A 79-year-old female with a history of keratoconjunctivitis sicca presented with several years of epiphora of both eyes. Thirteen years earlier, intracanalicular Herrick lacrimal plugs (Lacrimedics, Eastsound, WA, USA) had been placed in both eyes to treat her dry eye syndrome. After 13 years the patient felt the epiphora was intolerable and underwent endoscopic dacryocystorhinostomy (DCR) of the left, then the right side. Intraoperatively, during the right endoscopic DCR, a Herrick lacrimal plug was found in the common canaliculus into the lacrimal sac. Postoperatively, the patient did well with improved epiphora. The Herrick plug is designed to be intracanalicular, and this case illustrates that the plug can migrate and be retained for many years. Collared punctal plugs have a lower risk of this type of complication.

Purpose. To report a case of severe corneal thinning secondary to dry eye treated with a tectonic Descemet stripping automated lamellar keratoplasty (DSAEK) and amniotic membrane graft.Methods. A 72-year-old man with a history of long standing diabetes mellitus type 2 and dry eye presented with 80% corneal thinning and edema on the right eye and no signs of infectious disease, initially managed with topical unpreserved lubrication and 20% autologous serum drops. Eight weeks after, the defect advanced in size and depth until Descemetocele was formed. Thereafter, he underwent DSAEK for tectonic purposes. One month after the procedure, the posterior lamellar graft was well adhered but a 4 mm epithelial defect was still present. A multilayered amniotic membrane graft was then performed.Results. Ocular surface healed quickly and reepithelization occurred over a 2-week period. Eight months after, the ocular surface remained stable and structurally adequate.Conclusion. Tectonic DSAEK in conjunction with multilayered amniotic graft may not only provide structural support and avoid corneal perforation, but may also promote reepithelization and ocular surface healing and decrease concomitant inflammation.

AbstractOBJECTIVE. The aim of the study was to evaluate the endoscopic dacryocystorhinostomy (DCR) with some particularities and its advantages over external DCR. MATERIAL AND METHODS. We evaluated 3 female patients (age range 35-84 years) diagnosed with chronic dacryocystitis - lacrimal sac abscess (2 cases) and lacrimal sac fistula (1 case). All patients presented a history of repeated episodes of acute dacryocystitis initially treated with broad spectrum antibiotics by the ophthalmologist. The evaluation protocol consisted in cranio-facial CT scan or MRI for the evaluation of the lacrimal sac and nasal structures, an ophthalmologic examination with the catheterization of the superior and inferior lacrimal punctum with a light probe to verify the permeability of the lacrimal pathway, and the Jones test with fluorescein to receive information about the common lacrimal path. Lacrimal syringing with regurgitation of fluid from the opposite punctum was the only criteria to decide the surgery. In all three cases we performed an endoscopic DCR, with stent insertion in two cases and a laco-dacryo-rhinostomy in one case. In only one case a reintervention was needed 12 months later. CONCLUSION. The endoscopic DCR, with its different particularities, is a safe surgical procedure with a low rate of complications, being the treatment of choice for the treatment of nasolacrimal duct obstruction.

Abstract Background This study aims to explore a case of exophytic nasal papilloma with acute dacryocystitis as the first symptom. Case presentation A 72-year-old male patient complaining of “a 10-year history of tearing and purulent discharge from the right eye, with subsequent redness and pain in the inner canthus for three days” was initially diagnosed with acute dacryocystitis of the right eye. The patient was treated with anti-inflammatory therapy. However, the redness and swelling of the inner canthus continued to increase. An endoscopic dacryocystorhinostomy of the right eye was performed under general anesthesia. A large amount of purulent secretion was drained during the operation. As a result, the swelling of the inner canthus was significantly reduced. A routine intra-operative biopsy of the wall of the lacrimal sac revealed an exophytic nasal papilloma. A second biopsy, 1 week after the surgery, revealed the same result. The patient was advised to undergo a dacryocystectomy once the swelling had subsided. However, the patient was reluctant to undergo this surgery and remains under clinical observation. Conclusion It is rare for an exophytic nasal papilloma, which is a benign tumor in the lacrimal sac, which has the potential for recurrence and malignant transformation, to manifest with acute dacryocystitis as the first symptom. Therefore, this case report could provide a reference for the future clinical diagnosis of this disease.

