Introduction. Glaucoma is one of the most significant eye diseases. It is often diagnosed, not always amenable to therapy, and can lead to a complete loss of visual functions. In recent years, the method of osteopathic correction has become widespread as one of the effective methods of treatment and rehabilitation of patients with pathologies of various body systems. In the pathogenesis of glaucoma, it is customary to distinguish a dystrophic concept, which considers primary open-angle glaucoma as a result of dystrophic changes in the connective tissue, as well as in the endothelial lining of the trabeculae and Schlemm′s canal, especially destructive changes in mitochondria and the alteration of their functional activity. A vascular concept is also distinguished. According to this concept, the central link in the pathogenesis of glaucoma is circulatory disorder in the ciliary vessels, ocular artery, and major vessels of the head and neck, it can be assumed that osteopathic correction in the treatment of patients with open-angle glaucoma will be pathogenetically substantiated and will have a positive effect on intraocular pressure and trophicity of the optic nerve. The goal of research — to study the influence of in osteopathic correction on the nature of unoperated glaucoma (stage IIA) and to substantiate the possibility of using osteopathic correction in the complex treatment of patients with this pathology.Materials and methods. A prospective controlled randomized study was conducted at 52 city polyclinics, branch 3, Moscow, from January 2018 to January 2019. 40 patients (70 eyes) aged 50 to 75 years with primary open-angle glaucoma IIA stage were examined. At this stage of the disease, patients most often seek medical care and the issue of conservative management is primarily considered. All patients were divided into two groups of 20 people: the main group and the control group. The treatment in the main group included hypotensive drug therapy and osteopathic correction. Patients of the control group received only drug therapy. All patients underwent ophthalmic (visometry, tonometry, perimetry) and osteopathic examination twice: before the treatment and after 3 months.Results. For patients with primary open-angle IIA non-operated glaucoma, regional (most often regions of the head, neck, dura mater) and local (abdominal diaphragm, iliac bones, hip and knee joints) somatic dysfunctions were the most typical. In the main group a statistically significant decrease in the frequency and severity of dysfunctions at all levels was stated. Also, in patients receiving osteopathic correction, a significant decrease in the level of intraocular pressure and perimetric indices was noted. In patients of the control group, no reliable changes in these indicators were obtained.Conclusion. The results obtained indicate that osteopathic correction is clinically effective in the complex treatment of patients with primary open-angle II A glaucoma.

A case report on a 49-year-old female with diagnoses of ocular hypertension in her left eye (LE) treated with 250 mg/day acetazolamide for 2 years. During the slit-lamp examination, complete occlusion of both iridocorneal angles was detected. Intraocular pressure (IOP) was 10 and 35 mmHg in the right eye and LE, respectively. Phacotrabeculectomy was performed in the LE. After 1 month of the procedure, the patient developed a slowly progressive miopization from −1 to −3 diopters (D) the following months. Approximately 3 months after surgery, the patient developed an episode of acute pain, athalamia, and IOP 45 mmHg in her LE. Late-onset malignant glaucoma was suspected and the patient was treated with topical hypotensive and cycloplegic agent until a prompt vitrectomy was performed. Deepening of the anterior chamber and restoration of IOP to normal range was obtained after surgery.

Objective: The objective of the study was to compare the posture-induced intraocular pressure (IOP) changes in primary angle-closure glaucoma (PACG) with or without glaucoma medications, and healthy control eyes with normal IOPs in Indian subjects. Materials and Methods: The IOP was measured in the sitting position and the supine position after 10, 20, and 30 min with a rebound tonometer. Results: Twenty-five patients with PACG and 30 controls with normal IOPs were studied. The IOP in the sitting position measured with the rebound tonometer was 13.8 + 3.2 mm Hg in eyes with PAC, and 12.9 + 2.9 mm Hg in eyes with normal IOPs. The IOP increased to 14.4 mm Hg, 16.8 mm Hg, and 18.9 mm Hg at 10 min, 20 min, and 30 min in PACG subjects. In normal age-matched controls, the IOP increased to 13.4 mm Hg, 14.9 mm Hg, and 17.8 mm Hg at 10 min, 20 min, and 30 min, respectively, but none of these differences were significant (P = 0.09; P = 0.08, P = 0.08). The mean postural IOP change from baseline was also not significant between the two groups. Only three patients were on single antiglaucoma medication with well-controlled IOP in the PACG group. Conclusions: Postural IOP changes are comparable among eyes with PACG with and without glaucoma medications, and control eyes.

