Effectiveness of Chitosan Nanohydrogel as a Bone Regenerative Material in Intrabony Defects in Patients With Chronic Periodontitis: A Randomized Clinical Trial

Aim: Different procedures have been attempted to regenerate intrabony defects in periodontitis but complete regeneration is still a herculean feat. Nanoparticles could prove to be advantageous because of their small size and increased surface area. The aim was to evaluate the effectiveness of a chitosan nanohydrogel as a periodontal regenerative material in the management of intrabony defects. Materials and Methods: Twenty patients within the age group of 30 to 60 years with intrabony defects >3 mm and clinical attachment levels >5 mm were selected. They were divided randomly into two groups: group-1, which received only open flap debridement with bone graft and acted as control, and group-2, which received open flap debridement, bone graft mixed with a chitosan nanohydrogel, and acted as the test site. Clinical and radiographic measurements were recorded at baseline, three months, and six months. Intragroup comparison was done using a paired t-test and intergroup comparison was done using an unpaired t-test with the statistical value set at P < .05. Results: Chitosan hydrogel incorporated bone grafts showed improved bone regenerative potential. The mean reduction in clinical attachment level in group-2 (8.7 ± 0.6 to 1.6 ± 0.8) was significantly higher than that of group-1 (8.6 ± 0.5 to 2.4 ± 0.7). The mean reduction in probing depths in group-2 (test group) was significantly higher (8.3 ± 0.9 to 1.6 ± 0.8) when compared to group-1 (8 ± 0.8 to 2.3 ± 0.7). Significant amount of defect fill was obtained in both the groups from baseline to six-month period. Conclusion: Organic materials such as chitosan could have potential bone regenerative capacity.

Physical fitness is ability to perform the activity or activities without feeling tired excessively. Physical fitness associated with a person`s organs to carry out its tasks properly every day without experiencing significant fatigue. In a child’s mental retardation with a les mobile lifestyle that will affect his physical fitness. The purpose of this study to determine which type of gymnastic exercise better to the physical fitness on the students in Sekolah Dasar Luar Biasa Kuncup Bunga Denpasar.This research used experimental method using Randomized study Pre and Post Test Group Design. The subjects in this study were students in Sekolah Dasar Luar Biasa Kuncup Bunga Denpasar. There were 18 students which is divided into two groups: group-1 is senam penguins and group-2 is senam ria anak Indonesia. Data were obtained after a six-week training analyzed by paired t-test and unpaired t-test.The results of physical fitness test before training with Senam Penguins was 23.39 ± 1.69 and the test results after the training was 27.87 ± 3.28. the physical fitness test in group-2 before training with Senam Ria anak Indonesia was 22.69 ± 1.76 and the results after training was 24.64 ± 2.11. Paired t-test in both group before and after training showed significant differences after training in group-1 and group-2 with a value of p > 0.05 with an increase in physical fitness test results of (19.15) in group-1 and (8.64) in group-2. The percentage increase physical fitness test results of group-1 with senam penguins is higher than those in group-2 with senam ria anak Indonesia.So we can conclude that the training of senam penguin in Sekolah Dasar Luar Biasa Kuncup Bunga Denpasar improved the physical fitness test results higher than senam ria anak Indonesia.

ABSTRACTHeadache can be caused by increased tension in upper trapezius muscle. This headache is characterized bytrigger point and upper trapezius muscle tautband. The purpose of this study was to compare the positional releasetechnique and infrared interventions with contract relax stretching and infrared on the reduction of headache due toupper trapezius muscle tension. This study was experimental with pre and post-test group design designs of 24 peopledivided into two groups. Group 1 received positional release technique and infrared, while Group 2 received contractrelax stretching and infrared. The result of data analysis with paired sample t-test in Group 1 showed the average 2,242± 0,329 and p = 0,001, while Group 2 showed the average 2,225 ± 0,201 and p = 0,001. From the results of the analysisshowed in each group there is a significant reduction of pain. Based on independent samples test t-test between groups1 and 2 obtained p value = 0.882 (p> 0.05). It can be concluded that the positional release technique and infraredintervention has no significant difference as contract relax stretching and infrared to decreased headache due to uppertrapezius muscle tension.Keywords: Headache, upper trapezius muscle, positional release technique, contract relax stretching, infrared, VAS

