Large Canal of Nuck Hernia: The Female Equivalent of the Inguinoscrotal Hernia

Our patient was a 78-year-old woman with a large indirect inguinal hernia who underwent Lichtenstein repair. Literature review of the pathophysiology of indirect hernias revealed important discussion of the canal of Nuck in women and understanding embryologic development of ovaries and descent of testes.

Aim: To compare the outcome of Desarda`s technique with that of Lichtenstein mesh hernioplasty in terms of chronic pain, recurrence and infection. Methods: A prospective comparative study with randomized controlled trial was conducted at Lahore General Hospital in Surgical Department to appraise the outcome of Desarda Hernioplasty in comparison with Lichtenstein Hernioplasty technique to evaluate recurrence, wound infection and chronic groin pain. The Desarda repair is used to treat inguinal hernia without the use of mesh. Results: Total sixty (n=60) patients were included in the study by dividing into two groups Group A and B with mean age 40.5 and 39.5 years for Desarda vs Lichtenstein groups correspondingly. Insignificant statistical difference was noted in both groups regarding wound infection but considerable statistical advantage was noted regarding recurrence and Chronic groin pain for Group A patients in comparison to Group B. Conclusion: We concluded that Desarda repair is emerging technique and cost effective with lesser pain, infection and recurrence than other techniques. The canal of Nuck is analogous to a patent processus vaginalis in a male, which normally loses its communication to the peritoneal cavity within the first year of life. Failure of obliteration of this tract can result in a hydrocele. We present a case of 38 years old lady referred to our surgical unit from Gynecologist as left obstructed femoral hernia. A fluid filled sac (Encysted Hydrocele of Canal of Nück) with small indirect inguinal hernia.

In women, the round ligament is attached to the uterus near the origin of the fallopian tube, and a small evagination of parietal peritoneum accompanies the round ligament through the inguinal canal to the labium majorum. This small evagination of parietal peritoneum, named the canal of Nuck in women, is the equivalent of the processus vaginalis in men. Incomplete obliteration of the processus vaginalis causes indirect inguinal hernia or hydrocele of the canal of Nuck, a very rare condition in women. Here, we report a case of ovary-containing hernia of the canal of Nuck that was diagnosed with ultrasonography and was surgically confirmed. Gray scale and color Doppler ultrasonographic features studied were: the site and the size of the hernia, the texture of the hernia contents and the presence or absence of blood flow in the hernia contents.J Enam Med Col 2017; 7(1): 39-41
