Purpose Before October 2000, physicians in our institution handwrote chemotherapy orders on blank order sheets. There was no standard to which the physician could include variables that were crucial to the completeness of a chemotherapy order. For this reason, chemotherapy orders were frequently incomplete and had to be adjusted by the pharmacist after discussing the missing variables with the ordering physician. As a part of our goal to minimize errors, standard chemotherapy forms were initiated at our institution in October 2000. Methods The first standard form implemented was a written order form that constituted a standard of the ideal variables necessary to accurately complete chemotherapy orders. These variables were the diagnosis, regimen, height, weight, body surface area (BSA), route, frequency, duration and chemotherapy dose and calculation based upon BSA. The next updated form was an electronic version similar to the original, and was implemented in April 2002. Results From February 1999 to March 2000, using the traditional unstandardized blank order sheets, the average order completeness was 45%. After the standard written forms were introduced, from October 2000 to March 2002, the average chemotherapy order completeness was 81%, an improvement of 36%. Completeness improved to an average of 93% from April 2002 to December 2003, after the implementation of the electronic chemotherapy form. Conclusion Chemotherapy order completeness improves considerably through the standardization of chemotherapy order forms. The electronic forms show an additional improvement over handwritten forms. Electronic standardization of chemotherapy forms should be adopted as a best-practice model in hematology-oncology practices throughout the country.

The criteria of GB/T 19580 are listed in form of linguistic term and its scoring system lacks of interim between adjacent grades. When the criteria are used in quality performance assessment, it will deal with large of qualitative indicators’ measurement and synthesis. The linguistic cloud model is introduced to implement the transition between qualitative criteria and quantitative measurement, and the evaluation approach of cloud center gravity is used to measure the change of multi-dimensional synthetic cloud. The degree of deviation θ is used to weigh the deflection between actual quality performance and best practice. Finally, a case is discussed to demonstrate the method proposed in this paper.

Forthcoming in 29 (1) National Law School of India Review 2017 India has experienced a dramatic increase in consumer Internet shopping due to its convenience and the myriad choices of the Internet. Further, India enjoys a rapidly developing economy, rising living standards, and improving middle-class income, all of which promote Internet shopping. However, worldwide, few consumers read their e-standard forms and India is likely no exception. In addition, the legal framework in India is inadequate to police vendor opportunism in the form of drafting unfair e-standard terms. Because consumer confidence is a prerequisite to growing Internet commerce, a more comprehensive and direct solution would be beneficial to India’s economyConsumer Internet Standard Form Contracts in India: A Proposal first examines existing strategies to protect consumers and concludes they may be inadequate. The article argues that the most promising approach to consumer protection comes from the American Law Institute’s (ALI) Principles of the Law of Software Contracts. The ALI Principles strategy is to encourage early Internet disclosure of a vendor’s standard terms, before a consumer initiates a transaction. The Principles treat such disclosure as a “best practice” that increases the likelihood of enforcement of the vendor’s e-standard terms. The ALI Principles also call for a more robust judicial use of policing tools, including unconscionability and public policy, despite the limitations of these principles. The ALI disclosure approach likely would not improve consumer reading of e-terms. However, rating services and watchdog groups (of which there are many on the Internet) would have better access to problematic terms and could publicize vendors’ use of them. Vendors would be motivated to draft reasonable terms to establish and preserve good will and good reputations.

ABSTRACTBackgroundCompetence committees play a key role in a competency-based system of assessment. These committees are tasked with reviewing and synthesizing clinical performance data to make judgments regarding residents’ competence. Canadian emergency medicine (EM) postgraduate training programs recently implemented competence committees; however, a paucity of literature guides their work.ObjectiveThe objective of this study was to develop consensus-based recommendations to optimize the function and decisions of competence committees in Canadian EM training programs.MethodsSemi-structured interviews of EM competence committee chairs were conducted and analyzed. The interview guide was informed by a literature review of competence committee structure, processes, and best practices. Inductive thematic analysis of interview transcripts was conducted to identify emerging themes. Preliminary recommendations, based on themes, were drafted and presented at the 2019 CAEP Academic Symposium on Education. Through a live presentation and survey poll, symposium attendees representing the national EM community participated in a facilitated discussion of the recommendations. The authors incorporated this feedback and identified consensus among symposium attendees on a final set of nine high-yield recommendations.ConclusionThe Canadian EM community used a structured process to develop nine best practice recommendations for competence committees addressing: committee membership, meeting processes, decision outcomes, use of high-quality performance data, and ongoing quality improvement. These recommendations can inform the structure and processes of competence committees in Canadian EM training programs.