Background: Rheumatoid arthritis (RA) a chronic autoimmune inflammatory disease and Methotrexate being widely used as monotherapy and in combination for the treatment of RA. Combination therapy provides additional or even more potentiating effects, therefore have been widely used. This study is aimed at comparing the efficacy and safety of Methotrexate either alone or in combination.Methods: This study was conducted for one year from 1st January to 31st December 2015. 52 patients diagnosed with RA were included in this study and were divided in two groups, Group-1 (n=29) patients received methotrexate (MTX) 7.5-15mg/week and Group-2 (n=23) patients received methotrexate 7.5mg once a week + Hydroxychloroquine (HCQ) 200mg twice daily. The patients were followed up for a period of 16 weeks. Statistical analysis was done by using graph pad insta and p value of <0.05 was considered significant.Results: A total of 52 patients were included in the study. The mean age of patients was 50.53±14.81 years. 43 (82.69%) patients were females and 9 (17.30%) were males. 23 (44.23%) patients had positive family history of the disease. The Rheumatoid factor was positive in 42 (80.70%) and anti-CCP was raised in 50 (96.15%) patients. The RAPID-3 and DAS-28 scores on intragroup comparison was highly significant (p <0.0001) and the intergroup comparison was significant (p <0.01) between baseline and 16 weeks in both the groups. The improvement in inflammatory marker CRP was highly significant on intragroup (p <0.0001) and intergroup (p<0.001) comparison between baseline and 16 weeks in both the groups. The mean changes in Vitamin-D3 between baseline and at 16 weeks were insignificant (p >0.05) in both the groups. Total of Adverse drug reactions reported were 42 (50.60%) in group-1 and 41 (49.40%) in group-2.Conclusions: The combination therapy was found to be more efficacious than MTX monotherapy in improving symptoms and quality of life. The adverse drug reactions were mild, more in combination therapy and none of the patients required withdrawal of treatment.

Background: Footwork training is important in coaching basic movements in badminton. This research aims to learn about the influence of footwork training on improving leg muscles strength. Methods: The study used a randomized pretest-posttest control group design. The participants consisted of 42 male badminton players of Universitas Pendidikan Ganesha, Bali, Indonesia, who split into two groups of footwork training (ten repetitions two sets (group 1) and five repetitions with four sets (group 2)). The coach gave the footwork exercise three times per week for 6 weeks to each group and measured the leg muscle strength using the dynamometer test. Results: The result of the within-group paired T-test before and after the training shows group 1 and group 2 (p<0.05). For group 1, the mean of leg muscle strength was 114.19±33.13 kg before the training and 183.19±33.56 kg after the training, with an increase of 60% (69 kg). On the other hand, for group 2, the mean score of leg muscle strength before the training was 113.05±31.30 kg and after the training was 141.10±34.91, with an increase of 25% (28 kg). The leg muscle strength difference between the groups before and after the training was tested using the independent T-test with a significant α=0.05. Comparing the differences of leg muscle strength for both the groups before the training was p= 0.90, and after the training was p=0.00. Conclusion: The conclusion is training badminton footwork ten repetition two sets better than five repetitions four sets to improve leg muscle strength.

