Folie à Cinq: A Clinical Study

Ever since the clinical entity of “Folie à Deux” was first introduced (Lasègue and Falret, 1877), a great number of clinical and theoretical studies have appeared in the literature dealing with this relatively rare and interesting mental syndrome. In the English medical literature, up to date, 93 cases of “Folie à Deux” have been reported (Gralnick, 1942), a further 17 cases of “Folie à Trois” (Wolff, 1957; Dewhurst and Eilenberg, 1961), one case of “Folie Imposée à Quatre” (Ropschitz, 1957), one case of “Psychosis of Association” involving a mother and three sons (Page, 1942) and one case of “Folie à Quatre” involving four members of a family (Gralnick, 1943), have also been reported. I personally recorded one example of “Folie à Trois” at the Lebanon Hospital for Mental Diseases in 1947, involving a mother and her two daughters, and another interesting case of “Folie à Deux” in Baghdad (1961) involving a young boy who broke down under the influence of his spiritualist cousin. During the course of the boy's illness the cousin broke down herself into an acute delusional state. Both patients eventually recovered completely.

A case of folie à deux dissociative (dissociative hysteria) disorder in an 8 and 12 year-old sister and brother is presented. Illnesses of this type are very rare and there is little medical literature on this subject. Our patients, almost simultaneously, abruptly had complete loss of memory, disorientation, loss of awareness about who they were, and much anxiety, which lasted about 15 hours. Both patients were physically well and no abnormalities were found on physical examination, routines laboratory tests and EEG studies. Speculations about the emotional and interpersonal causes of this illness in these two patients are given.

ZusammenfassungDie Übertragung des seltenen Dermatozoenwahns auf andere Personen kommt in etwa 5 bis 15% der Fälle vor. Es wird der Fall einer 72-jährigen Frau beschrieben, die den Dermatozoenwahn ihres schizophrenen Sohnes übernahm. Bei der Mutter war das Wahnkriterium der Unkorrigierbarkeit im Gegensatz zum Sohn jedoch nicht vollständig erfüllt. Erst nach Besserung der schizophrenen Symptomatik und des Dermatozoenwahns beim Sohn, kam es auch zur vollständigen Remission bei der Mutter. Die andauernde soziale Isolation von Sohn und Mutter förderte das Auftreten einer Folie á deux. Die möglichen Umstände, die zum induzierten Dermatozoenwahn führen, werden anhand dieses Falles diskutiert. Obwohl beim Dermatozoenwahn per definitionem ein Wahn vorliegt, ist die Wahngewissheit bei den induzierten Partnern im Vergleich zu den primär Erkrankten nicht immer derart ausgeprägt, sodass im Einzelfall eher überwertige Ideen vorliegen. Daraus können sich gezielte therapeutische Alternativen zur neuroleptischen Behandlung ergeben.

Na classificação de Gralnick, de 1942, a folie à deux é dividida em quatro subtipos básicos: folie imposée, folie communiquée, folie simultannée e folie induite. O último termo se refere à adição de novas idéias delirantes por paciente previamente psicótico sob influência de outro paciente. Relatamos um caso de folie induite ocorrido entre mãe e filho primariamente psicóticos (transtorno delirante e esquizofrenia, respectivamente), que passaram a compartilhar delírios de natureza persecutória e sexual. Casos de folie à deux geralmente correspondem ao transtorno psicótico induzido (DSM-IV-TR) ou transtorno delirante induzido (CID-10). No entanto, o paciente deste relato não pode receber tais diagnósticos, já que nos critérios validados atualmente percebe-se a exigência de que o parceiro induzido não possua um transtorno psicótico anterior ao início do compartilhamento do delírio. Consideramos que o presente relato exemplifica a insuficiência das modernas classificações nesta área. Casos de folie induite podem necessitar de manejo distinto dos demais casos de psicose compartilhada ou não compartilhada. Sugerimos que os critérios diagnósticos correntes poderiam ser revisados para incluir condições psiquiátricas como essa.

AbstractSchwannoma of the tonsil is an extremely uncommon clinical entity with only one reported case in an adult in the medical literature to date. We report, to our knowledge, the first known case in a child.