Abstract: Tahun 2020, instrument akreditasi sekolah/madrasah mulai menggunakan Instrumen Akreditasi Satuan Pendidikan 2020 (IASP 2020). Dalam konteks IASP 2020, komponen mutu lulusan merupakan komponen yang memiliki bobot nilai paling tinggi dibandingkan dengan komponen lainnya. Penelitian ini bertujuan untuk mengeksplorasi proses penjaminan mutu lulusan berbasis akreditasi di Sekolah Menengah Kejuruan dengan menggunakan IASP 2020. Penelitian ini menggunakan metode penelitian studi kasus. Partisipan dalam penelitian ini ditentukan dengan metode purposive sampling, dengan kriteria: seluruh warga SMK yang telah mengalami proses penjaminan mutu lulusan dengan menggunakan IASP 2020. Jumlah partisipan dalam penelitian ini sebanyak 16 orang guru dan karyawan. Hasil penelitian menunjukan bahwa proses penjaminan mutu lulusan berbasis akreditasi dilakukan melalui program: 1) Persiapan: pemahaman IASP 2020, pembentukan tim penjamin mutu lulusan; 2) Melakukan EDS mutu lulusan berbasis IASP 2020; 3) Analisis kesenjangan antara hasil EDS dengan kondisi ideal; 4) Pemenuhan gap melalui perbaikan kinerja pada komponen mutu lulusan; 5) Evalusi program ketercapaian kinerja mutu lulusan dan tindak lanjut. Abstrak: In 2020, the school accreditation instrument began to use the 2020 Education Unit Accreditation Instrument (IASP 2020). In the context of the 2020 IASP, the component of graduate quality is the component that has the highest value weight compared to other components. This study aims to explore the process of quality assurance for accreditation-based graduates in Vocational High Schools using the IASP 2020. This study uses a case study research method. Participants in this study were determined by purposive sampling method, with the criteria: all SMK citizens who have undergone a graduate quality assurance process using IASP 2020. The number of participants in this study was 16 teachers and employees. The results showed that the accreditation-based graduate quality assurance process was carried out through the following programs: 1) Preparation: understanding IASP 2020, forming a graduate quality assurance team; 2) Conducting the 2020 IASP-based graduate quality EDS; 3) Gap analysis between EDS results and ideal conditions; 4) Fulfilling the gap through improving the performance of the graduate quality component; 5) Evaluation of the program for the achievement of the quality performance of graduates and the follow-up. A “new charts program” for the Americal Association of Variable Star Observers was instigated in 1966 via the gift to the Association of the complete variable star observing records, charts, photographs, etc. of the late Prof. Charles P. Olivier of the University of Pennsylvania (USA). Adequate material covering about 60 variables, not previously charted by the AAVSO, was included in this original data, and was suitably charted in reproducible standard format.Since 1966, much additional information has been assembled from other sources, three Catalogs have been issued which list the new or revised charts produced, and which specify how copies of same may be obtained. The latest such Catalog is dated June 1978, and lists 670 different charts covering a total of 611 variables none of which was charted in reproducible standard form previous to 1966.

Purpose This study examined the current pediatric amplification practice with an emphasis on hearing aid verification using probe microphone measurement (PMM), among audiologists in Klang Valley, Malaysia. Frequency of practice, access to PMM system, practiced protocols, barriers, and perception toward the benefits of PMM were identified through a survey. Method A questionnaire was distributed to and filled in by the audiologists who provided pediatric amplification service in Klang Valley, Malaysia. One hundred eight ( N = 108) audiologists, composed of 90.3% women and 9.7% men (age range: 23–48 years), participated in the survey. Results PMM was not a clinical routine practiced by a majority of the audiologists, despite its recognition as the best clinical practice that should be incorporated into protocols for fitting hearing aids in children. Variations in practice existed warranting further steps to improve the current practice for children with hearing impairment. The lack of access to PMM equipment was 1 major barrier for the audiologists to practice real-ear verification. Practitioners' characteristics such as time constraints, low confidence, and knowledge levels were also identified as barriers that impede the uptake of the evidence-based practice. Conclusions The implementation of PMM in clinical practice remains a challenge to the audiology profession. A knowledge-transfer approach that takes into consideration the barriers and involves effective collaboration or engagement between the knowledge providers and potential stakeholders is required to promote the clinical application of evidence-based best practice.