Abstract Objective: To compare the dentofacial effects of mandibular incisor intrusion using mini-implants with those of a conventional incisor intrusion mechanic, the utility arch. Materials and Methods: Twenty-six deep-bite patients were enrolled to one of the two groups. In group 1 the mandibular incisors were intruded using a 0.16 × 0.22–inch stainless-steel segmental wire connected to two mini-implants. In group 2 the mandibular incisor intrusion was performed using a conventional utility arch. Conventional lateral cephalometric radiographs were taken at pretreatment and at the end of intrusion. Thirty landmarks were identified to measure 23 linear and 20 angular measurements. Intragroup comparisons were made using a paired t-test or a Wilcoxon test. Intergroup comparisons were made using a Student's t-test or a Mann-Whitney U-test. Results: The duration of intrusion was 5 months for group 1 and 4 months for group 2. In the implant group, the mean amount of change was 0.4 mm/mo for the incisor tip and 0.3 mm/mo for the center of resistance, and in the utility arch group, the mean amount of change was 0.25 mm/mo for the incisor tip and 0.2 mm/mo for the center of resistance. The mandibular incisors showed an average protrusion of 7° in the implant group and 8° in the utility arch group. Conclusions: Incisor intrusion that was achieved using an implant-supported segmented archwire was no different than the movement achieved with a conventional intrusion utility arch. The only difference between the two methods was in the molar movement.

Football is a sport that is played outside the room of 11 players with the aim of getting the ball into the opponent's goal. The results of observations in the field that there are still many players in passing while competing so that it is not in accordance with the target. The formulation of the problem in this study is whether there is an effect of wallpass training and exercise with groups of 4 to increase the passing results of Bali United Mataram. The purpose of this research is to find out. The research design was a twogroup pretest-postest design. Population and sample are all players totaling 22 people. Instrument Passing for 30 seconds. The research data were analyzed using SPSS version 22. The results of the t-test for group 1 obtained significant values (2-tailed) <0.05: 0.003 <0.05 and group 2 significant values (2-tailed) <0.05: 0.038 <0,05. So these results indicate a significant difference. Based on the test for the difference in the mean post-test data for groups 1 and 2, it was obtained a significant value (2-tailed) <0.05: 0.026 <0.05 with an increase in the percentage value in group 1 of 12.93% and group 2 of 3, 96%. Based on the results of the analysis, it can be concluded that the effect of wallpass training and exercise with groups of 4 is an increase in the passing results of Bali United Mataram in 2021.Keywords: Passing, Wallpass, with groups of 4

ABSTRACT Objective: To compare posttreatment and postretention occlusal changes with the physiologic occlusal changes caused by natural development of untreated subjects. Materials and Methods: The sample was divided into three groups. Group 1 comprised 97 subjects treated with four premolar extractions at a mean pretreatment (T0) age of 13.03 years, a mean posttreatment (T1, first observation) age of 15.12 years, and a mean postretention (T2, second observation) age of 20.52 years. The mean observation period (T2-T1) was 5.39 years. Group 2 comprised 58 subjects treated nonextraction at a mean pretreatment age of 12.83 years, a mean posttreatment age of 14.99 years, a mean postretention age of 20.22 years, and a mean observation period of 5.22 years. Group 3 comprised 114 untreated subjects at a mean age at T1 of 14.91 years and at T2 of 20.48 years. The mean observation period was 5.56 years. Dental casts were evaluated using the Peer Assessment Rating (PAR) index and the Little irregularity index in maxillary and mandibular arches. Changes in PAR and Little indexes were compared among the three groups by analysis of variance and Tukey tests. Results: Intergroup comparison showed that at T1 and T2 the treated groups presented smaller PAR and Little indexes than the untreated group. In the observation period, the treated groups showed greater increase in PAR and Little maxillary indexes than the untreated group. The extraction group showed a greater increase of the Little mandibular index than the untreated group. Conclusions: The treated groups showed more changes according to PAR and Little maxillary indexes than the untreated group. The posttreatment change of the mandibular anterior crowding of the treated extraction group was greater than the mandibular crowding caused by physiologic changes in the untreated group.

