Major clinical challenges exist with differentiated thyroid cancers with distant metastases or rare but aggressive types, such as poorly differentiated thyroid carcinomas and anaplastic thyroid carcinomas. The precise characterization of the mutational profile in these advanced thyroid cancers is crucial. Samples were collected from primary tumors and distant metastases of 64 patients with distant metastases from differentiated thyroid cancer, poorly differentiated thyroid carcinoma, or anaplastic thyroid carcinoma. Targeted next-generation sequencing was performed with 50 known thyroid-cancer-related genes. Of the 82 tissues, 63 were from primary tumors and 19 from distant metastases. The most prevalent mutation observed from the primary tumors was TERT promoter mutation (56%), followed by BRAF (41%) and RAS (24%) mutations. TP3 was altered by 11%. Mutations in histone methyltransferases, SWI/SNF subunit–related genes, and PI3K/AKT/mTOR pathway-related genes were present in 42%, 12%, and 22%, respectively. When the mutational status was analyzed in 15 matched pairs of thyroid tumors and their matched distant metastases and one pair of distant metastases with two distinct sites, the concordance was high. A similar frequency of mutations in TERT promoter (58%) and BRAF (42%) as well as histone methyltransferases (37%), SWI/SNF subunits (10%), and PI3K/AKT/mTOR pathway (26%) were noted. The same main, early and late mutations were practically always present in individual primary tumor–metastasis pairs. Enrichment of TERT promoter, BRAF, and RAS mutations were detected in highly advanced thyroid cancers with distant metastasis. The genetic profiles of primary thyroid tumors and their corresponding distant metastases showed a high concordance.

The contribution of intratumor heterogeneity to thyroid metastatic cancers is still unknown. The clonal relationships between the primary thyroid tumors and lymph nodes (LN) or distant metastases are also poorly understood. The objective of this study was to determine the phylogenetic relationships between matched primary thyroid tumors and metastases. We searched for non-synonymous single-nucleotide variants (nsSNVs), gene fusions, alternative transcripts, and loss of heterozygosity (LOH) by paired-end massively parallel sequencing of cDNA (RNA-Seq) in a patient diagnosed with an aggressive papillary thyroid cancer (PTC). Seven tumor samples from a stage IVc PTC patient were analyzed by RNA-Seq: two areas from the primary tumor, four areas from two LN metastases, and one area from a pleural metastasis (PLM). A large panel of other thyroid tumors was used for Sanger sequencing screening. We identified seven new nsSNVs. Some of these were early events clonally present in both the primary PTC and the three matched metastases. Other nsSNVs were private to the primary tumor, the LN metastases and/or the PLM. Three new gene fusions were identified. A novel cancer-specific KAZN alternative transcript was detected in this aggressive PTC and in dozens of additional thyroid tumors. The PLM harbored an exclusive whole-chromosome 19 LOH. We have presented the first, to our knowledge, deep sequencing study comparing the mutational spectra in a PTC and both LN and distant metastases. This study has yielded novel findings concerning intra-tumor heterogeneity, clonal evolution and metastases dissemination in thyroid cancer.

ABSTRACT Introduction Sinonasal cancers have variable biological behavior and outcomes. The physical proximity of several critical structures renders radiotherapy challenging for these cancers. Purpose To report our early experience of helical tomotherapy (HT)-based image-guided intensity-modulated radiation therapy (IMRT) in midline sinonasal cancers. Materials and methods Patients with midline sinonasal cancers were accrued on a prospective generic protocol of HT-based IMRT. HT plans were evaluated using standardized indices. All patients were followed up clinicoradiologically. Local control was defined as absence of failure (recurrence/ progression) in the tumor bed, whereas distant disease control was defined as absence of distant metastases. All time-to-event data was analyzed using Kaplan-Meier methods. Results Ten patients with a median age of 42 years (range: 29-62 years) were included. HT was able to achieve excellent target volume coverage, good high-dose conformality with exquisite sparing of organs at risk. The acute toxicity of HT was generally mild and self-limiting. Seven patients experienced acute grade I-II ocular toxicity that responded to topical steroids, while one patient developed grade III conjunctivitis. The same patient later developed bilateral cataract necessitating extraction (late grade III ocular toxicity). No patient experienced dry-eye syndrome, corneal opacity or blindness. With a median follow-up of 27 months (interquartile range: 13-35 months), the 3-year Kaplan-Meier estimate of local progression-free survival, distant metastases-free survival, disease-free survival and overall survival was 59.3, 90, 53.3 and 90% respectively. Conclusion HT-based image-guided IMRT for midline sinonasal cancers achieves good high-dose conformality and is associated with mild, self-limiting acute ocular toxicity, minimal late morbidity with acceptable disease control.