The article analyzes the solidarity of “Sabot ul – ojizin's” work with the present period, which took an important place in the history of Uzbek enlightenment in the second half of the XVII century and the beginning of the XVIII century in Central Asia, the major representative of the Naqshbandian sect, Sofi Allahyar's "Sabot ul – ojizin". The work of sohfi Allahyar "Sabot ul - ojizin" is a work created due to the spiritual need of his time. The main purpose of the creation of the work is to educate the perfect person, to strive for the perfection of the individual. It is up to the people to start them on the right path by revealing the Enlightenment of the Islamic religion, to encourage them not to fall under the influence of the ideas of the memorization of different currents and fanatic groups. In the article, the work studied the socio – philosophical views aimed at starting the people on the right path, and in turn revealed that at that time for material benefit, he was struggling with enlightenment, occupying the minds of the common people and distributing various superstitious teachings. The article analyzes the ideas put forward in the work" Sabot ul – ojizin", the philosophical views, the solidarity of such enlightened views as leading the people towards perfection with today's times.

Background: Rhinosporidiosis is a chronic granulomatous disease. Aim of the study was to do a clinicopathological evaluation of the cases of rhinosporidiosis. Study design was longitudinal.Methods: The study was undertaken in MKCG medical college. 151 cases of rhinosporidiosis were taken. Pedunculated or sessile, fleshy, polypoidal, reddish looking mass with spores on the surface were clinically diagnosed as rhinosporidiosis. Age, sex distribution, site of presentation and socioeconomic status was studied. The swellings of lacrimal sac were doughy in consistency. The conjunctival mass was excised and thermocautery applied. For the cases of lacrimal sac dacryocystectomy was done.Results: The cases were more common in male children of low socioeconomic status. Palpebral conjunctiva is the common site of involvement. History of pond bath was present in almost all the cases. History of bleeding from nose and eye was present in many cases.Conclusions: Rhinosporidiosis is a common condition of our locality. Treatment is simple by excising the growth and cauterising the site. Taking bath in pond water is supposed to be the cause of the disease. The disease can be prevented by avoiding pond bath.

This chapter is devoted to the history of the law of marriage. The formation of marriage was for many centuries a matter for the Church and its law. In medieval times marriage was held to be a sacrament and indissoluble. Divorce a vinculo matrimonii meant a decree of nullity, not dissolution. Divorce a mensa et thoro, or judicial separation, was available on grounds of misconduct, but the parties were not free to remarry. Bastardy, the status of children born outside marriage, was also for the canon law. The second part of the chapter goes into the common law of coverture, the status of married women, and the slow progress towards giving wives the right to own property and make contracts. It ends with the piecemeal reforms of divorce law, following the establishment of a secular divorce court in 1857.

Purpose Few studies have focused on the intranasal localization of the lacrimal sac during endoscopic dacryocystorhinostomy: landmarks in order to find the medial wall of the lacrimal sac have been described, but there is a lack of description of methods for the verification of the complete marsupialization of the lacrimal sac during surgery. In this report, we propose an easy and effective method for certain intraoperative identification of lacrimal sac. Methods A method in order to verify the effective marsupialization of the lacrimal sac is applied and described: to ensure that the opening of the sac in the nasal cavity is complete, the surgeon should identify the Rosenmuller valve, which is the end of the common canaliculus in the lacrimal sac. Continuous irrigation with saline solution through the inferior canaliculus can be useful to obtain a clean surgical area and to permit easy intraoperative identification of the valve. Results Between 2007 and 2015, 193 endoscopic dacryocystorhinostomies were performed in our institutions. Postoperative surgical success at last follow-up (minimum 12 months) was 93.8% (181 out of 193 of cases). No major complications were observed. Conclusions Correct and complete exposure of the lacrimal sac during surgery is crucial for a good outcome: when the opening of the common canaliculus is identified, the surgeon is assured that the sac has been correctly and completely marsupialized inside the nasal cavity.

The historiography of the Peasants' War is currently dominated by Peter Blickle's The Revolution of 1525. Blickle builds upon Günther Franz's Der deutsche Bauernkrieg, long the standard history of the Peasants' War. Blickle uses the concept of “the Revolution of the Common Man” to describe the Peasants' War. The common men in both the towns and villages were united in a revolutionary effort to establish the “communal Reformation.” Blickle writes: “Evangelical doctrine gripped urban and rural communities alike, and the lay community claimed the right to decide right doctrine.… Thus, the Reformation's dependence on community erased the barriers between urban and rural communities, between burghers and peasants.”