Background. The present study aimed to assess the clinical and radiographic effect of strontium ranelate and metal-substituted hydroxyapatite as bone graft materials on treating chronic periodontitis among diabetes mellitus patients. Methods. A randomized split-mouth study was conducted on 20 sites in 10 controlled type II diabetic patients suffering from chronic periodontitis. After phase I therapy, the sites were randomly allocated by a computer-generated table into two groups. Group 1: A mucoperiosteal flap was elevated in 10 sites, followed by the placement of strontium ranelate mixed with Gengigel. Group 2: A mucoperiosteal flap was elevated in 10 opposite sites, followed by the placement of metal-substituted hydroxyapatite mixed with Gengigel. Clinical parameters were assessed at baseline and 3- and 6-month intervals. Cone-beam computed tomography (CBCT) was used at baseline and after six months to assess bone gain. Results. The two treatment modalities resulted in a statistically significant reduction in clinical parameters at the 3- and 6-month intervals compared to the mean baseline value. Intergroup comparison showed a significant reduction in probing pocket depth and clinical attachment loss in group 1 compared to group 2. Comparison of the two sides by CBCT showed a significant increase in the alveolar bone height in the SR group than the metal-substituted hydroxyapatite group. Conclusion. Clinical and radiographic results showed a significant improvement in the two groups and provided evidence that strontium ranelate is promising in treating periodontal diseases.

ABSTRACT Background and objectives The primary goal of periodontal therapy is to restore the tooth supporting tissues lost due to periodontal disease. The aim of the present study was to compare the efficacy of combination of GTR membrane and alloplastc bone graft with open flap debridement (OFD) in treatment of periodontal intrabony defects. Methods Twenty paired intrabony defects were surgically treated using split mouth design. The defects were randomly assigned to treatment with OFD, GTR membrane + bone graft (test) or OFD alone (control). The clinical efficacy of two treatment modalities was evaluated at 6 months postoperatively by clinical, radiographical parameters. The measurements included probing pocket depth (PD), clinical attachment level (CAL), gingival recession (GR), bone fill (BF), bone density (BD). Results The mean reduction in PD at 0 to 6 months was 3.20 ± 0.82 mm and CAL gain of 3.10 ± 1.51 mm occurred in the GTR membrane + bone graft (test) group; corresponding values for OFD (control) were 2.10 ± 0.63 mm and 1.90 ± 0.57 mm. Similar pattern of improvement was observed when radiographically postoperative evaluation was made. All improvement in different parameters was statistically significant (p < 0.01). Conclusion Treatment with a combination of collagen membrane and bone graft led to a significantly more favorable clinical outcome in intrabony defects as compared to open flap debridement alone.  Objectives: Automatic devices based on oscillometric principle are widely used for the estimation of blood pressure (BP). Mercury sphygmomanometer mean systolic BP (MSBP) and its derived cuff pressure are the traditional mode of estimation which is a validated and authenticated procedure. Automated machines using oscillometric method are slowly replacing the conventional technique. This study was done to compare the BP recorded by the mercury sphygmomanometer MSBP and the automated technique using oscillometric method automated office BP (AOBP).Methods: Two hundred subjects aged 40–65 years with mid-arm circumference 27–34 cm were recruited. MSBP and AOBP were recorded adhering to guidelines given by the American Heart Association Joint National Committee. The subjects were divided into two groups as normotensive (Group 1) and hypertensive (Group 2), and statistical analysis was performed.Results: The mean systolic and diastolic pressures estimated by oscillometric method and sphygmomanometer were calculated and compared with each other by paired t-test separately for Groups 1 and 2. In mormotensives (Group 1), the mean systolic pressure MSBP was 114.21±7.5 mmHg and AOBP was 118.24±11.0 mmHg. The mean diastolic MSBP was 72.1±3.5 mmHg and AOBP was 76.4±1.2 mmHg. Subjects of Group 2 (hypertensive) showed mean systolic pressure MSBP of 144.42±18.5 mm Hg and AOBP of 159.74±22 mmHg. The mean diastolic MSBP was 87.2±9.5 mmHg and AOBP was 96.9±9.2 mmHg. In Group 1, the difference was statistically not significant, while in Group 2, comparison by paired t-test showed a mean difference of systolic pressure by 15.32±1.25 (p<0.005), and mean difference of diastolic pressure by 8.9±5.8 (p<0.005).Conclusion: Hence, we conclude that oscillometric pressure recordings by the automated device are closer to manual measurements in normotensives but showed a significant high value in hypertensives. We suggest caution in treating hypertensives with values derived from automated machines only.
