples endoscopies, colonoscopies and enteroscopy without any success to reaching it. It was decided to perform a right lumbotomy to extract the prothesis. The stent was surgically removed, a planned stoma was left on the right flank on the extraction site.

Uterine leiomyoma is one of the most common benign pathology in women and lipoleiomyoma is an extremely rare and specific type of leiomyoma. Here we are reporting and incidental of lipoleiomyoma in a 43 year old perimenopausal women presented with lower abdominal pain since 2-3 days and discharge per vaginum on and off.
