Aims and MethodThis paper is based on two audits carried out in 1996 and 1998 atThe State Hospital, Carstairs. Each audit looked at anti-muscarinic drug use within the hospital, in relation to approved prescribing standards issued in 1996. All patients within the hospital on anti-muscarinic drugs were identified at the timeof each audit. These prescriptions were compared with the approved prescribing standards. In the 1998 audit additional information was obtained from the case notes and the consultants, when the approved standards were not met.ResultsThe percentage of patients on regular anti-muscarinic drugs, falling outwith the prescribing standards, reduced between the two audits. However, in 1998 a small number of patients were still outwith the approved prescribing standards set in 1996.ConclusionsAn improvement in the prescribing practice of anti-muscarinics occurred following the introduction of prescribing guidelines. However, the guidelines were not fully met in the 1998 audit. This demonstrates the need for further audit and continued monitoring of anti-muscarinic prescription at the State Hospital.

OBJECTIVE: To review and highlight the opportunities and challenges of pharmacologic advances in the use of antipsychotics for the state hospital system. METHODS: A critical review was performed of studies published either as articles or abstracts, on the use of novel antipsychotics, particularly as they relate to the patient population within the state mental hospital system. FINDINGS: The recent availability of new antipsychotic medications within state facilities has resulted in more progressive treatment, reduced recidivism (and consequently cost savings), and preliminary evidence of preferential and superior treatment response in specific patient subgroups (e.g., those with aggression). At the same time, inpatient pharmacy budget increases and uncertainty in guiding the use of novel antipsychotics have influenced the availability of these agents in state hospitals. CONCLUSIONS: State hospital services have, by and large, embraced the developments in pharmacotherapy of schizophrenia. Optimal use of these new agents in this population requires additional information on their relative efficacy in specific patient subgroups.

Adolescence is characterized by premature experimentation with new experiences and sensations. These experiences sometimes include drugs, which even though legal and socially accepted, begin to have noticeable negative consequences to the adolescent’s development. In recent years, a decrease in use of tobacco by Spanish adolescents has been observed, but not in alcohol. One of the causes of initiation in drug use is impulsive personality or behavior. Thus the purpose of this study was to analyze the relationship between impulsiveness and frequency of use of alcohol and tobacco in 822 students aged 13 to 18 years of age. The State Impulsivity Scale (SIS) and an ad hoc questionnaire on demographic characteristics and use of alcohol and tobacco were used for this. The results showed that students who stated they were users scored significantly higher on impulsivity. Thus detailed analysis of the profile of individuals with this risk factor could favor more adequate intervention program design.

The article problematizes state penality as a mechanism of repression of precarious workers through a war on drugs in the Philippines. The narratives of 27 arrested ‘drug personalities’ in Metro Manila tell of how methamphetamine energizes bodies and motivates minds for productive work. Bidding to be classified as willing and able workers and family men, the study’s participants orient to a moral stratification that pits the ‘moral versus immoral’ and the ‘hardworking versus lazy’. Qualifying their drug use as strategic and calculated, they uphold the neoliberal values of individual choice and accountability. Their support for the anti-drug campaign stems from their recognition of a drug problem and the socioemotional toll of the dysfunctions of living in the slums. While trade liberalization facilitates methamphetamine inflow, a war on drugs fuels an authoritarian populism. As the state reaffirms symbolic mission to protect its citizens, it blames precarity to a problem population.

Total Knee Replacement (TKR) is an important and in-demand procedure for the aging population of the United States. In recent decades, the number of TKR procedures performed has shown an increase. This pattern is expected to continue in the coming decades. Despite medical advances in orthopedic surgery, a high number of patients, approximately 20%, are dissatisfied with their procedure outcomes. Common causes that are suggested for this dissatisfaction include loosening of the implant components as well as infection. To eliminate loosening as a cause, it is necessary to determine the state of the implant both intra- and post-operatively. Previous research has focused on passively sensing the compartmental loads between the femoral and tibial components. Common methods include using strain gauges or even piezoelectric transducers to measure force. An alternative to this is to perform real-time structural health monitoring (SHM) of the implant to determine changes in the state of the system. A commonly investigated method of SHM, referred to as the electromechanical impedance (EMI) method, involves using the coupled electromechanical properties of piezoelectric transducers to measure the host structure’s condition. The EMI method has already shown promise in aerospace and infrastructure applications, but has seen limited testing for use in the biomechanical field. This work is intended to validate the EMI method for use in detecting damage in cemented bone-implant interfaces, with TKR being used as a case study to specify certain experimental parameters. An experimental setup which represents the various material layers found in a bone-implant interface is created with various damage conditions to determine the ability for a piezoelectric sensor to detect and quantify the change in material state. The objective of this work is to provide validation as well as a foundation on which additional work in SHM of orthopedic implants and structures can be performed.
