Out-patient clinics are widely used to asssess and treat patients. In psychiatric practice, out-patient clinics are used both to assess new referrals and follow up existing patients. More than three-quarters of psychiatric out-patient attendances are for follow-up (DHSS, 1984). With modern emphasis on community psychiatry, the role of out-patient clinics may become less prominent and more refined. Very little appears to have been published on the subject of psychiatric follow-up clinics.

Kidney cysts are quite common in adults. Though small simple renal cysts in an adult over 30-40 years of age are not too unusual, however, if the same cysts are seen in a child, and especially if there are additional findings, then several diagnostic possibilities may come to mind. The role of ultrasound, together with the help of intravenous contrast agents and Doppler mode, are very critical in describing the morphologic features and follow-up of the complex or multiple and bilateral renal cysts. These sonographic signs are occasionally specific for diagnosis, but in many cases sonographic clues should be evaluated together with the other genetic and clinical data to reach diagnosis.The first part of this pictorial essay included the introduction into the subject and the classification of non-genetic cystic renal diseases. The key features for the non-genetic cystic renal diseases are illustrated. In the second part, eye-catching features of genetic cystic renal diseases are demonstrated.

Objective: To reflect on the role of psychiatry in authorising physical treatments for Gender Dysphoria and to examine the quality of evidence for gender-reassignment. Method: A Medline search was performed with the subject term “transsexualism” or “gender dysphoria” and “outcome” or “follow-up” in the title. Studies published from 2005 onwards reporting psychosocial outcomes were selected for review. Results: Most available evidence indicating positive outcomes for gender reassignment is of poor quality. The few studies with robust methodology suggest that some patients have poor outcomes and may be at risk of suicide. Conclusion: The author raises questions about the implications for ethical treatment of transgender individuals.

Background:The ability to predict conversion to multiple sclerosis (MS) accurately when assessing a patient with a clinically isolated syndrome (CIS) is of paramount importance.Magnetic resonance imaging (MRI) is the best paraclinical tool currently available; however the significance of a history of an event suggestive of demyelination prior to CIS presentation has not been evaluated.Methods:Aretrospective chart review of all optic neuritis cases presenting as CIS to a single neuro-ophthalmologist in London, Ontario between 1990 to 1998 was performed. Data were collected regarding demographics, past medical history, history of present illness, and family history. Conversion to MS was determined by the McDonald criteria after ten years of follow-up. Bayesian statistics and logistic regression were used to determine the best predictors of conversion to MS from CIS.Results:One hundred and sixteen optic neuritis subjects were included in the analysis. After ten years, 42.2% had converted to MS. The best predictor of future conversion remained at least one brain lesion, disseminated in space, on MRI (sensitivity 0.90, specificity 0.75). However, if the subject additionally had a history suggestive of a demyelinating event in the past that had not been confirmed clinically, the specificity increased to 0.96. These two traits taken together had an odds ratio of 27.8 for conversion to MS in the next ten years (p<0.001).Conclusions:A history of an event suggestive of demyelination prior to presenting with optic neuritis as CIS increases the ability of the clinician to predict conversion to MS in the next ten years.

This article reports on the ways in which psychiatric practice and power were constituted in a Danish asylum at the beginning of the nineteenth century. The point of departure will be a complaint by a former patient questioning the practice at the asylum in 1829. In an analysis of this narrative the study draws upon Foucauldian concepts like disciplinary power, confession, pastoral power and subjectivation. I will argue that the critique of the patient provides us with an example of the way that disciplinary power works in the case of an informal indictment of the methods and practice at an asylum. A key issue is whether the critique is not itself a part of the self-legitimation of disciplinary power.

Abstract The transgender universe comprises a wide range of individuals who do not identify with the gender role related to their birth sex, presenting distinct gender identities that transcend the binary concept of female and male. The follow-up of this population requires specific knowledge and training for its demands and peculiarities. Hormone therapy is a key point in the process of gender adequacy, and despite the increase in demand for specialized health services, there are still many barriers to full and free of prejudice health care. This is a descriptive and exploratory study about the characteristics of the professional training of the doctors involved in transgender health care, in particular the endocrinologist, and to enable an overview of the doctor-patient relationship and medical follow-up in the context of transsexuality in the Federal District. For this purpose, questionnaires were used for physicians: endocrinologists, family and community physicians, urologists, and psychiatrists; and transgender people residents of the Federal District. This study shows that most of the professionals involved in the process of gender adequacy, in particular the endocrinologist, do not present confidence or knowledge to accomplish it, and prejudice is still presented in a striking way in health care. In Federal District, services are not adequately structured for the care of this population, both from the perspective of doctors and transgender people. Moreover, in this sample, it was observed that the higher degree of specific knowledge in the subject increases the sensation of confidence of the professional to treat transgender people but does not correlate with the prejudice.

