This paper focuses on the lives of women scavengers heads of households and how the state guarantees the fulfillment of the right to health. Women are relatively more vulnerable to various risks related to their life cycle and role in the family; thus, their access to health insurance is an obligation that must be met by the government. This paper has two objectives: to map social life and, at the same time, analyze the accessibility of the Healthy Indonesia National Health Insurance Card for female head of household scavengers in TPA Cipayung, Depok City. Using qualitative methods, and focusing on five dimensions of accessibility: availability, accessibility, accommodation, affordability, and acceptance, this study found that all health insurance arrangements are still very problematic. Even though these women scavengers are protected by JKN-KIS PBI, in reality, they are still challenging to get access to health services, ranging from the availability of doctors, the scarcity of drugs, to the discrimination of health services. This shows that they have not yet received full health insurance, which has a direct impact on the social and economic lives of the scavengers.

The Mexican Constitution establishes that everyone has the right to health protection and therefore, the law itself will define the bases and modalities for all to have access to health services. However, not everyone has access to quality medical services despite being in the supreme regulation. The State must understand that any alteration to health generates social security problems, mainly due to its consequences. The right to health is inalienable and does not distinguish between the social, economic, cultural or racial status of the individual. And, by stablishing it as a constitutional regulation, it amounted to an obligation of the State, which must provide quality services for all the society.  There are cases in which the right to health is violated, such as obstetrics, where pregnant women suffer the consequences of bad practices, or where minors are involved and their human rights are violated. We must focus our attention in the fact that their neglect has serious consequences and their impact generates human conditions that affect the dignity of the human being.

Introdução: O Brasil, juntamente com outros países, tem vivenciado o aumento acelerado do número de pessoas com mais de 60 anos de idade, desta forma à medida que a estrutura da população está mais envelhecida, a proporção de pessoas que utilizam os serviços de saúde aumenta. Portanto a disponibilidade de serviços, a distribuição geográfica, os mecanismos de financiamento dos serviços e a sua organização representam características que podem facilitar ou dificultar o acesso aos serviços de saúde para as pessoas idosas. Objetivo: Analisar as evidências cientificas publicadas sobre o acesso de pessoas idosas aos serviços de saúde. Método: Foi feita uma busca por artigos publicados de janeiro de 2009 a maio de 2019 nas bases de dados PubMed e Lilacs. Utilizaram-se os seguintes descritores (health services accessibility" OR "health services utilization" OR "access to health services") and ("elderly" OR "major adults" OR "older people" OR "aged" OR "older adults"). Os critérios de elegibilidade foram textos na forma de artigos científicos originais disponíveis on-line, na íntegra, que abordassem a temática nos idiomas português, inglês e espanhol. Resultados: A partir da combinação dos descritores, foram encontrados 1076 artigos, mas apenas 7 selecionados por se adequarem ao objetivo e fatores de inclusão. A maior parte dos estudos foi publicada em 2017 (42%), predominando delineamentos transversal (42%). Constatou-se entre os escolhidos, que alta escolaridade e renda estão associados com acesso a serviços de saúde por idosos fazendo a renda familiar influenciar na frequência de procura da assistência de saúde. Outro ponto está no fato do maior numero de consultas por parte dessa população e aalternativa de acesso com os serviços de home & care. Foi evidenciado também que a população de mais de oitenta anos de idade precisa ter um acesso ampliado aos serviços de saúde e as limitações funcionais são um impedimento ao acesso a saúde, evidenciando maior dificuldade aos serviços públicos do que privados. Conclusão: Desta forma o poder de compra e o grau de escolaridade estão intimamente ligados ao acesso aos serviços de saúde, toda via, são os serviços privados que estão mais adaptados para a demanda do paciente idoso, isso devido as lacunas do serviço de saúde púbica e o descaso do poder público

Abstract Objective: To estimate the prevalence and factors associated with the difficulties of access to health services among non-institutionalized older adults in the town of Montes Claros, Minas Gerais, Brazil. Method: A cross-sectional study nested in a population-based cohort of community-dwelling older adults was carried out in Montes Claros, Minas Gerais, Brazil. Data collection was performed in the homes of the older adults between November 2016 and February 2017. Demographic, socioeconomic, and health-related variables and access to and use of health services were evaluated. Bivariate analyzes (Pearson’s chi-squared test) were conducted, adopting a level of significance lower than 0.20 for inclusion of the independent variables in the multiple model. The final model was generated by Poisson regression analysis, with robust variance, and the variables maintained were associated with difficulty in using the health services up to a level of significance of 0.05 (p<0.05). Results: 394 older adults participated in this study, 33% of whom reported difficulties with access. In multiple analysis, greater difficulty of access was registered among older adults without a partner; who could not read; were frail and had a negative self-perception of health. Older adults face greater difficulties with access when seeking public services. Conclusion: A high perception of difficulty with access was identified, determined by social and physical aspects inherent to aging, and which may be worsened by the characteristics of public services. There is a need for investments in the health care of older adults, in order to guarantee care that promotes healthy aging.

Meeting men’s health needs by improving healthcare service access is a key objective of comprehensive primary health care. The aims of this qualitative study were to explore the perception of nurses in men’s health services and to describe men’s expectation of the nurse. The comparative component identifies the barriers and facilitators to improved access to health services. A purposive sample of 19 nurses and 20 men was recruited from metropolitan and regional settings in the state of Victoria, Australia, and each participant was interviewed individually or as part of three focus groups. The main findings were: nurses and men were unclear on the role of the nurse in men’s health; and health promotion provided by nurses was predominantly opportunistic. Both participant groups indicated barriers to healthcare access related to: the culture and environment in general practice; limitation of Australia’s Medicare healthcare financing system; out-of-pocket costs, waiting time and lack of extended hours; and men not wanting to be perceived as complainers. Facilitators related to: positive inter-professional relations; effective communication; personal qualities; and level of preparedness of nurse education. The findings demonstrate a need for the role to be better understood by both men and nurses in order to develop alternative approaches to meeting men’s healthcare needs.

The present study focuses on traditional usages of animals and plants species for medicinal purpose and indigenous knowledge system existent in Darai ethnic group of Mangalpur VDC, Chitwan, Nepal. A total of 28 animal species and 76 plant species were found to be used by Darai people to treat 22 and 36 different ailments, respectively. Darai people have their own indigenous knowledge for making various bamboo products and fishing equipments and local drink (moat/muna). The existence of knowledge associated with animals and plants and their medicinal utility is currently threatened mainly due to modernization, easy access to health services, lack of interest of youngsters, few local healers and fading of ethnic characters of Darai ethnic group.
