Pharyngeal injuries caused by trauma are common and have been reported previously in the medical literature. In some cases of a penetrating injury there is a collection of air in the retropharyngeal space that can be shown on lateral soft tissue radiography of the neck. If this condition is not diagnosed or adequately treated the patient may develop severe complications such as mediastinitis. A case is reported of a patient with penetrating injury caused by a pencil and the subsequent treatment with review literature are described.

UNSTRUCTURED A case report is a detailed narrative that describes the symptoms, signs, diagnosis, treatment, and follow-up of a medical problem of an individual patient. They usually describe an unusual or novel occurrence and although they lie at the bottom of the hierarchy of the Evidence Based Medicine Pyramid, they still remain one of the cornerstones of medical progress and provide key additions to the existing medical literature. Unfortunately, abysmally few physicians-in-training receive a formal education regarding what constitutes a publishable case report. The article throws light on this aspect of medical education

Chlorhexidine gluconate is a chemical antiseptic that is effective against Gram positive and negative bacteria and on certain viruses and fungi. A bacteriocidal and bacteriostatic agent, this cationic drug is absorbed on to negatively charged cell surfaces of organisms, disrupting the cell membrane, which results in increased permeability. Its use is mainly topical as a surgical hand antisepsis, site preparation/cleansing and for pre-genitourinary procedures such as urethral catheterisation. Like any drug, caution should be employed with its use as hypersensitivity reactions are being documented increasingly in the medical literature. In the following case, we present a patient who was catheterised with the chlorhexidine containing Instillagel® (CliniMed, High Wycombe, UK), prior to undergoing elective orthopaedic surgery.

Worldwide occurrence of snakebite in pregnancy is little known in medical literature. Furthermore to the best of our knowledge there is no case report on Krait envenomation in pregnancy in Bangladesh. It carries significant fetal wastage and maternal morbidity and mortality. Background: Myxopapillary ependymoma (MPE) with anaplastic features is extremely rare. There are very few such case reports in the medical literature. Case Description: A 23-year-old female presented with lower back pain, and both urinary and fecal dysfunction. The patient underwent gross total surgical excision of the MR documented expansile intrasacral tumor. The histology was compatible with a MPE containing anaplastic features. Conclusion: The medical literature contains a few comparable cases of subcutaneous sacrococcygeal MPE with anaplastic components. Here, however, we have a young female with an anaplastic intrasacral MPE treated with gross total surgical excision.

I believe this is one of the earliest case reports of precocious puberty in American medical literature. I take the liberty to acquaint you with an extraordinary case of prematurity in a child, which fell under my observation in this city. Matilda H. was born of a white family in low circumstances, the 31st of September, 1827. She came into this world with her mammae perfectly formed, and the mons veneris covered with hair, as much as a girl between thirteen and fourteen years old; when precisely three years of age the menses made their appearance, and have continued to reappear regularly every month until the present time, and as copious as any woman might have them-each period lasting four days. She is now four years and five months old; she measures forty-two inches and a half in height, French measure; her features are regular; she has a rosy complexion; her hair chestnut colour; her eyes bluish-gray; she is what may be called handsome; and conformation of her body is very strong; her mammae are now of the size of a full-grown orange, and the dimensions of the pelvis are, in my opinion, such as to enable her to bear children when eight years old, and very likely sooner. She constantly enjoys good health.

Bannayan-Riley-Ruvalcaba syndrome (BRRS) is a rare autosomal dominant congenital disorder, characterized bymacrocephaly, lipomas, hamartomas, and pigmented macule in genitalia.Several dozen cases have been reported in the medical literature, but no case has been reported in Bangladesh. We report a case of BRRS in a 11-year-old male child with recurrent per rectal bleeding with hamartomatouscolonic polyposis & multiple subcutaneous lipomas on the anterior abdominal wall.In addition, patient had macrocephaly, intellectual impairment.Bleeding polyps were removed by colonoscopic polypectomy. A case of gangliocytic paraganglioma of the duodenum, the 25th in world medical literature, is reported. The histologic appearance consists of epithelioid cell nests resembling « Zellballen » of carotid body tumors, together with gangliocyte-like elements. Ultramicroscopically, the cytoplasm of the cells contains round electron-dense granules or a large number of filaments. Occasional cells contain both granules and filaments.