ABSTRAKSI: Pengembangan media ajar mata kuliah PETISI, atau Pendidikan Anti-Korupsi, berbasis visual komik diharapkan dapat membantu mahasiswa dan generasi muda untuk menjadi agen perubahan, kaderisasi, dan pengenalan tindakan korupsi yang dilakukan mahasiswa dalam keseharian melalui media gambar yang mudah dipahami. Komik juga bisa digunakan untuk berbagai tujuan, dari adanya transformasi tujuan dimana komik bisa menjadi media yang informatif dan edukatif. Penelitian ini bersifat deskriptif yang mengacu pada Teori Pengembangan dengan pendekatan 4-D, yakni: Pendefinisian atau “Define”, Perancangan atau “Design”, Pengembangan atau “Develop”, dan Penyebaran atau “Disseminate”. Aspek efektivitas penggunaan media ajar PETISI ini cukup baik dan efektif untuk mengenalkan dan mengoptimalkan peran mahasiswa dalam gerakan anti-korupsi di Pulau Madura, Jawa Timur, Indonesia. Dengan diberikan media ajar PETISI, ianya lebih memudahkan mahasiswa untuk mendiskusikan segala bentuk gerakan anti-korupsi yang dapat dilakukan di lingkungan kampus. Perlu adanya peranan yang lebih spesifik terkait dengan bidang pembelajaran pendidikan anti-korupsi itu dimasukkan pada materi mata kuliah dan komik seperti apa yang akan disampaikan. Tindak lanjut terkait dengan komik PETISI adalah perlu adanya peran dan gerakan yang lebih mengilustrasikan mahasiswa di luar kampus.KATA KUNCI: Media Ajar; Pendidikan Anti-Korupsi; Komik; Peran Mahasiswa. ABSTRACT: “Development of Educational Media on PETISI or Anti-Corruption Education”. The development of teaching media on visual comic-based of PETISI, or Anti-Corruption Education, course is expected to help students and young generation to become agents of change, regeneration, and recognition of corruption acts done by students in everyday life through an easily understood media. Comics can also be used for various purposes, from the transformation of purpose itself where comics can be an informative and educative media. This research is descriptive refers to Development Theory that is 4-D approaches include: Define, Design, Development, and Disseminate. Effectivity aspects of the use of teaching media of PETISI is quite good and effective to introduce and optimize the role of students in the anti-corruption movement in the island of Madura, East Java, Indonesia. With the given teaching media of PETISI, it is easier for students to discuss all forms of anti-corruption movement that can be done in the campus environment. The need for a more specific role related to the field of learning anti-corruption education was included in the subject matter and what’s kind of the comics will be dessiminated. The role of top-down processing on the horizontal-vertical line length illusion was examined by means of an ambiguous room with dual visual verticals. In one of the test conditions, the subjects were cued to one of the two verticals and were instructed to cognitively reassign the apparent vertical to the cued orientation. When they have mentally adjusted their perception, two lines in a plus sign configuration appeared and the subjects had to evaluate which line was longer. The results showed that the line length appeared longer when it was aligned with the direction of the vertical currently perceived by the subject. This study provides a demonstration that top-down processing influences lower level visual processing mechanisms. In another test condition, the subjects had all perceptual cues available and the influence was even stronger.

Abstract. Background: Little is known about the role of resilience in the likelihood of suicidal ideation (SI) over time. Aims: We examined the association between resilience and SI in a young-adult cohort over 4 years. Our objectives were to determine whether resilience was associated with SI at follow-up or, conversely, whether SI was associated with lowered resilience at follow-up. Method: Participants were selected from the Personality and Total Health (PATH) Through Life Project from Canberra and Queanbeyan, Australia, aged 28–32 years at the first time point and 32–36 at the second. Multinomial, linear, and binary regression analyses explored the association between resilience and SI over two time points. Models were adjusted for suicidality risk factors. Results: While unadjusted analyses identified associations between resilience and SI, these effects were fully explained by the inclusion of other suicidality risk factors. Conclusion: Despite strong cross-sectional associations, resilience and SI appear to be unrelated in a longitudinal context, once risk/resilience factors are controlled for. As independent indicators of psychological well-being, suicidality and resilience are essential if current status is to be captured. However, the addition of other factors (e.g., support, mastery) makes this association tenuous. Consequently, resilience per se may not be protective of SI.
