Female Nursing of Male Patients in Asylums

The question of utilising female nursing to a greater extent than formerly in the care of male patients in asylums has roused much interest of late years, and has been brought under the consideration of the Association on several occasions. In advocating the adoption of this form of nursing one is apt to give the impression that the method is something entirely new as applied to asylum patients. But that is not intended, for in reality the system has existed to some degree for a long time; and it is only the question of the advantages of extending it, and making it much more systematic and complete, that is now raised. In some places on the Continent it has been in use for a number of years, and is developed to a greater extent than is usual in this country. In April last I had an opportunity, in company with Sir John Sibbald, formerly Commissioner in Lunacy for Scotland, Dr. Fraser, Commissioner in Lunacy for Scotland, and Dr. Robertson, medical superintendent of the Stirling District Asylum, of visiting two institutions in Holland and seeing the method in practice; and I now venture to submit some notes, not by any means exhaustive, of what was observed there, and to make these, and my own further experience of the system at the Fife Asylum since 1896, when I had the privilege of reading a paper descriptive of it at the annual meeting of the Association, the basis of my remarks at this time.

At the meeting of the National Council in Washington, February 20, 1932 the following motion was passed without dissenting vote: “The National Council of Teachers of Mathematics reaffirm their interest in the question of geometry as expressed at the annual meeting in Detroit, and hope that the Board of Directors will find it possible to initiate and carry forward a study of the whole question of geometry in our schools.” The directors forthwith empowered the new president, Mr. Betz, to appoint a Committee on Geometry; and he, anxious that some work should go forward during the long time necessary for the final organization of this and other committees, selected six members of the National Council for immediate service on the committee, intending to enlarge the membership as soon as possible in order that it might include an even wider range of interest and opinion.

It will be within the recollection of the readers of the “Journal of Mental Science,” that at the Annual Meeting of the Medico-Psychological Association, which was held last year in Dublin, a paper was read by Dr. Stewart, of Bristol, on the subject of the Irish Lunatic Asylum Service. Dr. Stewart's paper, which was listened to with great interest, contrasted the position of English and Irish Medical Superintendents of Public Asylums, and pointed out that Irish Superintendents were required to perform so many and such various duties that little time was left to them for the cultivation of the scientific branches of psychology, that they are, for instance, held responsible for the discharge of duties that in England fall to the share of the Clerk to the Committee of Visitors, and to the Steward of the Asylum. English Medical Superintendents were Bomewhat surprised to find their Irish colleagues charged with the duty of making out voluminous returns, conducting the whole correspondence of their establishments, paying the salaries and wages of the staff, managing the asylum farms; and that in the majority of Irish institutions for the insane, these duties had to be performed without even the assistance of a second resident medical officer. The fact of the meeting being held in Dublin, and the large number of Irish members of the asociation who were present, led to a long discussion upon the subject-matter of Dr. Stewart's paper, and the Irish Superintendents were gratified to observe the warm interest manifested in their behalf by the English and Scotch members of the Association present. For the information of those who were not able to attend the Annual Meeting, we may observe that the position of Medical Superintendent of Irish District Asylums, until the discussion of last year, had been little known or understood by their English brethren. In England the appointment of Superintendent rests in the hands of the Committee of the respective Asylums—he is their officer, entirely under their control, and is paid by them. Whether this is in accordance with the most advanced ideas of asylum management or is the best possible relative position, either for the medical men themselves, or the patients under their care, may perhaps be a matter of doubt. In Ireland the case is very different; there the appointment of Superintendent is made by the Lord Lieutenant; the Superintendent holds his office from Government, is dismissed only by order of the Lord Lieutenant, is responsible to him, but receives his pay from the county cess levied for the general support of the Asylum—an anomalous position, and one which, in these days of change, can hardly be expected to last.

