Serious mental illness (SMI) results in functional disability that imposes a significant burden on individuals, caregivers, and society. Development of novel treatments is under way in an effort to improve the illness domains of cognitive impairment and negative symptoms and subsequently to improve functional outcomes. The assessment of functional outcomes in SMI faces a number of challenges, including the proliferation of assessment instruments and the differential prioritization of functional goals among stakeholder groups. Functional assessments relying on self- and informant report present a number of limitations. Identifying alternative strategies to assess functioning that are reliable, valid, and sensitive to change is necessary for use in clinical trials. Measures of functional capacity have been proposed for clinical trials investigating compounds to treat cognitive impairment in schizophrenia. Alternative approaches employing effort-based decision making or daily activity recording using instruments such as the Daily Activity Report may be more appropriate for studies focused on improving negative symptoms.

SummaryThe present study, conducted in collaboration between the Departments of Psychiatry in Swiss Universities and the World Health Organization, had two main goals: to develop assessment methods which could subsequently be used in the Swiss centres in a standard manner; and to make arrangements for continuing collaboration between the centres in Switzerland and the acquisition of new knowledge about the distinctions between depression and cognitive impairment. For this aim, three different groups of elderly patients of either sex were selected during the period of November 1989 to July 1991 for inclusion in the study. The first two groups included the first ten patients of either sex over 60 years of age consecutively contacting the participating institutions and showing depression with or without clinically significant symptoms of cognitive impairment; the control group included patients showing no depression or clinically significant symptoms of cognitive impairment. A total of 125 patients were included in the initial evaluation, 69 of which were reassessed at a seven-month follow up (on average). Each patient was administered a number of clinician-rated or self-report instruments for the assessment of depression, cognitive impairment, disabilities, physical status and onset of disorders. The study has shown that a variety of instruments can be used for the reliable assessment of depression or cognitive impairment in the elderly; but the instruments for the assessment of depression differentiate only poorly between patients with or without cognitive impairment. Because of the importance of identifying both depressed and cognitively impaired patients among the elderly, different assessment instruments targeted at the different symptom clusters need to be administered simultaneously.

Abstract: Schizophrenia is a major mental illness characterized by psychosis, apathy and social withdrawal, and cognitive impairment, which often results in impaired functioning in the areas of work, school, parenting, self-care, independent living, interpersonal relationships, and leisure time. This chapter focuses on the assessment of schizophrenia in adults. It begins with a review of the nature of the disorder, which is followed by a review of clinical assessment instruments designed for the assessment purposes of (a) diagnosis, (b) case conceptualization and treatment planning, and (c) treatment monitoring and evaluation. Recommendations are included for instruments with the greatest scientific support and for assessing schizophrenia in a clinically sensitive manner.

Growing evidence demonstrates that people with disabilities face more challenges in accessing healthcare and wellness resources, compared to non-disabled populations. As mobile applications focused on health and wellness (mHealth apps) become prevalent, it is important that people with disabilities can access and use mHealth apps. At present, there is no source of unified information about the accessibility and usability of mHealth apps for people with disabilities. We set out to create such a source, establishing a systematic approach for evaluating app accessibility. Our goal was to develop a simple, replicable app evaluation process to generate useful information for people with disabilities (to aid suitable app selection) and app developers (to improve app accessibility and usability). We collected data using two existing assessment instruments to test three top-rated weight management apps with nine users representing three disability groups: vision, dexterity, and cognitive impairment. Participants with visual impairments reported the lowest accessibility ratings, most challenges, and least tolerance for issues. Participants with dexterity impairments experienced significant accessibility-related difficulties. Participants with cognitive impairments experienced mild difficulties and higher tolerances for issues. Our pilot protocol will be applied to test mHealth apps and populate a “curation” website to assist consumers in selecting mHealth apps.

The performance of 123 language-disordered children on the DeRenzi and Faglioni form of the Token Test and the DeRenzi and Ferrari Reporter's Test were analyzed using two scoring conventions, and then compared with the performances of children with presumed normal language development. Correlations with other commonly used language assessment instruments are cited. Use of the Token and Reporter's Tests with children exhibiting language disorders is suggested.