Актуальность. Патологическая агрегация γ-синуклеина, одного из членов семейства синуклеинов, ассоциирована не только с болезнью Паркинсона, боковым амиотрофическим склерозом, но и с развитием глаукомы. Данный белок, являясь синаптическим, участвует в процессе везикулярного транспорта и высвобождении медиаторов, в частности дофамина. Цель. Обоснование возможности прогнозирования нарушения функции γ-синуклеина по влиянию регуляторов дофаминергической системы на уровень ВГД. Материал и методы. У 15 мышей (30 глаз) синактивированным геном, кодирующим γ-синуклеин (γ-KO) из лаборатории V. Buchman (TheJackson Laboratory – B6.129P2-Sncgtm1Vlb/J и008843) и у 20 контрольных мышей дикого типа (40глаз) того же возраста (2,5-3,5 месяцев) проводили измерение внутриглазного давления (ВГД) до и после однократной инстилляции по 10 мкл в оба глазарегуляторов дофаминергической системы (мелатонина 0,1%, Галоперидола 0,2%, дофамина 10%).Мышам проводили измерение ВГД через каждые30 минут в течение двух часов. Для каждого препарата-регулятора формировалась отдельная группа мышей. Результаты. У мышей γ-KO выявлено снижение ВГД в течение 2 часов более, чем на 4-5 ммрт. ст в ответ на инстилляцию мелатонина 0,1%(р<0,03), Галоперидола 0,2% (р<0,03) и дофамина10% (р<0,03) по сравнению с мышами дикого типа, где наблюдалось незначительное снижение уровня ВГД соответствующих регуляторов. Выводы. Предложенный способ измерения ВГД после инстилляции препаратов, влияющих на метаболизм дофамина, позволяет выявлять и прогнозировать развитие недостаточности функции γ-синуклеина и его влияние на регуляцию ВГД и формировать группы риска для динамического наблюдения и выбора превентивных лечебных мероприятий.

The effect of increased intraoperative intraocular pressure on blood flow in the central retinal artery and posterior short ciliary arteries during phacoemulsification

Актуальность. Принципом факоэмульсификации является поддержание баланса ирригационно-аспирационных потоков. Баланс достигается за счет различных систем подачи жидкости, позволяющих хирургу задавать и поддерживать постоянный уровень ВГД во время операции. Вопрос об оптимальном безопасном уровне ВГД во время вмешательства на сегодняшний день остается открытым. Цель. Оценить влияние повышенного уровня интраоперационного офтальмотонуса на скорость кровотока в центральной артерии, вене сетчатки, и задних коротких цилиарных артериях. Материал и методы. В исследование было включено 29 пациентов с начальной катарактой. Факоэмульсификация выполнялась на приборе Alcon Centurion vision system на предустановленном интраоперационном уровне ВГД 60 мм.рт.ст. Скорость кровотока измеряли путем дуплексного сканирования в режиме ЦДК трёхкратно: до вскрытия глазного яблока, интраоперационно на уровне ВГД (58.77±8.28 мм.рт. ст) и сразу после герметизации доступа и нормализации ВГД. Каждое измерение сопровождалось контролем ВГД и АД. Результаты. При значении уровня ВГД 58.77±8.28 мм.рт. ст в ЦАС и ЗКЦА отмечалось статистически достоверное снижение максимальной систолической скорости кровотока. С 12,62±3,07 до 9,93±2,77 см/с и снижение конечной диастолической скорости кровотока с 3,94±1,09 до 1,79±1,64 см/с. В 11 из 29 (37,9%) случаев скорость кровотока в ЦАС в диастолическую фазу не регистрировалась. Максимальная систолическая скорость кровотока в ЗКЦЛА снизилась на 22%. Конечная диастолическая снизилась с 5,11±1,83 до 2,97±1,27 см/с. Аналогично, максимальная систолическая скорость кровотока с медиальной стороны упала с 12,37±2,74 до 9,50±1,68 см/с, а конечная диастолическая скорость кровотока снизилась с 4,54±1,35 до 2,73± 0,91 см/с. Скорость кровотока в ЦВС менялась незначительно и не зависела от уровня ВГД. Выводы. На уровне ВГД 58.77±8.28 мм. рт. ст. у человека отсутствуют компенсаторные механизмы ауторегуляции глазного кровотока в ответ на резкое повышение интраоперационного уровня ВГД, вплоть до полного прекращения кровотока в ЦАС в диастолическую фазу, что может приводить к ишемии тканей сетчатки.