Colon cancer is an abnormal growth of cells that occurs in the large intestine. Sometimes growth remains restricted for a relatively long time before it becomes a malignant tumor and then spreads through the intestinal wall to the lymph nodes and other parts of the body. The study aims to estimate the effectiveness and partial purification of lipoxygenase (LOX) enzyme and measure gamma-glutamyle transferase (GGT) activity in serum patients of colon cancer in Baghdad. The study included (80) case male patients with colon cancer with (50) samples of apparently healthy males (control) as comparison group. The result displayed a noteworthy increase in lipoxygenase effectiveness (805.0±517.23 IU/L) in serum of patients with colon cancer (T3 stage) compared with control (114.6±49.77 IU/L). The enzyme was purified by the precipitation of the serum protein using (40% (NH4)2SO4) then removing the remaining salts by dialysis. The column of gel (sephadex G.100) was used to separate the enzyme from another protein, in this step a single peak was obtained. The effective part of lipoxygenase at yield (71.42%) and folds (11.033). The ion exchange chromatography (DEAE–CeA50) was used to isolate LOX isoenzyme, two bands (LOX1 and LOX2) were acquired with different degree of purity (16.372) and (12.16) folds respectively. The result displayed a noteworthy increase in the (GGT) activity in patients (58.69±16.94IU/L) (probability P≤ 0.000) compared with control (12.79±5.68 IU/L). The increase in activity of LOX can be used as a tumor marker to detect the colon cancer disease.

RESUMO Objetivo: identificar as percepções de pacientes do sexo masculino em tratamento dialítico substitutivo sobre a sexualidade. Método: trata-se de estudo quantitativo, descritivo e exploratório, junto a 39 homens em tratamento hemodialítico. Coletaram-se os dados por meio de questionamentos. Realizaram-se procedimentos para a análise exploratória de dados, apresentados em tabelas e recorte dos registros das falas. Resultados: citaram-se, pela maioria dos entrevistados, alterações no cotidiano devido às limitações físicas e restrições por apresentarem uma doença crônica com prognóstico incerto e por terem que ficar por muito tempo à disposição das sessões de hemodiálise. Relataram-se sobre a sexualidade, queixas sexuais, levando-se a crer que a adaptação à nova rotina, decorrente do tratamento e da limitação, causou prejuízos à vivência sexual do paciente; destacaram-se a disfunção erétil, a ejaculatória e o cansaço físico. Conclusão: torna-se importante o cuidado do enfermeiro para desenvolver atividades que auxiliem na vivência e na aceitação das modificações ocasionadas pela condição crônica englobando não só os pacientes, mas, também, seus familiares, companheiras e amigos, promovendo a atenção integral ao paciente renal crônico, em todas as dimensões do cuidado, incluindo a sexualidade. Method: this is a quantitative, descriptive and exploratory study, together with 39 men undergoing hemodialysis. The data were collected through questioning. Results: most of the interviewees reported changes in their daily lives due to physical limitations and restrictions because they had a chronic disease with an uncertain prognosis and because they had to remain at the disposal of the hemodialysis sessions for a long time. They reported on sexuality, sexual complaints, leading one to believe that the adaptation to the new routine, due to the treatment and the limitation, caused damages to the sexual experience of the patient; erectile dysfunction, ejaculatory and physical fatigue were highlighted. Conclusion: Nursing care is important to develop activities that help in the living and acceptance of the changes caused by the chronic condition encompassing not only the patients, but also their relatives, partners and friends, promoting integral care for the renal patient chronic, in all dimensions of care, including sexuality. Resultados: Se ha citado, por la mayoría de los entrevistados, alteraciones en el cotidiano debido a las limitaciones físicas y restricciones por presentar una enfermedad crónica con pronóstico incierto y por tener que quedarse por mucho tiempo a la disposición de las sesiones de diálisis. Se relataron sobre la sexualidad, quejas sexuales, llevándose a creer que la adaptación a la nueva rutina, decurrente del tratamiento y de la limitación causó pérdidas a la vivencia sexual del paciente; se destacaron una disfunción eréctil, la eyaculatoria y el cansancio físico. Conclusión: Se torna importante el cuidado del enfermero para desarrollar actividades que puedan auxiliar en la vivencia y en la aceptación de las modificaciones ocasionadas por la condición crónica, englobando no solo los pacientes, pero, también sus familiares, compañeras y amigos, promoviendo la atención integrativa al paciente renal crónico, en todas las dimensiones del cuidado, incluyendo la sexualidad.
